Serum Protein profile in IPMN
Objectives The incidence of intraductal papillary mucinous neoplasm (IPMN) is increasing and thereby the number of patients under surveillance. There is a need for easily available serum biomarkers to distinguish patients with low or moderate grade dysplasia from those with high grade dysplasia or IPMN-associated cancer needing surgery. The actual incidence of symptoms due to pancreatic cystic neoplasms: Are we overestimating a major indication for surgery? A prospective propensity matched cohort analysis
Objectives The prevalence of symptoms in pancreatic cystic neoplasms (PCNs) is mainly based on retrospective surgical series with consequent overestimation and uncertainty of their role in the decision-making. Aim of this study is to describe the actual prevalence of symptoms in PCNs without indication for surgery compared to a control group without PCNs. (Source: Pancreatology)

The role of age in pancreatic intraductal papillary neoplasms: same survival but different implications for the management
Objectives Little is known regarding the natural history of Intraductal Papillary Mucinous Neoplasms (IPMNs) of the pancreas diagnosed in younger individuals, as current guidelines do not specifically address this aspect. The aim of this study was to evaluate whether clinical features and malignancy risk of IPMNs are influenced by patient's age at diagnosis. (Source: Pancreatology)

Pre-operative cytologic diagnosis of pancreatic cystic lesions: Value of molecular analysis
Objectives Cystic lesions of the pancreas comprise a wide spectrum of entities, ranging from non neoplastic lesions to benign and malignant neoplasms. The management of pancreatic cystic lesions, especially when based on imaging alone, often presents a challenge. Cytological diagnostics are increasingly performed, but results can be inconclusive. Various mutations are associated with pancreatic cystic neoplasms, e.g. GNAS, KRAS and VHL mutations. Thus, molecular analysis of cytologic samples could improve pre-operative diagnostics. Is main pancreatic duct dilation really an independent risk factor for malignancy in main-duct and combined-IPMNs?
Objectives In main duct (MD) or combined (CMD) IPMN pancreatic duct (MPD) diameter between 5-9 mm is identified by IAP guidelines as a worrisome feature (WF) requiring close follow up. Some authors argue that the risk of degeneration in these pts is already high enough to recommend surgery. (Source: Pancreatology)

Progression of pancreatic branch duct intraductal papillary mucinous neoplasm associates with cyst size
Objectives Most guidelines for managing patients with intraductal papillary mucinous neoplasms (IPMNs) vary considering proposed surveillance intervals and durations, usually determined based on expert opinions rather than substantial evidence. Studies on the progression and optimal surveillance intervals of branch duct IPMNs (BD-IPMN) are limited. Thus, we evaluated the progression of BD-IPMN at a single center and determined the optimal follow-up intervals and duration. (Source: Pancreatology)

Peripoerative analysis and long term results of complications after surgical therapy of IPMN
Objectives Intraductal papillary mucinous neoplasms (IPMNs) are detected more frequently due to advanced medical imaging. IPMNs are considered to be premalignant lesions with different rates of malignant transformation dependent on histological characteristics and localization. Malignancy rate in IPMNs with high-risk stigmata is up to 62%. Although premalignant lesions, the risk of operation is high. Therefore, the risk of a malignant transformation must be balanced with the perioperative risks and with the quality of life of patients. (Source: Pancreatology)

Pancreatic cystic lesions in diabetes mellitus patients
Objectives According to literature and previous data from our department, exocrine pancreatic insufficiency is relatively common among patients with diabetes mellitus (DM). Marked alterations in the exocrine pancreas are observed in patients with DM, including changes in size, morphology, and histology. Atrophy of the gland and acini, lymphocytic infiltration, moderate to severe fibrosis, and fatty changes are usually noted. Pseudocysts are the most common cystic lesions and may be formed in the setting of acute or chronic pancreatitis. (Source: Pancreatology)

Reappraisal of main pancreatic duct in IPMN under surveillance: dilatation alone is not an absolute indication for surgery
Objectives The correlation between the risk of pancreatic cancer (PC) and a dilated main pancreatic duct (MPD) in IPMNs is a matter of debate. The aim of this study was to assess the role of MPD size in predicting PC in a large cohort of both resected and surveilled IPMNs. (Source: Pancreatology)
