[The sacroiliac joint and pain].
Abstract Pain caused by the sacroiliac joint (SIG) makes up a relevant proportion of lumbar back pain and can have a variety of specific and non-specific causes. The SIG represents the central link between the spine and the lower extremity. It is characterized by high stability and low mobility. There are significant inter-individual differences. In addition to the possibility of asymptomatic situations, SIG dysfunction can trigger pain in the lower back with possible radiation to the lower extremity. In addition to a structured medical history, clinical examination requires an examination of the adjacen...

[From external assessment of pain to automated multimodal measurement of pain intensity : Narrative review of state of research and clinical perspectives].
CONCLUSION: Pain is generally recorded multimodally, based on external observation scales. With regard to automated pain recognition and on the basis of the 14 selected studies, there is to date no conclusive evidence that multimodal automated pain recognition is superior to unimodal pain recognition. In the clinical context, multimodal pain recognition could be advantageous, because this approach is more flexible. In the case of one modality not being available, e.g., electrodermal activity in hand burns, the algorithm could use other modalities (video) and thus compensate for missing information. [Pain management in sickle cell disease].
e M Abstract Sickle cell disease is associated with numerous symptoms and complications. Acute painful crisis is the most characteristic manifestation of the disease. In addition, many patients report chronic pain. As both acute and chronic pain severely diminish quality of life, adequate pain management is crucial. Recommendations for the treatment of acute painful crises are based on the World Health Organization analgesic ladder, which has been developed for cancer-related pain. Chronic pain can be treated with basic long-acting opioids and on-demand short-acting opioids. If patients show signs of neuropathic p...

[Is interdisciplinary multimodal pain therapy threatened? : Increasing controversy on the necessity for inpatient treatment].
sellschaft e.V. Abstract Interdisciplinary multimodal pain therapy (IMST) is recognized as a scientifically founded form of therapy for inpatient treatment of chronic pain conditions. The indications are assessed by private practitioners and must be confirmed by physicians at the inpatient institution. The health insurance companies are obliged to remunerate this treatment but are allowed to test the conditions in individual cases. Recently, reports from members of the German Pain Society on a drastic increase in testing and rejection quotas have become more frequent. Therefore, this article discusses the legal fo...

[From why to what for : On the relevance of a philosophical-existentialistic approach in the treatment of patients with chronic pain]. Schmerz. 2020 Mar 18;: Authors: Bozzaro C, Koesling D, Frede U Abstract BACKGROUND: People have the need to find explanations for the events that occur in their lives. This is especially true when it comes to experiences that impact a person's entire existence and endanger their self-esteem and sense of identity. Chronic pain is one such experience. The search for the cause behind such pain is primarily motivated by the expectation that once the cause of the ...

[The effects of cannabis and cannabinoids on anesthesia and analgesia during the perioperative period].
Abstract Cannabis is the most frequently used recreational drug worldwide. Moreover, the use of cannabinoids for medical purposes is also constantly growing and medical cannabis products are legalised by an increasing number of countries. First clinical reports have shown enhanced requirements for propofol and analgesics used for general anesthesia and perioperative pain management in cannabis users. Therefore, this article discusses the potential impact of medical or recreational cannabis use on patients with regard to the recent literature. Besides the significantly increased requirement for propofol and fentany...

[Pathophysiological role of calcitonin gene-related peptide (CGRP) in migraine and cluster headache].
er K Abstract Calcitonin gene-related peptide (CGRP) is released from trigeminal afferents during migraine and cluster headache attacks and can be detected in the jugular plasma. Infusion of CGRP can induce headache attacks in migraine and cluster patients. Inhibition of the CGRP signal system is therapeutic in migraine and cluster headache. CGRP is a potent dilator of intracranial arteries but does not immediately activate the trigeminal pain system. CGRP may act as a signal molecule between different cells in the trigeminal ganglion and enhances nociceptive transmission in the spinal trigeminal nucleus. Per...

This article focuses on pain asymbolia, which was also first described by Schilder, and is currently little known and considered to be rarely encountered. Pain asymbolia is a central impairment of pain experience with no negative affective-emotional component. The basis of Schilder's discovery and the differential diagnosis of pain asymbolia was the detailed examination of eleven medical cases between 1928 and 1930. His publications on the condition are characterized by meticulousness, progressive thinking and critical reflection. He nosologically assigned pain asymbolia to the group of agnosias and integrated it into...
