Priapism associated to zuclopentixol and  the choice for an alternative
Tijdschr Psychiatr. 2024;66(1):42-45.ABSTRACTA 29-year-old man developed priapism following the (re)administration of zuclopentixol. In the previous days, a significant amount of alcohol was consumed, presumably in combination with amphetamine and cannabis. Priapism is a rare but serious side effect of various psychoactive medications and recreational drugs, leading to permanent loss of erectile function if not treated in time. In this case the side effect was discovered in a late stage, at which curative treatment was no longer viable. A clear guideline for choosing an alternative antipsychotic agent is currently lacking,...

Catatonia in youth with developmental disabilities: challenges in recognition and  management
We describe a 14-year-old boy with an intellectually disability and autism spectrum disorder (ASD) in which clinicians did not diagnose catatonia until 1 year after the development of symptoms. Moreover, hesitations surrounding the correct treatment led to its delayed initiation. With this case report we aim to contribute to reduced reluctance and increased alertness in the treatment of catatonia in adolescents with developmental disorders.PMID:38380488 (Source: Tijdschrift voor Psychiatrie)

The value of re-evaluation and thorough family history taking for the diagnostic work-up of chorea
Tijdschr Psychiatr. 2024;66(1):51-54.ABSTRACTThe differential diagnosis of chorea encompasses a broad range of disorders. In psychiatry, tardive dyskinesia may be difficult to discern from other causes, particularly when the family history is negative. A 59-year-old man with an unclear medical history had been using risperidone for over a decade when we first saw him. He presented with severe dyskinesia in all extremities. The family history for neuropsychiatric disorders was negative. We interpreted the movement disorder as tardive dyskinesia, but later he turned out to suffer from Huntington’s disease. To impro...

The revised version of the Y-BOCS: responsivity and other psychometric  properties
CONCLUSION: The Y-BOCS-II severity scale is a reliable and valid instrument for accurately assessing the severity of OCD symptoms and for measuring treatment-induced change. This second version also has clinical and psychometric advantages over the YBOCS-I. When these findings are sufficiently replicated, use of the YBOCS-II as the new common standard seems recommendable.PMID:38380483 (Source: Tijdschrift voor Psychiatrie)

The relationship of mental health providers with health insurers; an  exploration
CONCLUSION: With the supported content of the vision document, there is to some extent shared governance. The change steps (shared innovation) considered desirable will be promoted by partly granting the intended benefits to each other (shared savings).PMID:38380484 (Source: Tijdschrift voor Psychiatrie)
