Providing Care for Lesbian, Gay, Bisexual and Transgender Immigrants at Health Centers and Clinics
Approximately 637,000 lesbian, gay, bisexual, or transgender (LGBT)-identified documented adult immigrants live in the United States. LGBT people face persecution in dozens of countries around the world, where having an LGBT identity may be criminalized or even punishable by death. LGBT immigrants have often survived traumatic experiences in their countries of origin. In the United States, both LGBT non-immigrants and immigrants may experience significant stigma-related health disparities, which range from mental health problems to sexually-transmitted infections. Response to Letter to the Editor: Psychiatric Disease Among Patients with Takotsubo Syndrome
We read with great interest the letter to the editor on behalf of Drs. El-Battrawy et al. regarding our recent publication in Psychosomatics on the prognostic significance of pre-existing psychiatric illness in takotsubo cardiomyopathy (TC).1,2 In response to the letter, the pattern of wall motion abnormality in TC was not included in our study ′s multivariable models assessing mortality or recurrence. However, in univariate analysis, there was no significant difference in the rate of “atypical” TC patterns between patients with or without pre-existing psychiatric illness. (Source: Psychosomatics)

Lithium Toxicity after Roux-en-Y Gastric Bypass is Not Just Limited to Perioperative Period
TO THE EDITOR: There have now been several case reports and a recent case series by Bingham et al.1 regarding lithium toxicity following Roux-en-Y gastric bypass surgery.2,3 These cases have routinely examined the relationship only in the perioperative period. It is important to note that in the discussions of these cases they make mention of an in vitro experiment that shows the dissolution of unaltered lithium carbonate capsules to be about double in the postgastric bypass environment as compared to pregastric bypass environment. Psychosis in a 22-Year-Old Woman With Narcolepsy After Restarting Sodium Oxybate
Narcolepsy is a chronic, often debilitating sleep disorder characterized by intense and excessive daytime sleepiness, hypnopompic and hypnagogic hallucinations, sleep paralysis, and cataplexy.1 Sodium oxybate, the sodium salt of γ-hydroxybutyrate (GHB), is an effective treatment for moderate-to-severe excessive daytime sleepiness and cataplexy associated with narcolepsy.2,3 The precise mechanism of sodium oxybate is unknown, but likely involves GABAB activation in addition to other pathways causing central nervous system ( CNS) depression. Psychosis in a 22-Year-Old Woman with Narcolepsy after Restarting Sodium Oxybate
Sodium oxybate, the sodium salt of γ-hydroxybutyrate (GHB), is a safe and effective treatment for excessive daytime sleepiness and cataplexy in patients with narcolepsy. Other case reports have described acute psychosis in narcoleptic patients shortly after starting or increasing the dose of sodium oxybate.
