[Surgical treatment of a juxtaarticular bonecyst communicating with the ankle joint in a severe haemophilia A case].
tal I Abstract The authors report the case of a 19-year-old male patient with inhibitor-positive factor VIII deficiency. The patient had painful, limited range of motion and swelling in the left toe, due to haemarthrosis 1-2 times a month. The MR images depicted a bone cyst penetrating into the tibiotalar joint space at the distal end of the left tibia. Due to the size and localisation of the cyst, there was a high chance of a pathological fracture with a potential intraarticular component. The clinical scenario was complicated by the high level of antifactor VIII antibodies. To prevent pathological fracture and s...

Conclusion: Our patients were on lower levodopa doses (compared to literature), but the combinations were used more often. Device-aided therapies should be considered in patients with severe motor complications who receive at least 750-1000 mg levodopa daily, divided minimum 5×/day. These patients need to be tested in specialized centers by multidisciplinary teams in order to make the best decision for further action. Orv Hetil. 2019; 160(17): 662-669. [Importance of thromboprophylaxis in hospitalized non-surgical patients].
án G Abstract Venous thromboembolism (including deep vein thrombosis and pulmonary embolism) is a group of diseases with high morbidity. Mortality caused by venous thromboembolism is also highly significant. It is one of the most frequent preventable causes of death in hospital treated patients. It is not easy to assess the real prevalence of the disease because of the frequent symptomless manifestations (as autopsies become less and less frequent, the number of post-mortem diagnoses also decreases) and also because the disease often generates after hospital discharge. There are a number of factors contribu...

[The immunomodulatory role of sodium].
, Szabó AJ Abstract High salt intake, which is common in the Western world, is the cause of several lifestyle diseases. Recent investigations shed light on novel extrarenal processes, which play role in the maintenance of sodium balance. In the short term, sodium storage of the skin may serve as a buffer against volume overload arising from the osmotic properties of sodium. Increased tissue sodium concentration may also potentiate immune response against infections. In the long run, however, tissue sodium concentration over a certain limit may initiate pathophysiological processes by provoking inflammatory ...

[Up-to-date classification and multidisciplinary symptoms of Ehlers-Danlos syndromes].
e;ti S, Medvecz M Abstract In this review article, the authors summarize the clinical aspects of the novel classification of Ehlers-Danlos syndrome, which is a group of rare, hereditary connective tissue disorders. The leading symptom of the Ehlers-Danlos syndrome group is joint hypermobility, skin hyperextensibility and generalized tissue fragility. Ehlers-Danlos syndrome displays a high clinical and genetic heterogeneity and harbors many multidisciplinary properties. Certain subtypes only affect the quality of life, while other forms may lead to severe, even fatal vascular or intestinal complications. Last year,...

They present operative solutions using isoperistaltic transverse colonic segment in oesophageal stenosis caused by gastric outlet obstruction, or when these two presented together. Further indications for the above were bronchial or tracheo-oesophageal fistulas and oesophageal perforation. Late adaptation of the colonic grafts were measured by radiokinematography and histochemistry. The overall morbidity was 4.9%. Postoperative salivary fistulas closed spontaneously. Late postopera...

AIM: We aimed to assess alternative methods for use with the EOS. METHOD: After investigating 9 alternatives, five methods were selected - cervical vertebra (Hassel-Farman), iliac crest (Risser 'plus'), hip (Oxford), knee (O'Connor), calcaneus (Nicholson) - and applied to EOS scans of 114, 2-21-year-old normal individuals. Intraclass correlation coefficient tests for reliability and Spearman correlation with calendar age were assessed. ...

METHOD: Between 1 January 2006 and 31 December 2016, 29 patients were examined with a symptom-causing Zenker diverticulum. In 13 cases, transcervical surgery, in 16 cases, transoral surgery were performed. Perioperative and long-term results were evaluated and compared. [Ultrasound imaging has a potential to exhibit biotechnical advance in perioperative pain management].
si RG Abstract The perioperative pain management - instead of the efforts, guidelines and protocols - is underestimated and undertreated. Even in the case of general anaesthesia, the nervous system is overwhelmed by copious quantities of nociceptive stimuli at surgical incision. Stress and pain-modulation processes are triggered which can have significant influence on the outcome. Often the pain-management is discontinued, so a notable part of patients complain about pain in the ward after surgery. Regional anaesthesia conceptually prevents noxious inputs to enter the central nervous system, beyond surgical anaest...

Thus, the genetic pre-disposition, co-morbidities and behavioral factors all play a role in the individual variations in the disease outcomes. On the other hand, the essential part of the therapeutic strategy is the complete withdrawal of the harmful etiological agent. The present paper is devoted to overview the genetics, the environmental factors and the effects of abstinence in alcoholic liver disease. Genetic variants in two ...

CONCLUSIONS: The hsTnT levels significantly change after EPS and RFCA, in all patients who underwent ablation, and in 80% of those with EPS had hsTnT positivity in the early post-procedural phase. hsTnT levels depended significantly on the type of the subgroups and correlated with the ablation time. sz BD, Kürti Z, Lakatos PL Abstract The diagnostics of gastrointestinal diseases have evolved significantly in the past few decades. Besides endoscopy and conventional imaging modalities, there is a growing interest for rapid point-of-care laboratory tests to help discriminate between diseases with similar clinical symptoms and/or help the follow-up of chronic conditions, predicting relapses. The fecal calprotectin testing is a routine diagnostic tool in many countries. It is also more and more accessible in Hungary as well. We aim to present a short review on the role and performance of fecal calprotectin te...

Abstract The control and planning of the treatment of hypertensive patients need specific attention. As regards concomitant diseases and treatments, glaucoma and the use of eye drops should be taken into consideration. The ingredients of the administered eye drops get through the nasolacrimal canal and can be absorbed by the nasal mucosa. Because of the lack of enterohepatic 'first pass' effect, they can act systemically - like after intravenous administration. This way they can cause systemic side effects. The authors present a case of a patient, too, who was examined and medically checked regularly for years wit...

Although many cases are mild and relatively asymptomatic, hyponatremia is nonetheless important clinically because of the potential for substantial morbidity and mortality. Up to this time, the optimal treatment strategies have not been well defin...

Since 2009, there has been a continued worldwide decline in demand for allogenic blood products. [Health insurance pharmaceutical expenditures in Hungary].
CONCLUSION: The trends of health and drug expenditure show a growing tendency. We can find significant regional inequalities in case of both the drug traffic based on gross consumer prices and the health insurance subsidies. It would be needed to strengthen the elements of prevention, and to popularize health-conscious lifestyle and doing sports. Orv Hetil. 2019; 160(Suppl 1): 49-54. [Changes in disability, reduced working ability and rehabilitation benefits in Hungary between 1990 and 2015].
CONCLUSION: The sociological changes that occurred during the long period of time regrettably helped to initially increase the number of recipients of invalidity benefits, to stabilize them at a high level and to have a significant burden on the budget. This could not be counterbalanced by the rehabilitation approach of money supply either. Orv Hetil. 2019; 160(Suppl 1): 29-36. PMID: 30724599 [PubMed - in process] (Source: Orvosi Hetilap)
