General practices and community care points work hand in hand in the care of multimorbid patients: What are the advantages? - A qualitative study with general practitioners and medical practice assistants
CONCLUSION: Cooperation between general practices and community care points has the potential to improve care for multimorbid patients and reduce the workload burden on general practices. Measuring transitional patient safety: Adaptation and validation of the German version of the Care Transitions Measure
CONCLUSIONS: With a validated and standardized German version of the CTM, an instrument is now available to assess the quality and safety of the discharge process from the patients' perspective.
