Occlusal Concepts and Considerations in Fixed Prosthodontics.
Authors: Amin K, Vere J, Thanabalan N, Elmougy A Abstract The UK has an increasingly ageing population, many of whom are retaining more of their own teeth. This has led to an increase in the prevalence of tooth wear and the need to replace pre-existing failing restorations. In many cases this will be achieved by fixed prosthodontics. This paper provides a brief overview of important occlusal concepts that should be considered when providing non-implant fixed prosthodontics using either a conformative or reorganised approach. The aim is to give general dental practitioners the confidence to provide these types of re...

Demystifying Modern Dental Ceramics.
Authors: Butt K, Thanabalan N, Ayub K, Bourne G Abstract With increasing patient expectation for aesthetic dental restorations, there has been a drive towards developing ceramic materials to meet this expectation. Multiple ceramic systems have been introduced over the past four decades with considerable advances in material properties. Survival rates of all-ceramic crowns differ by type of ceramic used, fabrication method and clinical indication. Zirconia and lithium disilicate are the most commonly used contemporary ceramic materials in dentistry. Survival data for these types of restorations appears to be promisi...

Accuracy of Intra-Oral Scans Compared to Conventional Impression in Vitro.
Authors: Haddadi Y, Bahrami G, Isidor F Abstract AIM: The purpose of the present study was to assess the accuracy of intra-oral scans and conventional impression measured at various points on a single tooth preparation. METHODS: Ten conventional silicone impressions, and ten intra oral-scans using eight different digital intra oral digital scanners were taken of a prepared master tooth. The conventional impressions were directly digitised using a laboratory scanner. Each scan/impression was superimposed on a high-accuracy digital model of the prepared master tooth. For each superimposition, the deviation from t...

Interocclusal Records in Fixed Prosthodontics.
Authors: Thanabalan N, Amin K, Butt K, Bourne G Abstract An accurate interocclusal record is essential for the successful delivery of fixed prosthodontic restorations. There are various materials and techniques used to obtain an interocclusal record in order to facilitate mounting of the dental casts on an articulator. The interocclusal record describes the vertical and horizontal relationship of the maxillary and mandibular teeth. In circumstances where the vertical relationship is not supported through a tripod of widely spaced opposing contacts, the interocclusal record will be needed to restore this vertical su...

Evolution of Indirect Restorations for Fixed Prosthodontics: Planning, Preparation and Cementation.
Authors: Patel M Abstract The provision of indirect restorations utilising contemporary materials for single or multiple abutment units in primary health care is an essential requirement for all general dental practitioners to ensure predictable patient outcomes. This paper highlights the important considerations for comprehensive planning and adept treatment execution and delivery that can enhance the patient's dental experience and outcome. A selection of clinical cases highlighting established and novel restorative materials utilising conventional and adhesive clinical techniques is presented and discussed. ...

Complex Dentistry in General Practice - What Would You Do?
Authors: Eliyas S, Chana P, Briggs P Abstract In 2019, in a world of instant gratification, what are the expectations of patients and dentists? Is training sufficient to meet these expectations? Decision-making in dentistry impacts the treatment choices patients are given, and may influence the outcomes of such treatment. It is therefore important to ensure as much standardisation as possible. In order to achieve this, it is important to know the current standard and the views of dentist as this will influence dental treatment planning. Clinical Relevance: This paper captures the treatment planning dilemmas of dent...

Professional Standards: Where are We Now?
This article discusses the use and misuse of standards in the dental professional environment and reviews their fitness for purpose. it provides an overview of the legal, regulatory and professional framework within which such standards are likely to be used and applied, and examines the differing needs of the various parties within that framework. The consequences of poorly constructed and ill-communicated standards, including those whose intention and context is not made clear, are considered from the perspectives of the public and patients, the individual clinician, the wider profession and other stakeholders in the pro...

Human Factors in Dentistry.
Authors: Horton MA Abstract The term "human factors" has been batted around in the healthcare setting, and more recently in relation to dentistry. you may think this doesn't relate to you, but in reality, human factors encompass every member of the team in every process and procedure we carry out. It is by recognising that human factors exist that we also recognise that as professionals we cannot be perfect all the time. recognising and attempting to mitigate those errors forms a fundamental part of human factors. In our everyday life we make numerous errors which have little consequence normally, but the...

This study was carried out to evaluate the difference in prices for dental treatments carried out privately in general dental practice within the Birmingham area. Ten different practices were chosen at random, which were spread across Birmingham, in order to get a better insight into the differences across the districts. Their prices for pre-determined dental procedures were procured off the practices' respective websites or through telephoning. The findings of this study have shown a wide variation in prices for each dental procedure, with the greatest variation in prices between practices being ?850 for dental implants. ...

Dental Pain: Dentine Sensitivity, Hypersensitivity and Cracked Tooth Syndrome.
Authors: Longridge NN, Youngson CC Abstract Dentine hypersensitivity is a frequently encountered patient complaint that can present with a number of associated factors including erosion and abrasion. the hydrodynamic mechanism responsible for dentine hypersensitivity is intimately related to the anatomical and physiological composition of teeth. Alterations to the integrity of the enamel and dentine through processes of trauma, decay and toothwear can increase dentine permeability. This gives rise to symptoms of sensitivity as dentinal fluid movement in response to thermal, chemical and mechanical cues stimulate th...

Introduction to Pain.
Authors: Renton T Abstract This edition of PDJ is intended to provide a wide overview on orofacial pain for dental and medical teams. Both acute and chronic orofacial conditions relevant to dentistry and medicine will be covered, and emphasising a holistic and pragmatic approach. Orofacial pain is the most common presenting symptom for patients presenting to their dentist and increasingly commonly presenting to doctors in general practice and A&E departments. Pain in the trigeminal system causes much higher psychological and neurophysiological distress compared with other body regions, as the trigeminal nerve i...

Psychological Theories of Pain.
This article considers psychological and multidimensional theories of pain, which are described within the historical context within which they were developed, including behavioural, cognitive, contextual and functional theories. Research into the multifactorial nature of persistent pain has tended to focus on mechanisms of pain development and maintenance or on the function of pain. psychological approaches, which have focused on mechanism traditionally use disability, mood and quality of life measures to assess outcome, claiming little or no impact on pain intensity itself. By contrast, functional approaches include an e...

Optimal Local Anaesthesia for Dentistry.
Authors: Renton T Abstract Dentistry is unique in that high-volume surgery is undertaken efficiently on conscious patients, an anathema to most other surgical specialties, who predominantly operate on unconscious patients. local anaesthesia (LA) provides an efficient block to nociceptive pain (the first stage of the pain pathway) but only addresses one small part of the pain experience. Currently the inferior dental block is the "go to" standard for dental LA for mandibular dentistry, despite its significant short comings. Unfortunately, habit means that we continue to practise what is taught to us at den...
