Vitamin E –doped cross-linked polyethylene (VEPE) has encouraged the use of larger heads in thinner liners in total hip arthroplasty (THA). However, there are concerns about wear and mechanical failure of the thin liner, especially when metal heads are used. The aim of this randomized controlled trial was t o investigate if the use of a large metal head in thin VEPE liner would increase polyethylene wear compared with a standard 32-mm metal head and to compare periacetabular radiolucencies and patient-reported outcomes in THA. We enjoyed reading the recently published work by Wenzel et  al on the comparison of ultracongruent (UC) with posterior-stabilized (PS) and cruciate-retaining (CR) polyethylene inserts in total knee arthroplasty (TKA) [1]. The main highlight of this study was that it was perhaps the first meta-analysis to compare the kinematics of UC and CR inserts, and for this we congratulate the authors. Letter to the Editor Commenting on “Obesity is Associated With Greater Improvement in Patient-Reported Outcomes Following Primary Total Knee Arthroplasty”
Bosler and colleagues conducted a retrospective single-site cohort study of the prognostic significance of preoperative body mass index (BMI) measures obtained on persons undergoing primary knee arthroplasty (KA) [1]. The prognostic interest was in prediction of 1 to 5-year postoperative pain and functional status as well as change from baseline to postoperative follow-up pain and functional status. As the authors note, single-site retrospective studies like the one conducted by Bosler et  al have limited generalizability compared to multisite prospective studies [2] and in particular, compared to systematic reviews [3,4]...
