Dedication
We would like to dedicate this issue of Orthopedic Clinics of North America to our beloved friend and colleague, Dr Benjamin Mauck, who was taken from us much too soon. Dr Mauck was a gifted and skillful surgeon who worked tirelessly to provide the best care for patients with difficult hand, wrist, and elbow problems. As an expert in congenital hand deformities and with a compassion for our most vulnerable population, he improved the lives of so many children. Dr Mauck pursued excellence in every aspect of his practice and life. (Source: Orthopedic Clinics of North America)

Intraoperative Challenges in Hand Surgery
A wide array of intraoperative issues can arise during surgery involving the hand and upper extremity. An understanding of the common pitfalls within hand surgery may help practicing hand surgeons circumvent such issues. Within this manuscript, we first identify problems with the increasingly popular technique of wide-awake local anesthesia no tourniquet (WALANT). Achieving appropriate hemostasis and anesthetic can be bothersome, especially for procedures proximal to the distal palmar crease. We discuss our local anesthetic timing and concentrations to help mitigate such issues, as well as other problems that may arise in ...

The Differentiation Between Infection and Acute Charcot
The differentiation between acute Charcot neuroarthropathy and infection in the foot and ankle should be supported by multiple criteria. A detailed history and physical examination should always be completed. Plain radiographs should be performed, though advanced imaging, currently MRI, is more helpful in diagnosis. Scintigraphy and PET may become the standard imaging modalities once they are more clinically available due to their reported increased accuracy. Laboratory analysis can also act as a helpful diagnostic tool. Histopathology with culturing should be performed if osteomyelitis is suspected. The prompt diagnosis a...

The State of Local Antibiotic Use in Orthopedic Trauma
Fracture-related infections are a challenging complication in orthopedic trauma that often necessitates multiple surgeries. Early administration of systemic antibiotics and surgical intervention remains the gold standard of care, but despite these measures, treatment failures can be as high as 35%. For these reasons, the introduction of local antibiotics at the site of at-risk fractures has increased over the past decade. This review looks at the various measures being used clinically including local antibiotic powder, polymethylmethacrylate, biodegradable substances, antibiotic-coated implants, and novel methods such as h...

Intraoperative Vertebral Artery Injury
Vertebral artery injury (VAI) is a serious and potentially life-threatening injury that is encountered with trauma to the cervical spine and less frequently during surgery. VAI can occur during either anterior or posterior cervical approaches or instrumentation and often involves anomalous courses of the artery. Although the incidence is rare, serious consequences including fistula formation, thrombosis, pseudoaneurysm development, cerebral ischemia, hemorrhage, and death may occur. Management of VAI can be divided into prevention, including review of preoperative imaging with knowledge of the anatomic course, utilization ...

Intraoperative Challenges of the Kinematic Knee
Total knee arthroplasty (TKA) is a widely accepted surgical procedure for managing end-stage knee osteoarthritis. Among the various TKA techniques, kinematic alignment has gained increasing popularity as it can potentially restore a more natural joint function. However, despite its theoretical advantages, kinematic total knee replacement presents several operative challenges that necessitate a thorough understanding and analysis of patient-specific anatomy during surgical planning and execution. This review article aims to critically evaluate the operative challenges associated with kinematic TKA and explore potential stra...

Neuromonitoring Changes in Spinal Deformity Surgery
Spinal cord injury is one of the most feared complications in spinal deformity surgery. The surgeon must be vigilant of direct and indirect sources of injury at all points during surgery. The incidence of complications has greatly decreased with the ability to monitor the motor and sensory pathways. Changes in signaling of these pathways provide context for what the insult is, and how to correct it before it becomes irreversible. There are well-established protocols that provide an algorithmic response to changes that can help all in the room determine the source of injury, and the appropriate reaction. (Source: Orthopedic...

The Medial Pivot Design in Total Knee Arthroplasty
Medial pivot total knee arthroplasty implants are designed to function in a similar manner to that of the native knee with a relatively fixed medial center of rotation and a less conforming lateral compartment that follows an arcuate path. Medial pivot implants in total knee arthroplasty have increased in popularity with many companies offering medial pivot or retrofitted medial congruent implants, and there are variations between the various medial pivot and medial congruent implants. Existing literature on medial pivot implants have demonstrated high survivorship and patient outcomes. More studies are needed to compare n...

Management of Intraoperative Acetabular Fractures During Total Hip Arthroplasty
Intraoperative fractures of the acetabulum are a rare but serious complication during total hip arthroplasty. Acute fractures generally require attention with plating, whereas chronic acetabular fractures may be approached with distraction, a Burch-Schneider cage, or a custom implant. It is imperative for arthroplasty surgeons to possess a thorough understanding of how to identify and manage these injuries. Collaborating with an orthopedic traumatologist for assistance with plating the anterior or posterior column, if necessary, can be invaluable. Management options encompass conservative management, revision style acetabu...

Articulating Spacers in Total Hip Arthroplasty
Although one-stage exchange arthroplasty is gaining popularity, two-stage exchange arthroplasty remains the gold standard for the treatment of periprosthetic joint infections. Use of an articulating spacer for this procedure offers an avenue for maintaining hip motion and controlled weight-bearing, allowing local antibiotic elution. However, there is no uniform consensus on the optimal surgical protocol for using articulating spacers. This review describes the surgical technique for undertaking a first-stage exchange arthroplasty using an articulating spacer and discusses the pertinent literature on key concepts relating t...
