Serotonin Syndrome Following Septal Myectomy in Association With Fentanyl and Methylene Blue: A Case Report
We report a case of serotonin syndrome following septal myectomy in a patient with hypertrophic obstructive cardiomyopathy. We attribute the emergence of the syndrome to the combined use of a selective serotonin reuptake inhibitor (SSRI), fentanyl, methylene blue, bupropion, and herbal supplements. Serotonin syndrome following septal myectomy in association with fentanyl and methylene blue: A case report
The present case report describes a case of serotonin syndrome which developed post septal myectomy in a patient with hypertrophic cardiomyopathy. This case contributes to an emerging awareness of serotonin syndrome in the context of prescribed serotonergic agents, such as SSRIs, and triggered by the addition of fentanyl and methylene blue, particularly in conjunction cardiological procedures. Clinicians should be aware of such interactions which increase the emergence of the serotonin syndrome and the appropriate management of comorbid delirium post-serotonin syndrome. A Case of Mast Cell Leukemia: A Review of the Pathophysiology of Systemic Mastocytosis and Associated Psychiatric Symptoms
Mast cells, a type of white blood cell, are activated by allergens and stimulate the immune system. Once activated, these cells release immune mediators in a process called degranulation. These immune mediators include tryptase, histamine, serotonin, and cytokines and are responsible for producing inflammation. Mast cells are found throughout the body, including in the brain, especially around blood vessels and nerve endings. Mast cell degranulation is responsible for common allergic responses including eczema and anaphylaxis. (Source: Psychosomatics)

Delirium Tremens and Takotsubo Syndrome: A Role of Monitoring the Autonomic Sympathetic Nervous System?
TO THE EDITOR: I read with great interest the report by Ishida et al.1 about the 57-year-old man with a past medical history of alcohol dependence, hypertension, and diabetes, who suffered ventricular fibrillation, in the setting of takotsubo syndrome (TTS), associated with a prolonged QTc interval and torsades de pointes ventricular tachycardia, precipitated by delirium tremens, following alcohol withdrawal. Herein there are some inquiries and comments for the authors ’ kind consideration: (1) There is reference to the role of the increased blood catecholamines in the pathophysiology of TTS,1,2 although direct intense s...
