The Association Between Oral Bone Mineral Density –Reducing Medications and the Risk of 2-Year Implant-Related Complications Following Total Knee Arthroplasty
Certain medications interfere with the bone remodeling process and may potentially increase the risk of complications after total knee arthroplasty (TKA). As patients undergoing TKA may be taking these bone mineral density (BMD) –reducing medications, it is unclear as to whether and which medications impact TKA outcomes. Therefore, the purpose of this study was to observe the impact of various BMD-reducing medications on 2-year implant-related complications following TKA. (Source: The Journal of Arthroplasty)

Goals of Care Discussions in Orthopaedic Surgery: Geriatric Hip Fractures
Due to the severity of hip fracture complications in the geriatric population, including major morbidity and mortality, it is crucial to establish treatment goals and manage expectations as a patient-centered multidisciplinary team. Goals of care (GOC) are personalized treatment programs designed to align with the individual patient ’s values and preferences. There is a paucity of literature on the topic of GOC discussions, especially regarding orthopaedic injuries. Therefore, this narrative review aims to provide an account of GOC discussions related to geriatric hip fractures. (Source: The Journal of Arthroplasty)

Outcomes Following Total Hip Arthroplasty in Patients Who Have Von Willebrand Disease Depend on Postoperative Anticoagulation
Von Willebrand disease (VWD) is the most common congenital bleeding disorder. This autosomal dominant condition arises from quantitative or qualitative defects of Von Willebrand factor. To our knowledge, this study leveraged a national database to characterize the largest VWD cohort of total hip arthroplasty (THA) patients to date, assessing 90-day postoperative adverse events and 5-year revision-free survival. How Do Spinopelvic Characteristics Change Post-Total Hip Arthroplasty? A Longitudinal Assessment Raising Awareness of the Postoperative Period
This study aimed to: 1) investigate the change in spinopelvic parameters at 7 days (early) and 1-year post-THA; and 2) identify patient characteristics associated with a change in SS of more than 7 ° post-THA. (Source: The Journal of Arthroplasty)

Goals of Care: A Patient-Centered Assessment
As patients age, their ability to make decisions about medical care becomes more challenging. This is especially challenging when serious illnesses or mental illnesses, such as dementia or delirium, impede the decision-making process. Goals of care refer to the process of making medical decisions that address the clinical situation, patient values, and goals prior to discussing a specific medical treatment or surgical intervention. These conversations are more successful if they begin with a shared understanding of the medical condition and possible outcomes, followed by a discussion of patient-specific values and desired ...

Intraosseous Versus Intravenous Vancomycin in Tourniquetless Primary Total Knee Arthroplasty: A Randomized Trial
Intraosseous (IO) administration of vancomycin at the time of total knee arthroplasty (TKA) has been shown to be safer and more effective than intravenous (IV) administration at preventing early periprosthetic joint infection. Previous studies have relied on tourniquet inflation to enhance local tissue concentrations and mitigate systemic release. (Source: The Journal of Arthroplasty)

The Impact of Mental Health on Patient Outcomes After Total Knee and Hip Arthroplasty
Mental health plays a key role in how we perceive and interact with the world around us. It affects a person ’s outlook on life, how they address challenges, and how they respond to the good and bad that occur in daily life. Mental health and successful total joint arthroplasty are intimately connected. A patient who has normal mental health is more likely to use fewer narcotics, have greater improvement s in clinical outcomes early in the post-operative period, and maintain those outcomes as time goes on.[1,2] There is growing interest in patient mental health and their perceptions after total joint arthroplasty.[3–6]...

Goals of Care: A Patient Centered Assessment
Providing timely, compassionate care for geriatric hip fracture patients is challenging. Continued efforts should be taken to encourage communication at key points of patient care to better discuss prognosis and recovery that focus on patients ’ goals and values. We should never take any fractures for granted as if “this is easy, I have treated this one before,” since no two fractures are exactly the same. The scenarios as well as the patients’ preferences vary, so a patient-centered assessment of their goals of care is essential. (Source: The Journal of Arthroplasty)

Cementless Total Knee Arthroplasty: A Resurgence —Who, When, Where, and How?
Total knee arthroplasty (TKA) is one of the most common procedures in orthopaedics, but there is still debate over the optimal fixation method for long-term durability: cement versus cementless bone ingrowth. Recent improvements in implant materials and technology have offered the possibility of cementless TKA to change clinical practice with durable, stable biological fixation of the implants, improved operative efficiency, and optimal long-term results, particularly in younger and more active patients. Risk Factors for Return to the Emergency Department and Readmission After Same-Day Discharge Total Joint Arthroplasty
Same day discharge (SDD) after total joint arthroplasty (TJA) is safe and cost effective. However, benefits may be offset by the potential cost of emergency department (ED) visits and readmissions. We identified risk factors for return to the ED and readmission in patients who underwent SDD and inpatient (IP) stays after TJA. (Source: The Journal of Arthroplasty)
