Comparison of electrocardiographic parameters between left bundle optimized cardiac resynchronization therapy (LOT ‐CRT) and left bundle branch pacing—cardiac resynchronization therapy (LBBP‐CRT)
ConclusionsIn patients with NICMP and LBBB, there was no significant difference in QRSd with LOT-CRT compared to LBBP-CRT. However, in patients with QRSd  >  131 ms after LBBP-CRT, LOT-CRT resulted in a significantly narrower QRS. (Source: Pacing and Clinical Electrophysiology : PACE)

J wave dynamicity during coronary angiography and intracoronary acetylcholine administration
ConclusionsBoth CAG and intracoronary Ach administration affected J waves similarly in the same individuals. A myocardial ischemia-induced conduction delay may be responsible for the changes in J waves, but further studies are needed. (Source: Pacing and Clinical Electrophysiology : PACE)

Transvenous lead extraction of implantable cardioverter ‐defibrillators: A comprehensive outcome‐and risk factor analysis
ConclusionsTLE is safe and efficacious in patients with 1- and 2-chamber ICD. Although lead dysfunction is the leading indication for extraction, systemic device-related infection is the main driver of all-cause mortality for ICD patients undergoing TLE. Cardiac electronic implantable devices: A call for a new coding system
AbstractOver the last decades, cardiac electronic implantable devices (CEID) have incorporated a myriad of technological capabilities that are not conveniently inferred by using the conventional ICHD and NBG coding systems. We propose a new coding system (i.e., the C-ARL-A coding system) aimed at overcoming this important limitation. (Source: Pacing and Clinical Electrophysiology : PACE)

Alternatives to transvenous lead placement in a patient undergoing surgical tricuspid valve repair
We present a case of multifactorial tricuspid regurgitation in a 48-year-old female with giant cell myocarditis and a transvenous implantable cardioverter-defibrillator (ICD). Key considerations in management and alternative CRM options are discussed. (Source: Pacing and Clinical Electrophysiology : PACE)

Catheter ablation of the left ‐sided variant of right top pulmonary vein in a case with persistent left superior vena cava
AbstractA 50-year-old woman underwent catheter ablation for atrial fibrillation. Preoperative computed tomography revealed a left-sided variant of the right top pulmonary vein (PV) and a persistent left superior vena cava. The right top PV was successfully isolated through a wide antral circumferential ablation line simultaneously with the right PVs. (Source: Pacing and Clinical Electrophysiology : PACE)

Epicardial connections and bi ‐atrial tachycardias: From anatomy to clinical practice
AbstractBi-atrial tachycardia (BiAT) is not rare after extensive atrial ablation or cardiac surgery. The complexity of bi-atrial reentrant circuits poses a great challenge for clinical practice. With recent advances in mapping technologies, we are now able to characterize atrial activation in detail. However, given the involvement of both atria and multiple epicardial conductions, endocardial mapping for BiATs is not easy to understand. Knowledge of the atrial myocardial architecture is the foundation for the clinical management of BiATs; as it is required to understand the possible mechanism of the tachycardia and identif...

Preclinical cardiac perforation reduction in leadless pacing: An update to the Micra leadless pacemaker delivery system
ConclusionsThis study, using computer modelling and benchtop experimentation, has indicated that increased surface area and rounding of the updated Micra catheter tip significantly improves preclinical perforation performance. It will be important to evaluate the impact of these catheter design changes with robust registry data. (Source: Pacing and Clinical Electrophysiology : PACE)

Delayed development of a huge chest wall hematoma post pacemaker implantation: A case report
AbstractAn 83-year-old Chinese man presented with a huge left chest wall hematoma and hemorrhagic shock 4 months after permanent pacemaker implantation. Computed Tomography of Angiogram of the left subclavian artery revealed a pseudoaneurysm. He underwent radiologically guided stenting followed by hematoma clearance. It is rare to have delayed formation of pseudoaneurysm at 4 months post pacemaker implantation. Radiologically guided stenting is the preferred treatment, followed by hematoma clearance. It is strongly advised against blind surgery for wound debridement or bleeding detection. Familiarizing with axillary vein a...

Electrophysiological evaluation following development of new and persistent left bundle branch block after transcatheter aortic valve replacement: A single center pilot study
ConclusionNormal HV interval up to 55  ms on modified EP study after TAVR and development of NP-LBBB can be utilized as a threshold for risk stratification to facilitate safe discharge.
