Unmarried status effect on stage at presentation and treatment patterns in primary urethral carcinoma patients
CONCLUSIONS: Unmarried female PUC patients more likely harbored locally advanced stage at presentation. Conversely, unmarried male PUC patients are less likely to benefit from bi-/trimodal therapy.PMID:38320935 | DOI:10.1016/j.urolonc.2024.01.022 (Source: Urologic Oncology)

Germline variants in early and late-onset Brazilian prostate cancer patients
CONCLUSIONS: Our findings contribute to a deeper understanding of the genetic factors associated with PCa susceptibility in different age groups, especially among the Brazilian population. This is the first investigation to explore germline variants specifically in younger Brazilian PCa patients, with high relevance given the genetic diversity of the population in Brazil. Additionally, our work presents evidence of functionally deleterious germline variants within the KMT2C gene among Brazilian PCa patients. The identification of novel and functionally significant variants in the KMT2C gene emphasizes its potential role in...

Standardization of the evaluation and surveillance of patients with BCG unresponsive high grade non-muscle invasive bladder cancer clinical trials
Urol Oncol. 2024 Feb 1:S1078-1439(24)00020-6. doi: 10.1016/j.urolonc.2024.01.019. Online ahead of print.ABSTRACTThere are multiple ongoing and planned clinical trials that are evaluating novel therapies to treat patients with BCG-unresponsive high grade nonmuscle invasive bladder cancer (NMIBC). Importantly, there is considerable variation in surveillance strategies between these clinical trials, specifically with regards to the use of advanced imaging, enhanced cystoscopy, and mandatory biopsies, which could impact landmark efficacy assessments of investigational agents. To present guideline recommendations for the standa...

Time to progression is the main predictor of survival in patients with high-risk nonmuscle invasive bladder cancer: Results from a machine learning-based analysis of a large multi-institutional database
CONCLUSION: Time to progression is the main predictor of OS in patients with high risk NMIBC treated with BCG and might be considered a coprimary endpoint. In addition, models including time to progression could be considered for patients' stratification in clinical practice and at the time of clinical trials design.PMID:38302296 | DOI:10.1016/j.urolonc.2024.01.001 (Source: Urologic Oncology)

Urologist-level variation in the management of T1a renal cell carcinoma: A population-based cohort study
CONCLUSIONS: Significant variation exists in the management of small renal masses and appears to be driven primarily by urologist preference and practice patterns. Our findings emphasize the need for unified guidance regarding management of these masses to reduce unwarranted variation in care.PMID:38278631 | DOI:10.1016/j.urolonc.2024.01.011 (Source: Urologic Oncology)

Expression of stem cell markers as predictors of therapeutic response in metastatic prostate cancer patients
CONCLUSION: Combination therapy involving chemotherapy and ADT appears to have a greater impact on suppressing CSCs compared to ADT alone. These findings highlight the potential of targeting CSCs as a prognostic and predictive marker therapeutic strategy in metastatic PCA.PMID:38278632 | DOI:10.1016/j.urolonc.2023.12.012 (Source: Urologic Oncology)

Patient experience and unmet needs in high-risk nonmuscle-invasive bladder cancer: Insights from qualitative interviews and a cross-sectional survey
CONCLUSIONS: Most HR-NMIBC patients were satisfied overall with BCG treatment. Approximately half of the patients had stopped BCG treatment, notably, most during the induction phase, suggesting nonadherence to guidelines which recommend maintenance treatment after induction. Future treatments should focus on delaying or avoiding recurrence and cystectomy while reducing patient discomfort and discontinuation prior to completing the recommended course of treatment.PMID:38272755 | DOI:10.1016/j.urolonc.2024.01.013 (Source: Urologic Oncology)

Prognostic impact of variant histology in bladder cancer: Would early and aggressive treatment shift the paradigm?
CONCLUSION: We confirmed that VH presents a more aggressive clinical course compared to PUC. An earlier radical treatment within the NMIBC setting could increase the oncological outcomes of the VH patients.PMID:38267300 | DOI:10.1016/j.urolonc.2024.01.009 (Source: Urologic Oncology)

Impact of variant histology on upstaging and survival in patients with nonmuscle invasive bladder cancer undergoing radical cystectomy
CONCLUSION: Patients with VH NMIBC undergoing RC are at significantly higher risk of upstaging at RC when compared to patients with pure urothelial NMIBC and have worse overall survival. While this study supports the concept of an aggressive treatment approach for patients with VH NMIBC, improvements in understanding of the disease are necessary to improve outcomes.PMID:38267301 | DOI:10.1016/j.urolonc.2023.12.008 (Source: Urologic Oncology)

Referral patterns and genetic testing outcomes in a contemporary hereditary renal cancer clinic
CONCLUSIONS: These findings are useful for referring physicians and patients in determining patient referral to hereditary cancer clinics, and for counseling patients undergoing genetic testing. Data from non-Caucasian patients and evolving implications of variants of unclear significance (VUS) may represent future research directions for hereditary renal cancer clinics.PMID:38267302 | DOI:10.1016/j.urolonc.2023.12.010 (Source: Urologic Oncology)

The association of body mass index with tumor aggression among men undergoing radical prostatectomy
CONCLUSIONS: Increased BMI is associated with adverse pathology in PZ tumors. TZ adverse pathology risk may be increased among obese men with GG1 or GG2 disease, which has implications for future studies assessing behavioral change among men whose tumors are actively monitored.PMID:38262868 | DOI:10.1016/j.urolonc.2023.12.013 (Source: Urologic Oncology)
