Tubular apocrine adenoma of the eyelid – a case report and literature review
We present a 63-year-old male with tubular apocrine adenoma of the left upper eyelid, which recurred following initial incomplete excision. Although the lesion showed focal morphologic similarity to the apocrine variant of pleomorphic adenoma (chondroid syringoma), the diagnosis of tubular apocrine adenoma was supported by fluorescence in situ hybridization studies, which demonstrated absence of PLAG1 and HMGA2 gene rearrangements seen in pleomorphic adenoma. This case illustrates the clinical, microscopic and immunohistochemical features of tubular apocrine adenoma. The recent advances in our understanding of the molecula...

Biometry characteristics in congenital cataract patients before surgery in a tertiary eye care centre in Nepal
ConclusionsThe axial length, mean keratometry and corneal astigmatism in congenital cataract patients varied with age, gender, and laterality. The data on biometry characteristics of Nepalese pediatric patients provided in the present study are of clinical significance and hence greatly enhance the guidelines for treatment decisions, IOL power calculations and management of congenital cataract patients. Acute subconjunctival pigmentation with an underlying ciliary body mass: an unusual case presentation
Publication date: Available online 17 July 2019Source: Saudi Journal of OphthalmologyAuthor(s): Hind M. Alkatan, Amany A. Fathaddin, Ammar Al Mahmmod, Hala A. Helmi, Khalid EmaraAbstractIntraocular melanocytoma is a rare slowly growing benign tumor that may occur anywhere along the uveal tract including the ciliary body but is more commonly found near the optic disc. It is a darkly pigmented and may exhibit fast growth and possibly malignant transformation. Ciliary body melanocytoma is rare and often misdiagnosed as a melanoma. We are reporting a case of CB melanocytoma in a 74-year old lady with unusual initial presentati...

Histiocytic Sarcoma of the Eyelid
Publication date: Available online 16 July 2019Source: Saudi Journal of OphthalmologyAuthor(s): Daisy Jiayi Liu, Jacob Rullo, Vladimir Kratky, James FarmerAbstractHistiocytic Sarcoma is a rare malignant hematopoietic neoplasm that can present in extranodal sites including lymph nodes, skin, gastrointestinal tract, and the central nervous system. Only 10% of cases manifest as skin lesions and very few are reported in the head and neck. The authors report a case of histiocytic sarcoma of the eyelid in a 72-year-old male that was clinically diagnosed as a chalazion. Initial excision was not sent for routine histopathological ...

Circumscribed Choroidal Hemangioma. Clinical Features and Outcomes by Age Category in 458 Cases
ConclusionYounger patients (≤20 years) with circumscribed choroidal hemangioma present with worse visual acuity and larger, more posterior tumors. Future studies are needed to improve early detection and treatment for this subgroup of patients. (Source: Saudi Journal of Ophthalmology)

Lacrimal sac rhinosporidiosis: case report and review of literature with a new grading system to optimize treatment
We present a grading system based on our own experience in a case of extensive rhinospodiosis of lacrimal sac and review of 31 studies published in the literature.A 24-year-old male from Nepal presented with the complaints of watering from his right eye of 13 years duration, swelling in the right medial canthal area with an extension to the inferior part of the orbit for 12 years and nasal blockage for 1.5 years. The patient had a history of previous intervention in which biopsy was taken from the nose and sent for histopathology that confirmed rhinosporidiosis. An extended intranasal endoscopic dacryocystectomy was ...

Accommodative insufficiency in a patient with Prader–Willi syndrome and SNRPN gene mutation
Publication date: Available online 31 May 2019Source: Saudi Journal of OphthalmologyAuthor(s): Piotr KanclerzAbstractAccommodative insufficiency (AI) is common in children, however, has not been described in Prader–Willi syndrome (PWS). This case report presents severe AI in a child with PWS and a rare mutation on chromosome 15 (methylation at locus SNRPN).A 15-year-old boy with PWS presented with the complaint about needing to remove distance glasses while reading. The visual acuity in his right eye was 20/20 with −2.0 D, and in his left eye 20/20 with −2.75/−0.25/173°. The defocus curve manifested with severe AI...

Transcatheter Arterial Embolization of an Orbital Lymphoma
Publication date: Available online 31 May 2019Source: Saudi Journal of OphthalmologyAuthor(s): Bipasha Mukherjee, Akruti Desai, Swatee HalbeAbstractA 69-year-old male patient presented with painless protrusion of his left eye for 6 months. A large, anterior orbital vascular mass was seen in the superior conjunctival fornix. Neuroimaging and Doppler studies revealed a highly vascular circumscribed, enhancing mass lesion in the superior orbit. Carotid angiogram showed a feeder vessel which was embolized followed by complete excision of the lesion. Further investigations confirmed primary extra-marginal zone B-cell non-Hodgki...

Isolated peripunctal sebaceous gland carcinoma in a young adult; an unusual presentation
Publication date: Available online 31 May 2019Source: Saudi Journal of OphthalmologyAuthor(s): Tarjani Vivek Dave, Adit Gupta, Swathi Kaliki, Dilip MishraAbstractSebaceous gland carcinoma (SGC) of the eyelid is one of the leading peri-ocular eyelid malignancies in Asian-Indians. It usually affects elderly women and has a high rate of local recurrence, regional and distant metastasis. This is partly attributable to a delay in clinical diagnosis since it mimics more benign conditions of the eyelid like chalazia. To the best of our knowledge, this is the first case report of a peri-punctal SGC. Atypical location and the young...

Smoldering multiple myeloma revealed by superior ophthalmic vein thrombosis
We report an unusual case of superior ophthalmic vein thrombosis, as the first manifestation of multiple myeloma. As far as we know, this is the first case described in the literature. Here we describe a patient presented with a painful, visual blur and a right-sided proptosis due to superior ophthalmic vein thrombosis. Appropriate medical workup was conducted, and smoldering multiple myeloma was diagnosed as the underlying cause. We further discuss the possible involved mechanisms.
