Occam versus hickam
A 23-year-old man presented with new onset horizontal diplopia 5 months after a left orbital floor fracture. Examination revealed bilateral abduction deficits and disc swelling. Urgent MRI and MRI showed no significant abnormalities in the CNS. Lumbar puncture revealed a minimally elevated opening pressure and significant leukocytosis. Additional CSF testing revealed probable Lyme meningitis. The patient responded to a course of oral doxycycline, with rapid resolution of his diplopia, abduction deficits, and disc edema. The safety of intracameral phenylephrine – A systematic review
Intracameral phenylephrine is commonly used in ophthalmic surgery as an alternative or supplement to mydriatic eye drops; hence, the importance of an evidence-based understanding of its risk-benefit profile is vital. We performed a comprehensive search in the PubMed, Google Scholar and Cochrane databases for published studies and case reports relating to the use of intracameral phenylephrine. Articles from 1958 to 2021 with the following keywords were used: “intracameral phenylephrine”, “intracameral mydriatics”, “phenylephrine”, “pupil dilation”, “complications”. Laser Treatment for Choroidal Melanoma: Current Concepts
Laser treatment has offered a relatively nonsurgical alternative for eye, life, and vision-sparing treatment of malignant melanoma of the choroid. Historically, the most commonly used forms of lasers were xenon-arc, argon laser, krypton laser, and the more recent transpupillary thermotherapy (TTT) and photodynamic therapy (PDT). Melanomas selected for laser treatment tend to be smaller and visibly accessible, which means these tumors are usually located in the posterior choroid. An eye for an eye, a tooth for a tooth
A 77-year-old man presented with the complaint of a painless decrease in vision in both eyes for one year. He underwent bilateral cataract surgery without improvement. Neuroimaging and genetic testing for mitochondrial disease was negative. Examination was consistent with a diagnosis of toxic-nutritional optic neuropathy. On further questioning the patient admitted chronic ingestion of mouthwash 3 times daily in the past 18 months after completion of dental work. After discontinuation of mouthwash ingestion, visual acuity and fields improved. (Source: Survey of Ophthalmology)

We read with great interest the review article on ocular syphilis published in your journal. 1 Furtado and Smith should be congratulated for the excellent article which has addressed the re-emergence of syphilis and highlighted the varied presentations of ocular syphilis. The article mentions vitritis associated with retinitis and chorioretinitis. Treat-and-Extend Versus Alternate Dosing Strategies with Anti-Vascular Endothelial Growth Factor agents to treat Center Involving Diabetic Macular Edema: A Systematic Review and Meta-analysis of 2346 Eyes
Anti-vascular endothelial growth factor (Anti-VEGF) agents are the standard of care for diabetic macular edema (CI-DME) with vision loss. They are commonly administered using several treatment protocols, including fixed, pro re nata (PRN) and treat-and-extend (T&E) regimens. Because of the lack of evidence defining an ideal treatment paradigm, we systematically compared T&E with fixed or PRN regimens. Switching between anti-VEGF agents in the management of refractory diabetic macular edema: A systematic review
Refractory diabetic macular edema (DME) to monthly intravitreal anti-vascular endothelial growth factor (VEGF) monotherapy has a prevalence of approximately 40% in landmark clinical trials. The prevalence of retinopathy in prediabetes: A systematic review
Prediabetes is defined as suboptimal glycemia that does not reach the threshold for type 2 diabetes 1,4. Worldwide, approximately one in 13 adults (374 million) aged 20-79 years have prediabetes, and the vast majority are unaware of the diagnosis 23.
