Negotiation Training for Case Managers to Improve Older Adult Acceptance of Services
Purpose of Study: Older adults frequently choose not to accept recommended social support services (e.g., caregiver and home therapy). Social workers/case managers (SWs/CMs) are often caught in the conflict encouraging patients to accept services, but facing resistance. As a result, older adults may experience unsafe home scenarios and hospital discharges. This research sought to examine whether business school negotiation and dispute resolution (NDR) training could ease these conflicts and potentially improve outcomes for both older adults and SWs/CMs. Practice Settings: Urban health care system (pilot), nation...

Back So Soon? Part 2: Use of the 5 “Whys” Process in Unplanned Hospital Readmissions
Purpose/Objectives: Readmission prevention strategies are the focus of many hospitals, but despite these efforts, unplanned, all-payer hospital readmission rates are increasing. The purpose of this study was to use root cause analysis (RCA) to explore the main cause (medical, behavioral health, and/or social) for the unplanned 30-day readmissions that the Readmission Prevention Team followed up and then to use this data to change and/or refine discharge planning interventions. Primary Practice Setting: The primary practice setting was the 229-bed study hospital where subjects with 30-day unplanned readmissions w...

Back So Soon? A Community Hospital Creates New Ways to Prevent Readmission
Conclusions: Enrollment consisted of 2,860 patients. The readmission rate for adult high utilizers fell from 40.1% to 24.6% (a 39% reduction). The PAC readmissions for both skilled nursing facilities and home health agencies fell from 12.9% to 12.5%—a small decrease. However, the overall PAC readmission rate for the CHART (Community Hospital Acceleration, Revitalization, and Transformation) program is significantly lower than the state's—the rate for home health agencies is 17.2% and for skilled nursing facilities is 20.0%—making the effort to reduce the readmission rate all the more challenging. The significant ...

The Pandemic's Secondary Statistics
Although accurate secondary statistics may never be known, the COVID-19 pandemic has certainly been a major contributor to several health care issues: social isolation/loneliness, behavioral sequelae (anxiety, drug overdoses, and suicide), delays in treating preventive care that lead to late-stage diagnoses, and even a resurgence of drug-resistant infections. Safety-Net Accountable Health Model Partnership Drives Inpatient Connection to Outpatient Social Services, Reducing Readmissions in a Population Experiencing Homelessness
Purpose of Study: To evaluate whether screening for homelessness during hospitalization and deploying outpatient care managers (CMs) to the inpatient setting to rapidly connect patients to community social services reduce hospital readmissions. Primary Practice Setting: A large safety-net hospital in Minnesota. Methodology and Sample: A quality improvement pilot proactively engaged hospitalized adults experiencing homelessness and enrolled in the Hennepin Health Accountable Health Model (HH AHM). Patients were screened for homelessness using a novel housing indicator tool, and eligible patients were visit...

Health Literacy, Health Confidence, and Simulation: A Novel Approach to Patient Education to Reduce Readmissions
Conclusions: Data collected included the following: Participation in Simulation Laboratory (SIM LAB) experience: During the time frame of the project, 323 patients were identified as eligible for participation in the project. Of those identified as eligible, there were 130 patients who completed the intervention (37.15%).Readmission events within 30 days from index admission: The intervention period (January–December 2019) noted a readmission rate for the target population of patients with chronic obstructive pulmonary disease/congestive heart failure of 10.12% compared with a readmission rate of 21.05% for the no...
