Hospital and departmental level strategies for managing the impact of adverse events on surgeons
Urol Oncol. 2024 Mar 20:S1078-1439(24)00347-8. doi: 10.1016/j.urolonc.2024.03.002. Online ahead of print.ABSTRACTAdverse events have a profound impact on surgeons. This impact extends to physical and mental health, psychological wellbeing, and professional performance. Surgeons are ill prepared for these consequences of adverse events and are under-supported when they inevitably occur. Here we review the data on how adverse events affect surgeons. We explore the efforts made to date to both prepare surgeons before and support them after such events and we make recommendations regarding how this should and could be done bet...

Prostatic adenocarcinoma: molecular underpinnings and treatment-related options
Urol Oncol. 2024 Mar 19:S1078-1439(24)00348-X. doi: 10.1016/j.urolonc.2024.03.003. Online ahead of print.ABSTRACTProstate cancer is heterogeneous with varied pathologic features and presents with a wide spectrum of clinical manifestations from indolent to advanced cancer. Interrogation of the molecular landscape of prostate cancer has unveiled the complex genomic alterations in these tumors, which significantly impacts tumor biology. The documented array of chromosomal alterations, gene fusions, and epigenetic changes not only play a crucial role in oncogenesis and disease progression, but also impacts response and resista...

The assessment of bladder cancer resectability with bimanual palpation: A prospective study in laparoscopy and open cystectomy patients
CONCLUSIONS: The identification of limited bladder mobility during preoperative EUA yielded prognostic information on surgical margin status. Our findings suggest that EUA has the potential to provide valuable insights in the assessment of bladder resectability. However, further research in a larger cohort of patients is warranted to validate and expand on these findings.PMID:38508941 | DOI:10.1016/j.urolonc.2024.02.006 (Source: Urologic Oncology)

Genitourinary toxicity in patients receiving TURP prior to hypofractionated radiotherapy for clinically localized prostate cancer: A scoping review
CONCLUSION: For those who have undergone prior TURP hypofractionated radiotherapy may increase the risk of late urinary toxicity, particularly hematuria. Those with persisting bladder dysfunction following TURP are at greatest risk and careful management of these men is required. Close collaboration between urologists and radiation oncologists is recommended to discuss the management of patients with residual baseline bladder dysfunction prior to commencing hypofractionated radiotherapy.PMID:38503591 | DOI:10.1016/j.urolonc.2024.02.011 (Source: Urologic Oncology)

Real-world use, dose intensity, and adherence to enfortumab vedotin in locally advanced or metastatic urothelial cancer
CONCLUSIONS: Among patients with advanced urothelial cancer treated with EV monotherapy in contemporary practice, EV dosing frequency, and dosage was lower in clinical practice than recommended in the product labeling. Further research is required to understand clinical factors and outcomes associated with the differences observed.PMID:38503592 | DOI:10.1016/j.urolonc.2024.03.001 (Source: Urologic Oncology)

Contemporary risk of biochemical recurrence after radical prostatectomy in the active surveillance era
CONCLUSIONS: Overall BCR risk has increased in the AS era, driven by a higher risk case mix and increased BCR risk among high-risk patients. Physicians should quote contemporary estimates of BCR when counseling patients.PMID:38490923 | DOI:10.1016/j.urolonc.2024.02.010 (Source: Urologic Oncology)

Ductal, intraductal, and cribriform carcinoma of the prostate: Molecular characteristics and clinical management
Urol Oncol. 2024 Mar 13:S1078-1439(24)00053-X. doi: 10.1016/j.urolonc.2024.01.037. Online ahead of print.ABSTRACTProstatic acinar adenocarcinoma accounts for approximately 95% of prostate cancer (CaP) cases. The remaining 5% of histologic subtypes of CaP are known to be more aggressive and have recently garnered substantial attention. These histologic subtypes - namely, prostatic ductal adenocarcinoma (PDA), intraductal carcinoma of the prostate (IDC-P), and cribriform carcinoma of the prostate (CC-P) - typically exhibit distinct growth characteristics, genomic features, and unique oncologic outcomes. For example, PTEN mut...

Different diagnostic strategies combining prostate health index and magnetic resonance imaging for predicting prostate cancer: A multicentre study
CONCLUSIONS: The PHI-MRI model was superior to PHI and MRI alone. It may reduce the number of biopsies and ensure the detection rate of CsCaP under an appropriate sensitivity at the cost of an increased number of MRI scans.PMID:38480077 | DOI:10.1016/j.urolonc.2024.02.009 (Source: Urologic Oncology)

Efficacy of cytoreductive radical cystectomy in metastatic urothelial bladder cancer based on site and number of metastases
CONCLUSION: These findings demonstrate an OS benefit of CRC with multiagent chemotherapy and pinpoint multiple visceral metastases as a potential contraindication for CRC. Although limited by the influence of unmeasured confounders, these findings may inform future prospective investigations into CRC.PMID:38480078 | DOI:10.1016/j.urolonc.2024.02.007 (Source: Urologic Oncology)
