Haemoglobin A1c as a screening tool to identify pre ‐diabetes and diabetes in postpartum women with gestational diabetes
Abstract The primary objective of this study was to determine the right test and time for postpartum screening of women with gestational diabetes to identify diabetes mellitus or impaired glucose tolerance. Retrospective data were collected on women with gestational diabetes followed at the Center for Diabetes and Metabolic Care at Saint Francis Hospital and Medical Center, Hartford, Connecticut, USA. Of the 150 women included in this study, blood work was completed in 42 (28%) of the subjects. Of the women completing laboratory tests, 23 of them (56.1%) had an abnormal test result. Using the standard tests, 15 (41.7%) of ...

The pregnancy journey for women with type 1 diabetes: a qualitative model from contemplation to motherhood
In conclusion, women with type 1 diabetes potentially experience seven distinct phases of the pregnancy journey, with eight themes varying in significance across phases. Contemplation begins well before Planning or Conception, and women may seek support from non‐mainstream sources. Mental health and emotional well‐being varies qualitatively and quantitatively across phases (focusing on pregnancy‐related distress, diabetes‐specific distress and general maternal well‐being), indicating a need for targeted assessment and support across the pregnancy journey. The impact of pre ‐diabetes diagnosis on behaviour change: an integrative literature review
Abstract Type 2 diabetes is a growing global problem that not only affects individuals but also has an impact upon the economic health of countries. The number of people developing type 2 diabetes can be reduced by up to by 80%; this can be achieved by targeting those who are ‘at risk’. This reduction can be achieved by appropriate lifestyle changes to diet and physical activity. It is not known what the impact of being informed of a diagnosis of pre‐diabetes has on an individual's motivation to make appropriate lifestyle changes. The aim of this study was to assess whether having the diagnosis of pre‐diabetes enco...

Acute pyelonephritis with renal vein and inferior vena cava thrombosis and pulmonary emboli in poorly controlled type 2 diabetes and HHS
We report a case of a 56‐year‐old woman with poorly controlled type 2 diabetes mellitus who presented with acute pyelonephritis and hyperglycaemic hyperosmolar state (HHS), complicated by thrombosis of the renal vein extending into the inferior vena cava. She was managed with intravenous antibiotics, intravenous fluids and insulin and with therapeutic low molecular weight heparin, despite which she developed pulmonary emboli two days after the diagnosis of renal vein and inferior vena caval thrombosis. Following optimisation of the dalteparin dose with factor Xa monitoring and a prolonged course of antibiotics, she mad...

Paediatric weight management programme for children with diabetes: a pilot study
Abstract The purpose of this pilot was to develop a curriculum and teaching plan for weight management for children with diabetes. Session aims and objectives were discussed with the diabetes multidisciplinary team. The programme was entitled ‘Stepping Stones’ and divided into a junior (4–10 year olds) and senior (11–15 year olds) programme. The Heart of England NHS Trust cares for 408 paediatric patients with diabetes. For pilot purposes Stepping Stones took place at one hospital site (n = 148). For the juniors programme 35 patients were invited (n = 7 were due to attend sessions, n = 5 actually attended). For the...

Slipping up on the sliding scale: fluid and electrolyte management in variable rate intravenous insulin infusions
This study aimed to analyse variations in intravenous fluid therapy and electrolyte management with variable rate intravenous insulin infusions (VRIIIs); and to quantify serum electrolyte changes pre‐ versus post‐VRIII and variations therein depending on supplemented fluid electrolyte compositions. A retrospective study was undertaken involving 174 VRIIIs prescribed over a 10‐week period at a tertiary teaching hospital. Each VRIII had their associated fluid prescription and serum electrolytes analysed. The results showed that 5% dextrose (46%) and 0.9% NaCl (34%) were the most commonly prescribed fluids; 64% of fluid...
