Anal trauma caused by bull-horn injury
ConclusionsThe main treatment of bull-horn injuries is extensive surgical debridement, antibiotic therapy, and lavage of the area. In cases involving the  anal sphincter, primary sphincteroplasty is recommended. The modern trend does not include the systematic performance of a colostomy however, it has been described in cases with catastrophic wounds and urological lesions associated. Microbiome-preserving antibiotics for the treatment of Clostridioides difficile infection: a systematic review and meta-analysis
ConclusionsFidaxomicin (in seven studies) demonstrated significant improvement in achieving sustained clinical cure. A limitation of this study may that more studies are needed to compare fidaxomicin with other antibiotics. (Source: Techniques in Coloproctology)

Major low anterior resection syndrome has equivalent health-related quality of life implications as having a permanent colostomy
ConclusionsPostoperative bowel dysfunction significantly impacts health-related quality of life. Patients with ‘major LARS’ have health-related quality of life as poor as those following APR. This requires consideration when counselling patients on postoperative health-related quality of life, especially where poor postoperative bowel function is anticipated following restorative surgery. Safety, feasibility, and short-term-outcome of anal endoscopic submucosal dissection for anal intraepithelial neoplasia: an option for focal lesions?
ConclusionsAnal endoscopic submucosal dissection seems to be a safe and feasible treatment for AIN. Recurrences are seldom and can be treated again with the same method. Nevertheless, indications for resection in comparison to radiofrequency ablation, pharmacological therapy, and watch-and-wait strategy are still unclear.Trial registrationEthics commission of Salzburg, Austria, EK-Nr. 1056/2023.  Keywords: Endoscopic submucosal dissection, anal intraepithelial neoplasia, anal cancer  (Source: Techniques in Coloproctology)

Association of radiotherapy for prostate cancer and second primary colorectal cancer: a US population-based analysis

Distalization of perianal fistulas after loose silicone seton drainage is a myth
ConclusionsNo statistically significant distalization of complex fistula tracts after loose silicone seton drainage was found. Some complex fistulas may downgrade to a less complex fistula after long-term seton drainage. However, loose silicone seton drainage should not be offered to patients as a treatment option to downgrade a complex fistula to a simple one or even have the hope to heal it. Efferent limb stimulation prior to loop ileostomy closure: a systematic review and  meta-analysis
ConclusionsPPS prior to stoma closure may reduce LOS and postoperative complications albeit without a demonstrable beneficial effect on POI. Further high-powered studies are required to confirm or refute these findings. (Source: Techniques in Coloproctology)

Cutting seton for the treatment of cryptoglandular fistula-in-ano: a systematic review and meta-analysis
ConclusionsOverall, this analysis shows that CS has comparable recurrence and incontinence rates  to other modalities. However, this may be at the expense of more postoperative pain and extended healing time. Further comparative studies between CS and other modalities are warranted. Surgical outcomes of minimally invasive trephine surgery for pilonidal sinus disease with and without laser therapy: a comparative study
ConclusionThe incorporation of laser therapy along with trephine/pit excision surgery significantly reduces the recurrence rate in patients with PNS. Further prospective studies are needed to confirm our findings. (Source: Techniques in Coloproctology)
