Prognostic Impact of Previous Atherosclerotic Cardiovascular Disease on Short- and Long-Term Outcomes in Acute Myocardial Infarction
Patients with previous atherosclerotic cardiovascular disease (ASCVD) are typically managed by secondary prevention modalities, but they may experience recurrent events. In acute myocardial infarction (MI), the prognostic effect of pre-existing ASCVD on short- and long-term outcomes remains uncertain. This retrospective multicenter registry included 2475 patients with acute MI who had undergone percutaneous coronary intervention. Previous ASCVD was defined as a history of ischemic events in the coronary, cerebral, and peripheral arterial territories. (Source: The American Journal of Cardiology)

Cerebral Embolic Protection in Valve-in-Valve TAVR
In patients with degenerated aortic bioprosthesis, valve-in-valve transcatheter aortic valve replacement (ViV-TAVR) has largely replaced the conventional reoperative surgical aortic valve replacement (SAVR).1 The Sentinel cerebral protection device (CPD) designed to capture and remove debris from the procedure received FDA approval in the USA in 2017 and is aimed at preventing post-procedural stroke.2 We analyze the impact of CPD use on outcomes of patients undergoing ViV-TAVR from a national, all-comers database. Prolonged Dual Antiplatelet Therapy in Acute Myocardial Infarction Patients without Revascularization: China Acute Myocardial Infarction (CAMI) Registry Study
At least 12-month dual antiplatelet therapy (DAPT) is one of the standards of care following Percutaneous Coronary Intervention (PCI) in patients with acute coronary syndrome. However, study on prolonged DAPT for acute myocardial infarction (AMI) patients without revascularization is limited. We studied 1744 AMI patients without revascularization from the China Acute Myocardial Infarction registry between January 2013 and September 2014. These patients were on DAPT and did not experience AMI, stroke, or bleeding events at 12-month follow-up. (Source: The American Journal of Cardiology)

How do the flow components of coronary flow reserve (CFR) change after aortic valve replacement?
To twist a lyric from the song “Dem [Dry] Bones”, we might summarize our purview as “valve bone connected to the heart bone” – technically the coupling between a stenotic aortic valve and myocardial function. In a practical sense, how does blood flow to the myocardium change in response to relief from severe aortic sten osis (AS)? The important contribution in this issue of the American Journal of Cardiology from investigators in Cádiz, Spain,1 provides new data to integrate into a unified hypothesis and help answer this question. REGROUPing on the importance of complete revascularization in patients undergoing coronary artery bypass grafting
Complete revascularization (CR) is widely recognized as the goal of revascularization with either percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG) for multivessel coronary disease. However, there has been limited robust prospective randomized data to support this objective, outside of patients with ST segment elevation myocardial infarction (STEMI). The data for revascularization of all anatomically significant non-culprit lesions after primary PCI of the culprit in STEMI patients is compelling. (Source: The American Journal of Cardiology)

Sex-Related Outcomes of Transcatheter Aortic Valve Implantation with Self-Expanding or Balloon-Expandable Valves: Insights from the OPERA-TAVI Registry
This study sought to evaluate the impact of sex on the treatment effect of Evolut-PRO/PRO+ (PRO) or Sapien 3 Ultra (ULTRA) devices on clinical outcomes. Comparative Analysis of Evolut PRO vs Sapien 3 Ultra Valves for Transfemoral Transcatheter Aortic Valve Implantation (OPERA-TAVI) is a multicenter multinational registry including patients undergoing latest-iteration PRO or ULTRA implantation. The Chronic Dilemma of Chronic Total Occlusion Revascularization
A chronic total occlusion (CTO) is encountered in up to 20% of patients with significant coronary artery disease (CAD) on coronary angiography and is a marker of advanced CAD and cardiovascular morbidity.(1) Several studies have evaluated surgical(2) and percutaneous revascularization in patients with CTO.(3) Percutaneous CTO revascularization of coronary vessels (CTO-PCI) remains a major challenge for interventional cardiologists, due to higher complexity, lower success rates and more frequent complications compared to revascularization of non-chronically occluded coronary vessels.(4) Notably, the last decade has witnesse...

Classification of the Urgency of the Procedure and Outcome of Acute Type A Aortic Dissection
Surgery for type A aortic dissection (TAAD) is associated with high risk of early mortality. The prognostic impact of a new classification of the urgency of the procedure was evaluated in this multicenter cohort study.Data on consecutive patients who underwent surgery for acute TAAD were retrospectively collected in the multicenter, retrospective European Registry of TAAD (ERTAAD).The rates of in-hospital mortality of 3902 consecutive patients increased along with the ERTAAD procedure urgency grades: urgent procedure, 10.0%; emergency procedure grade 1, 13.3%; emergency procedure grade 2, 22.1%; salvage procedure grade 1, ...

Intracardiac Versus Transesophageal Echocardiography Guided Percutaneous Debulking of Tricuspid Endocarditis
There has been growing interest in percutaneous debulking for aspirating large right-sided valvular vegetations.1 This approach aims to achieve several objectives: debulking septic intracardiac masses, reducing the infectious burden, and establishing clinical stability, all while adopting a less invasive approach and avoiding surgical valve replacement. This is particularly valuable for this vulnerable cohort of patients who are more likely return to intravenous drug use with consequent risk of prosthetic valve endocarditis. Real-World Comparison of Clopidogrel with Ticagrelor and Prasugrel In Patients with Chronic Coronary Disease Undergoing Atherectomy
Current societal guidelines reserve potent P2Y12 platelet receptor inhibitors (ticagrelor, prasugrel) for acute coronary syndrome (ACS) cases due to a paucity of evidence for their use in chronic coronary disease (CCD).1 Patients with CCD undergoing percutaneous coronary intervention (PCI) with adjunctive atherectomy represent a subset of patients with more complex coronary disease who may derive benefit from more potent platelet inhibition.2 Atherectomy is associated with an increased risk of microvascular injury, endothelial dysfunction, and platelet activation, which may contribute to the increased risk of thrombosis, m...

Association of Myocardial Injury and Outcomes in Patients hospitalized with COVID-19
Coronavirus disease (COVID-19), caused by the outbreak of severe acute respiratory syndrome coronavirus (SARS-CoV-2) has emerged as a major cause of morbidity and mortality globally.(1) Although, primarily a respiratory virus, symptoms of SARS-CoV-2 infection can be non-specific. Cardiovascular manifestations of COVID-19 are poorly understood but can include cardiac arrest, myocardial infarction (MI), arrhythmias, and myocarditis.(2) Myocardial injury in the form of troponin elevation is frequently seen in the clinical setting in hospitalized patients with COVID-19 and can range from demand ischemia to overt MI.(3) Through...

Subcutaneous versus Transvenous Implantable Defibrillators: a Systematic Review and Meta-Analysis
In patients with complex anatomy, high infection risk and venous access issues, subcutaneous implantable cardioverter-defibrillator (S-ICD) is presumed to be an appealing alternative to transvenous implantable cardioverter-defibrillator (T-ICD). T-ICD is known to cause perioperative and long-term complications. S-ICD does not offer anti-tachycardia pacing (ATP), back up pacing and chronic resynchronization therapy (CRT) but does have expanding indications. However, owing to0 ongoing uncertainty in efficacy and ambiguity in utility of S-ICD in low-risk patients, we performed this updated meta-analysis to study device relate...
