Use of triptans during pregnancy? With caution!
We report cases, registered in the French national pharmacovigilance database, of fetal death and intrauterine growth retardation in women exposed to triptans during pregnancy. Triptans have vasoconstrictive properties and we wonder about their responsibility for these side effects. The use of triptans and other drugs exhibiting vasoconstrictive properties in pregnant women requires a careful benefits/risks evaluation. [Evaluation and impact of "pregnancy" pharmaceutical pictograms among 281 women]. Therapie. 2019 Nov 25;: Authors: Bertrand D, Piedeloup I, Mattoug S, Liabeuf S, Gras-Champel V Abstract In France, since 2017 new pharmaceutical pictograms appeared on drugs with a potentieal risk during pregnancy. CONCLUSION: Our results report significant and worrying misuse of transmucosal fentanyl formulations with wide off-label use and also primary dependence on fentanyl, regardless of galenic formulation, in patients treated for chronic non cancer pain. Given the significant risks of fentanyl, it is necessary to continue the monitoring of misuse, in particular, thanks to the activities of the French Addictovigilance network allowing a multisource approach and who provides information concerning cases of abuse, misuse and dependence. Genital tract and reproductive characteristics in daughters of women and men prenatally exposed to diethylstilbestrol (DES).
CONCLUSION: These results must be considered as preliminary, due to the small numbers of patients, limited follow-up duration after birth due to young age of the studied population, and observational methods. An important point is that the high risk of reproductive dysfunction of women prenatally exposed to DES was not observed in their daughters. There is a signal on the high incidence of uterine defects, especially aplastic uterus, and its possible link with DES exposure through epigenetic effects is discussed in our findings. Inconsistent findings regarding pregnancy outcomes in the third generation are worthy of furthe...

Polypharmacy: A general review of definitions, descriptions and determinants.
Abstract Polypharmacy is considered as the administration of many drugs. It is a major public health concern, which is growing worldwide. The identification of polypharmacy relies on drug count on a given time window. Polypharmacy exists if this count exceeds a predefined threshold. Although there is no consensus among scientists, five is the most frequently used number. Depending on the time-windows, polypharmacy can be qualified as simultaneous, cumulative, or continuous. Drugs can be selected according to the duration or the recurrence of their use thereby introducing the concept of chronic polypharmacy. This g...

Liquid formulation of ifosfamide increased risk of encephalopathy: A case-control study in a pediatric population.
M, Simon C, Triquet L, Valnet-Rabier MB, Veyrac G, Faillie JL, Zenut MC Abstract BACKGROUND: Several clusters of encephalopathy occurred after the market change from Holoxan® (ifosfamide lyophilized powder) to Ifosfamide EG® (liquid formulation) and justified a formal survey in 2015. In June 2016, the regulatory authority decided to apply a precautionary measure in reducing the shelf life of Ifosfamide EG® at 7 months. One-year study from spontaneous reports lead to suspect a potential residual risk. Due to the many limitations associated with spontaneous notifications, we performed a multicentri...

Pharmacovigilance - The next chapter. Therapie. 2019 Oct 01;: Authors: Moore N, Berdaï D, Blin P, Droz C Abstract The discovery and quantification of adverse drug reactions has long relied on the careful analysis of spontaneously reported cases. In the absence of more specific information sources, these have resulted in the discovery of many new adverse reactions, alteri...

A capture-recapture method for estimating the incidence of off-label prescriptions: the example of baclofen for alcohol use disorder in France.
ier S Abstract The local/regional incidence of off-label prescriptions can be difficult to estimate. Capture-recapture models can be used to indirectly estimate population sizes. Here, we used a capture-recapture model to estimate the number of patients treated off-label with baclofen for alcohol use disorder in northern France in 2013. Three capture sources were used: (i) the active case file at the region's largest Addiction Unit, (ii) the regional pharmacovigilance centre, and (iii) a sample of community pharmacies. After between-source overlaps had been identified, we used a log-linear model to produced eight ...

French translation and linguistic validation of the Raynaud's condition score.
Abstract The Raynaud's condition score is a 11-point scale severity score used in Raynaud's phenomenon clinical trials since 1998. The Raynaud's condition score diary has been recommended for use in clinical trials assessing efficacy of interventions on scleroderma related Raynaud's phenomenon. However, this score has never been formally validated in French. We thus performed a translation and a linguistic validation of the Raynaud's condition score through a forward/backward translations process followed by an expert review and cognitive patient interviews. The translations led to a French version of the Raynaud'...

Interest of pharmacoepidemiology for pharmacodynamics and analysis of the mechanism of action of drugs.
Abstract Pharmacology is often divided in separate branches, such as molecular and cellular pharmacology, pharmacokinetics, pharmacodynamics, experimental and/or preclinical pharmacology, clinical pharmacology (and therapeutics), pharmacogenetics, pharmacogenomics, pharmacovigilance, pharmacoepidemiology, pharmacoeconomics… This enumeration gives a global picture of different scientific areas, which are however dealing with the same question. Another mindset should be a global interactive and continuous approach, which could be designed as "human pharmacology". An original and attractive way to il...

Interest of pharmacoepidemiology for the study of psychotropic drugs.
Abstract In psychiatry, drug evaluation using pharmacoepidemiological methods has been of growing interest in recent decades. Studies based on observational databases are particularly useful for psychotropic drugs due to their important prevalence in populations, and their use over long period. The authors discussed the specific interest of pharmacoepidemiological studies in the field of psychiatry through two examples: first, the use of antidepressants, and, second, the risks associated with antipsychotics. Pharmacoepidemiology in older people: Purposes and future directions.
Abstract Knowledge of the benefit/risk ratio of drugs in older adults is essential to optimise medication use. While randomised controlled trials are fundamental to the process of drug development and bringing new drugs to the market, they often exclude older adults, especially those suffering from frailty, multimorbidity and/or receiving polypharmacy. Therefore, it is generally unknown whether the benefits and harms of drugs established through pre-marketing clinical trials are translatable to the real-word population of older adults. Pharmacoepidemiology can provide real-world data on drug utilisation and drug e...

Abstract The evaluation of the real-life benefits and risks of statins in population is a major pharmacoepidemiological issue, given their widespread use for cardiovascular prevention. The purpose of this review was not to be exhaustive but to show the contributions of pharmacoepidemiology for various aspects of the evaluation of statins such as real-life drug use, effectiveness and risk. Statins are among the most used drugs in the world, but recent data show a slight decrease in use.
