Prediction of mortality in secondary peritonitis: a prospective study comparing p-POSSUM, Mannheim Peritonitis Index, and Jabalpur Peritonitis Index
Conclusionp-POSSUM and Mannheim Peritonitis Index can be used to predict mortality in patients with secondary peritonitis. Jabalpur Peritonitis Index is not suited for this purpose. Further studies are required to improve the diagnostic performance of p-POSSUM and MPI in patients with secondary peritonitis. (Source: Perioperative Medicine)

Anesthetic management of a huge retroperitoneal leiomyoma: a case report
ConclusionAdvanced hemodynamic monitoring and modern patient blood management strategies are particularly helpful for anesthetic management of huge retroperitoneal leiomyomas. (Source: Perioperative Medicine)

Fear of anesthesia for cesarean section among pregnant women: a multicenter cross-sectional study
ConclusionFear of anesthesia was prevalent among pregnant women in Palestine. Higher fear could be predicted by education level, place of residence, satisfaction with social life, satisfaction with religious commitment, having had cesarean delivery under anesthesia, and preference for normal delivery. Anesthesiologists, gynecologists and obstetricians, nurses, midwives, and other providers of antenatal and perioperative healthcare services should consider these factors while counseling pregnant women and addressing their fear of anesthesia. The effect of using virtual reality technology on anxiety and vital signs before surgery in patients undergoing open heart surgery
ConclusionAlthough virtual reality and ordinary video interventions effectively reduce anxiety in heart surgery patients, virtual reality seems to lower anxiety in heart surgery patients by diverting attention from external stimuli and immersing the person in the virtual world more than ordinary video. Efficacy and safety of perioperative use of non-steroidal anti-inflammatory drugs for preemptive analgesia in lumbar spine surgery: a systematic review and meta-analysis
ConclusionOur findings suggest that NSAIDs are effective and safe for preemptive analgesia in the perioperative period of lumbar spine surgery and that more and better quality RCTs and more in-depth studies of pain mechanics are still needed. (Source: Perioperative Medicine)

Mismatch of populations between randomised controlled trials of perioperative interventions in major abdominal surgery and current clinical practice
ConclusionsPatients encountered in our current local clinical practice are significantly different from those in our defined set of perioperative RCTs. Older trials recruit more low-risk than high-risk participants —trials may thus ‘expire’ over time. These trials may not be generalisable to current patients undergoing major abdominal surgery, and meta-analyses or guidelines incorporating these trials may therefore be similarly non-applicable. Comparison to local, rather than national cohorts, is importa nt for meaningful on-the-ground evidence-based decision-making. (Source: Perioperative Medicine)

Periarticular injection, iPACK block, and peripheral nerve block in pain management after total knee arthroplasty: a structured narrative review
ConclusionsThe literature review indicates that the best analgesic effect is obtained by combining PAI or iPACK with a peripheral nerve block, particularly with ACB, due to its analgesic, motor-sparing effect, and satisfactory analgesia. (Source: Perioperative Medicine)

Perioperative myocardial injury is associated with increased postoperative non-cardiac complications in patients undergoing vascular surgery: a post hoc analysis of a randomised clinical pilot trial
ConclusionIn this pilot study, we found that new peri-operative myocardial injury is associated with an increased risk of non-cardiac complications within 30 days after index surgery in patients undergoing vascular surgery. Pre-operative high-sensitivity cardiac Troponin T or N-terminal pro-B-type natriuretic peptide did not appear to predict non-cardiac complications. Larger studies are needed to confirm our findings.Trial registrationEudraCT database: 2016-001584-36 (Source: Perioperative Medicine)

Protocol: dexmedetomidine on myocardial injury after noncardiac surgery —a multicenter, double-blind, controlled trial
DiscussionThe results of the current study will illustrate the effect of dexmedetomidine on myocardial injury for elderly patients undergoing major noncardiac surgery.Trial registrationThe trial was registered with Chinese Clinical Trial Registry (CHICTR) on Aug 24, 2021 (ChiCTR2100049946,http://www.chictr.org.cn/showproj.aspx?proj=131804). Preoperative moderate to severe anemia is associated with increased postoperative major adverse cardiac and cerebral events and pulmonary complications: a propensity score-matched analysis in hip fracture surgery patients over 80  years old
ConclusionsModerate to severe preoperative anemia (<  11 g/dl) is associated with increased postoperative major adverse cardiac and cerebral events and pulmonary complications. Additionally, in-hospital mortality was not significant but was higher in the preoperative moderate to severe anemia cohort. Preoperative assessment and correction of hemogl obin level to above 11 g/dl might reduce MACCE, PPCs, and in-hospital mortality in hip fracture patients over 80 years old. Significant perioperative parameters affecting postoperative complications within 30  days following craniotomy for primary malignant brain tumors
ConclusionsFour parameters were demonstrated to significantly influence the risk of PC1M in patients undergoing primary malignant brain tumor removal. Measuring and verifying these markers, especially kinetic estimated glomerular filtration rate, would help early recognition of PC1M risk in clinical care. (Source: Perioperative Medicine)

Unplanned admissions after day-case surgery in an Italian third-level pediatric hospital: a retrospective study

Restrictive versus conventional ward fluid therapy in non-cardiac surgery patients and the effect on postoperative complications: a meta-analysis
ConclusionThis meta-analysis suggests restrictive fluid therapy on the ward may be associated with an effect on postoperative complication rate. However, the quality of evidence was moderate to low, the sample size was small, and the data came from both RCTs and non-randomized studies. (Source: Perioperative Medicine)
