Outcomes After Pseudomonas Prosthetic Joint Infections
CONCLUSION: Pseudomonas infections often present as polymicrobial PJIs but are difficult to eradicate in either polymicrobial or monomicrobial setting. A review of the current literature on Pseudomonas PJI reveals favorable infection clearance rates (63 to 80%) after DAIR while infection clearance rates (33 to 83%) vary widely after two-stage revision.PMID:38354412 | DOI:10.5435/JAAOS-D-23-00704 (Source: The Journal of the American Academy of Orthopaedic Surgeons)

Infection Rates After Aseptic Revision Total Hip Arthroplasty With Extended Oral Antibiotic Prophylaxis
This study compared PJI rates in aseptic R-THA performed with EOAP with PJI rates in published studies of aseptic R-THA patients not receiving EOAP.METHODS: Prospectively documented data on 127 consecutive aseptic R-THAs were retrospectively reviewed. Evidence-based perioperative infection prevention protocols were used, and all patients were discharged on 7-day EOAP. Superficial and deep infections at 30 and 90 days postoperatively and at mean latest follow-up of 27.8 months were statistically compared with all US studies reporting the prevalence of PJI after aseptic R-THA. Complications related to EOAP within 120 days of...

For Surgically Treated Geriatric Acetabular Fractures, Longer Duration between Admission and Surgery Is Associated with an Increase in Post-operative Complications: A Study Using the National Trauma Data Bank
DISCUSSION: In this database study, time from hospital admission to surgery was associated with a notable increase in inpatient complications but not inpatient mortality after adjusting for potentially confounding variables while age and mCCI were associated with both mortality and complications. Additional research is needed to determine the relationship between time to surgery with longer term mortality and complications and to assess causality.LEVEL OF EVIDENCE: Prognostic Level III.PMID:38335144 | DOI:10.5435/JAAOS-D-23-00511 (Source: The Journal of the American Academy of Orthopaedic Surgeons)

Thermal Damage in Orthopaedics
J Am Acad Orthop Surg. 2024 Feb 8. doi: 10.5435/JAAOS-D-23-00838. Online ahead of print.ABSTRACTThere are numerous potential sources of thermal damage encountered in orthopaedic surgery. An understanding of the preclinical mechanisms of thermal damage in tissues is necessary to minimize iatrogenic injuries and use these mechanisms therapeutically. Heat generation is a phenomenon that can be used to a surgeon's benefit, most commonly for hemostasis and local control of tumors. It is simultaneously one of the most dangerous by-products of orthopaedic techniques as a result of burring, drilling, cementation, and electrocauter...

Risk of Subsequent Fusion After Isolated Decompression of Lumbar Facet Cysts
CONCLUSION: Patients undergoing isolated decompression for lumbar facet cysts undergo subsequent lumbar fusion at a 5-year rate of 6.79%. Risk factors for subsequent decompression include chronic kidney disease, hypertension, and osteoarthritis. This study will assist spine surgeons in appropriately counseling patients on expected postoperative course and potential risks of isolated decompression.PMID:38320287 | DOI:10.5435/JAAOS-D-23-00765 (Source: The Journal of the American Academy of Orthopaedic Surgeons)

Osteochondritis Dissecans Lesions of the Knee: Evidence-Based Treatment
J Am Acad Orthop Surg. 2024 Jan 30. doi: 10.5435/JAAOS-D-23-00494. Online ahead of print.ABSTRACTOsteochondritis dissecans (OCD) of the knee is a potentially disabling condition in adolescent and young adults, which is likely multifactorial in origin. In recent years, there have been notable improvements in identification and treatment. Clinical presentation varies based mostly on OCD lesion stability. Patients with stable lesions generally present with vague knee pain and altered gait while mechanical symptoms and effusion are more common with unstable lesions. Lesions most commonly occur on the lateral aspect of the medi...

Subtle Syndesmotic Instability
J Am Acad Orthop Surg. 2024 Jan 30. doi: 10.5435/JAAOS-D-23-00707. Online ahead of print.ABSTRACTLatent or subtle syndesmotic instability is defined as an injury to the syndesmosis which is not apparent on static radiographs of the ankle. Syndesmotic injuries have also been referred to as high ankle sprains. Injury to the syndesmosis typically occurs with collision sports and often involves an external rotation force to the ankle. Diagnosis can be delayed because of negative initial imaging studies. Physical examination tests including the external rotation test, proximal squeeze test, and fibular shuck test can assist in ...

Academy of Orthopaedic Surgeons Technology Overview Summary: Concentrated Bone Marrow Aspirate for Knee Osteoarthritis
J Am Acad Orthop Surg. 2024 Jan 30. doi: 10.5435/JAAOS-D-23-00957. Online ahead of print.ABSTRACTThe Platelet-Rich Plasma (PRP) for Knee Osteoarthritis Technology Overview is based on a systematic review of current scientific and clinical research. Through analysis of the current best evidence, this technology overview seeks to evaluate the efficacy of PRP for patients with knee osteoarthritis. The systematic literature review resulted in 54 articles: 36 high-quality and 18 moderate-quality. The findings of these studies were summarized to present findings on PRP versus control/placebo, acetaminophen, non-steroidal anti-in...

The Impact of Socioeconomic Status on the Presence of Advance Care Planning Documents in Patients With Acute Cervical Spinal Cord Injury
CONCLUSION: Female patients and those from at-risk communities are markedly less likely to complete ACP. Attention to possible disparities during admission and ACP discussions may help ensure that patients of all backgrounds have treatment goals documented.PMID:38271675 | DOI:10.5435/JAAOS-D-23-00763 (Source: The Journal of the American Academy of Orthopaedic Surgeons)

Anterior Vertebral Body Tethering: A Review of the Available Evidence
J Am Acad Orthop Surg. 2024 Jan 23. doi: 10.5435/JAAOS-D-23-00312. Online ahead of print.ABSTRACTIdiopathic scoliosis is a complex three-dimensional deformity of the spine with anterior overgrowth (hypokyphosis), coronal curvature, and axial rotation. Scoliosis treatment in the skeletally immature spine is therapeutically challenging because of growth and was commonly limited to observation, bracing treatment, or fusion. Fusion accomplishes powerful deformity correction at the expense of future growth and mobility of the involved segments, increasing the risk of adjacent segment degeneration and intervertebral disk disease...
