A case of hemichorea ‐hemiballism presenting in association with diabetic ketoacidosis
Abstract Movement disorders are a rare but recognised manifestation of non‐ketotic hyperglycaemic episodes on a background of poorly‐controlled diabetes mellitus. The literature is otherwise sparse when it comes to an association between hemichorea‐hemiballism and diabetic ketoacidosis (DKA). A patient with no past medical history of note was admitted with a sudden‐onset involuntary movement disorder and was found to be in DKA during her inpatient stay. There was a delay in diagnosis and management as all efforts pointed in the direction of diagnosing the cause of the sudden‐onset hemichorea‐hemiballism as a po...

Let's Prevent Diabetes: from idea to implementation
We describe a programme of work, which developed a method for identifying those with non‐diabetic hyperglycaemia (a high risk group in which type 2 diabetes is preventable) in primary care and developed and tested a prevention programme suitable for delivery within the NHS. The subsequent implementation of this programme as part of the NHS Diabetes Prevention Programme, which was launched in 2016, is then described. Copyright © 2017 John Wiley & Sons. Biosimilar insulin: the current landscape
Abstract The first insulin biosimilar, Abasaglar, is now available in the UK and several more are in development. Biosimilars are manufactured copies of previously approved biopharmaceuticals no longer under patent protection. They are not identical versions of their reference product due to the complexity of their production and resultant protein product. The present article reviews the current biosimilar landscape including manufacture, regulatory requirements, safety and prescribing issues. Abasaglar is used to illustrate the process of biosimilar development and approval. Copyright © 2017 John Wiley & Sons. (Sourc...

Webcam consultations for diabetes: findings from four years of experience in Newham
Abstract Newham diabetes service supports an ethnically diverse population in a deprived urban area and exemplifies challenges faced throughout the NHS with rising demand, high outpatient ‘did not attend’ (DNA) rates (33–50% depending on age) and poor health outcomes. With funding from the Health Foundation, we aimed to provide more accessible and cost‐effective care by replacing selected follow‐up outpatient appointments with webcam consultations. This paper reports a mixed methods evaluation comprising attendance and biomarker data, and patient and staff interviews. Consultations were offered to clinically suit...

A case of haemoglobinopathy and elevated HbA1c
Abstract A 34‐year‐old woman was referred to the specialist diabetes service for investigation of fainting episodes. She was otherwise fit and well, spending 10 hours a week cycling, running and swimming. Multiple pathology investigations were undertaken prior to her appointment which were all within normal limits except her HbA1c result which was raised at 77 mmol/mol (9.2%). She had no symptoms of hyperglycaemia. Following a review in the diabetes clinic, a suspicion of a spurious HbA1c result arose. Her blood sample was reprocessed using a boronate affinity assay (Alere Afinion analyser) and the result came back as ...

Vitamin D and type 2 diabetes
This article gives an overview about the intriguing relationship between vitamin D and type 2 diabetes and explores the mechanisms which can potentially confirm the hypothesis of altered vitamin D levels and dysglycaemic status, and whether vitamin D replacement could possibly become a therapeutic strategy in managing people with type 2 diabetes in the future. The feasibility of a physical activity intervention for adults within routine diabetes care: a process evaluation
Abstract Physical activity is an important factor in diabetes management but is rarely implemented within routine diabetes care. The purpose of this process evaluation was to explore the feasibility of a 12‐month pilot physical activity consultation intervention delivered for adults within routine diabetes care. A 12‐month pilot physical activity consultation intervention was delivered, including face‐to‐face consultations at baseline, and at six and 12 months. Additional brief consultations were conducted monthly between baseline and six months via telephone, email or face‐to‐face. Consultations were led by a ...
