Brain metastases
Pneumologie. 2024 Jan 24. doi: 10.1055/a-2238-1840. Online ahead of print.ABSTRACTCerebral metastases in patients with metastatic lung cancer are found in more than 30% of patients at baseline and manifest themselves in two out of three patients during disease evolution. For a long time, the cerebral manifestation of the disease was classified as prognostically unfavorable and hence such patients were regularly excluded from therapy studies. In the context of targeted molecular therapy strategies and established immuno-oncological systemic therapies, the blood-brain barrier no longer represents an insurmountable barrier. H...

Intended and unintended effects of e-cigarettes in clinical studies: a plea for (more) transparency
Pneumologie. 2024 Jan 24. doi: 10.1055/a-2243-9399. Online ahead of print.ABSTRACTE-cigarettes are used in clinical trials to facilitate smoking cessation. Due to the health risks associated with the use of e-cigarettes, the results of clinical trials should be fully reported, including continued exclusive use and dual use of tobacco and e-cigarettes. Nicotine cessation outcomes should be reported as the primary endpoint as well as the analysis of tobacco cessation alone. As there is currently no robust evidence that reducing cigarette consumption provides substantial health benefits, cigarette consumption reduction should...

Bacterial load of the surroundings during rigid diagnostic bronchoscopy under high frequency jet-ventilation
CONCLUSION: When performing a rigid bronchoscopy, in which patients are mechanically ventilated in a controlled manner using an open HFJV system, there is an increased pathogen load in the ambient air and therefore a potential risk for the examiner.PMID:38198806 | DOI:10.1055/a-2229-4002 (Source: Pneumologie)

Home mechancial ventilation: quality of life and the final stage of life
Pneumologie. 2024 Jan 10. doi: 10.1055/a-2202-5558. Online ahead of print.ABSTRACTThe evaluation of health-related quality of life (HRQL) has gained importance in recent years. Disease-specific questionnaires are available for respiratory insufficiency, which enable the assessment of HRQL. The Severe Respiratory Insufficiency (SRI) questionnaire, which was specially developed for patients with respiratory insufficiency, is ideal for quantifying the quality of life in patients receiving home mechanical ventilation (HMV). Studies using the SRI questionnaire demonstrated that the physical functioning of patients with chronic ...

Pneumological Rehabilitation in Long COVID - A Comparative Exploratory Longitudinal Study of Long COVID and Asthma/COPD Rehabilitees
CONCLUSION: The majority of rehabilitation patients benefit greatly from pulmonary medical rehabilitation in terms of health and occupation. The content of pulmonary rehabilitation seems to be suitable for this indication group; however, there is an indication-specific use of therapy between CG and LC group. As a result, the LC group seems to need more and different therapies.PMID:38190991 | DOI:10.1055/a-2215-3067 (Source: Pneumologie)

Interdisciplinary diagnostics and therapy of malignant mesothelioma
Pneumologie. 2024 Jan 4. doi: 10.1055/a-2202-5445. Online ahead of print.ABSTRACTThe asbestos-related malignant mesothelioma (MM) is one of the common occupational cancers in Germany with approximately 1000 new cases per year. Provided that the appropriate diagnostic criteria are fulfilled, MM can be diagnosed with high specificity from both histological and cytological specimens. However, many MM are detected cyto-/histologically only at advanced stages. Clinical/radiological aspects complement each other and enable interdisciplinary assessment of tumor stage and individualized decisions on the best possible therapeutic o...

Reexpansion pulmonary edema after treatment of primary spontaneous pneumothorax
Pneumologie. 2023 Dec 14. doi: 10.1055/a-2216-0404. Online ahead of print.ABSTRACTA 24-year-old male patient, without further symptoms or comorbidities presented to the emergency room with acute dyspnea after heavy lifting two days before. On auscultation an attenuated vesicular breath was noticed on the right lung. In the initial chest radiograph a right-sided primary spontaneous pneumothorax with minor mediastinal shift was diagnosed. After insertion of a 12-French chest tube the patient's clinical condition deteriorated. The following chest radiograph and computed tomography of the thorax showed a reexpansion pulmonary ...

The search for the "node of life": breathing regulation
Pneumologie. 2023 Dec 14. doi: 10.1055/a-2215-2937. Online ahead of print.ABSTRACTThe article provides a historical overview of developments in the understanding of respiratory rhythm and its control mechanisms over the last two centuries. In the 19th century, a structure in the medulla oblongata was first described as the "node of life". In 1743, Taube discovered the carotid body, and in 1927 the Spaniard de Castro described its morphology and innervation. It was only with the work of father and son Heymans that the physiological and pharmacological significance of the carotid and aortic body was recognized. Today we unde...
