Reexpansion pulmonary edema after treatment of primary spontaneous pneumothorax
Pneumologie. 2023 Dec 14. doi: 10.1055/a-2216-0404. Online ahead of print.ABSTRACTA 24-year-old male patient, without further symptoms or comorbidities presented to the emergency room with acute dyspnea after heavy lifting two days before. On auscultation an attenuated vesicular breath was noticed on the right lung. In the initial chest radiograph a right-sided primary spontaneous pneumothorax with minor mediastinal shift was diagnosed. After insertion of a 12-French chest tube the patient's clinical condition deteriorated. The following chest radiograph and computed tomography of the thorax showed a reexpansion pulmonary ...

The search for the "node of life": breathing regulation
Pneumologie. 2023 Dec 14. doi: 10.1055/a-2215-2937. Online ahead of print.ABSTRACTThe article provides a historical overview of developments in the understanding of respiratory rhythm and its control mechanisms over the last two centuries. In the 19th century, a structure in the medulla oblongata was first described as the "node of life". In 1743, Taube discovered the carotid body, and in 1927 the Spaniard de Castro described its morphology and innervation. It was only with the work of father and son Heymans that the physiological and pharmacological significance of the carotid and aortic body was recognized. Today we unde...

Early detection of lung cancer - current status and implementation scenarios
Pneumologie. 2023 Dec;77(12):1016-1026. doi: 10.1055/a-1531-0131. Epub 2023 Dec 13.ABSTRACTThe prognosis of conventionally diagnosed lung cancer patients is still rather poor. Two large, randomized trials using screening by low dose CT could demonstrate that early detection in persons with smoking as risk factor can improve this prognosis. Early detection of lung cancer can be achieved by structured screening programs using low dose CT for persons at increased risk, but in addition also by consequent management of incidental pulmonary nodules, which are seen on imaging for other reasons. Integral part of these programs sho...

Development and Update of Guideline-based Quality Indicators in Lung Cancer
CONCLUSIONS: The guideline recommendations are implemented by LCC at a high level. The process presented confirms the successful implementation of the so-called quality cycle in oncology. The QIs developed by the German Guideline Program in Oncology (GGPO) are adopted by the certification program. The implementation of the QI is measured in LCC, evaluated by the German Cancer Society (DKG), and reflected back to the GGPO. The "real world" data have led to the deletion of one QI and show a high implementation of most QIs in LCC.PMID:38081218 | DOI:10.1055/a-2204-4879 (Source: Pneumologie)

4D electromagnetic navigation bronchoscopy for the diagnosis of peripheral pulmonary nodules - An overview and preliminary clinical results
CONCLUSION: Our preliminary data suggest that 4D-ENB is a promising new alternative for the diagnosis of PPNs. To further improve diagnostic yield, 4D-END, which lacks real-time visualization, should be embedded in a multimodal diagnostic procedure with rEBUS and/or fluoroscopy.PMID:38081219 | DOI:10.1055/a-2193-0966 (Source: Pneumologie)

Palliative aspects in pulmonary oncology
Pneumologie. 2023 Dec 11. doi: 10.1055/a-2206-5602. Online ahead of print.ABSTRACTPalliative medicine is an essential part in the treatment of patients with advanced or metastatic NSCLC. A structured palliative approach beginning from diagnoses improves quality of life and maybe even prolong survival. Besides symptom control, the disease trajectory and prognosis should regularly be re-evaluated and discussed with the patient and his loved ones.PMID:38081220 | DOI:10.1055/a-2206-5602 (Source: Pneumologie)
