Commentary: Re-consult surgery for patients with lung cancer? The role of resection after initial nonoperative therapy
Although the role of surgery for early and even more locally advanced non –small cell lung cancer (NSCLC) stages is generally well defined, the use of resection in patients for whom operative intervention was not initially considered appropriate is significantly less clear. Surgeons historically have considered attempts at resection when patients had persistent or recur rent disease after their primary nonoperative therapy (so-called “salvage surgery”) to not only be associated with greater risk but also unclear oncologic benefits. Simulation-Based Design of Bicuspidization of the Aortic Valve
Severe congenital aortic valve pathology in the growing patient remains a challenging clinical scenario. Bicuspidization of the diseased aortic valve has proven to be a promising repair technique with acceptable durability. However, most understanding of the procedure is empirical and retrospective. This work seeks to design the optimal gross morphology associated with surgical bicuspidization with simulations, based on the hypothesis that modifications to the free edge length cause or relieve stenosis. Transcatheter aortic valve replacement (TAVR) has rapidly increased in the United States with 98,504 commercial cases listed for 2022 in the Society of Thoracic Surgeons/American College of Cardiology Transcatheter Valve Therapy registry (Figure  1). The American Heart Association/American College of Cardiology 2022 guidelines suggest that surgical aortic valve replacement (SAVR) is still preferred for those ≤65 years of age or with a life span expected of at least 20 years. Commentary: Reverse Potts shunt: The answer in a high-pressure situation?
Advances in the medical therapy of patients with pulmonary arterial hypertension have resulted in a significant increase in survival over the last 2  decades.1 However, patients who do not respond to medical therapy have few options other than lung transplantation. Myocardial Bridges in a Pediatric Population: Outcomes Following a Standardized Approach
Pediatric patients with myocardial bridge can present with myocardial ischemia and sudden cardiac arrest. Provocative stress tests and intracoronary hemodynamics can risk-stratify symptomatic patients. Surgical repair can be effective in mitigating symptoms and abnormal functional tests.
