Optimization of Preoperative Anemia in Lower Limb Joint Replacement Surgery: Assessing the Rates of Allogenic Blood Transfusion and Duration of Hospital Stay
BACKGROUND: Orthopaedic procedures such as total hip and total knee replacements carry a significant risk of postoperative anemia, necessitating allogenic blood transfusions (ABTs), and an increased hospital length of stay. AIM: Our aim was to investigate whether the implementation of a local protocol designed to detect and treat preoperative anemia resulted in reduced ABT rates and a shorter duration of length of hospital stay (LOS). METHODS: We retrospectively audited 683 patients undergoing primary hip and knee replacements. We collated data for all patients about hospital length of stay and blood tran...

The Effect of Preoperative Education Prior to Hip or Knee Arthroplasty on Immediate Postoperative Outcomes
This study was a retrospective chart review of 707 hip and knee arthroplasty patients, comparing outcomes based on preoperative educational session attendance. Demographics, comorbidities, length of stay (LOS), discharge disposition, and PT performance were collected from the chart review. Patients who attended the preoperative education class had significantly greater ambulation distances (p (Source: Orthopaedic Nursing)

The Opinions of Turkish Older Adults Related to Frailty: A Qualitative Study
This study examined older adults' perceptions of frailty. The study used a descriptive qualitative design. Data were collected using semistructured interviews with 14 older adults admitted to the geriatrics outpatient clinic in one Turkish institution from September to December 2019. The interviews were transcribed and analyzed using the MAXQDA data analysis software. Codes were combined into themes. The respondents were aged 74.5 ± 8.7 years. Two themes emerged from the qualitative data: (i) the conceptualization of frailty; and (ii) coping with frailty. Some of the participants self-identified as frail, a natural conseq...

Preoperative and Postoperative Opioid Prescription Rates in the Total Hip Replacement Surgical Patient
The United States is facing an opioid epidemic that has only worsened with the COVID-19 pandemic. There is little evidence regarding patterns of opioid use among patients with total hip replacement (THR). Although the Centers for Disease Control and Prevention has put forward guidelines for prescribing opioids, it does not include guidance specifically for THR patients suffering from presurgical and postsurgical pain. The purpose of this study was to (1) compare presurgical and postsurgical opioid rates, (2) compare presurgical and postsurgical morphine milligram equivalents (MME), and (3) determine whether having a presur...

A Comparison of Patient Outcomes Using Multimodal Analgesia Versus Opioid-Based Pain Management in Total Joint Arthroplasty
Multimodal analgesia (MMA) pain management following total joint arthroplasty (TJA) is gaining momentum as a best practice. Many forces in healthcare are coming together challenging orthopaedic teams to reevaluate postoperative pain management following surgery including the opioid crisis and pressures to improve patient experience with early discharges following surgery. Measuring the effect of adjustments to pain management is an important step. This retrospective, observational study evaluated the effect of a multimodal postoperative analgesia regimen on patient outcomes and opioid use at a Midwest academic medical cent...

The Impact of Proper Opioid Disposal Education
CONCLUSION: Those who received education on appropriate opioid disposal were more likely to dispose of it in general as well as dispose of it correctly. Although not statistically significant, the findings from this study are clinically significant in that they provide direction on the importance of standardized and focused patient education on proper opioid disposal. Nurses' Ability to Recognize and Prevent Opioid-Induced Respiratory Depression: An Evidenced-Based Practice Project
Opioids are the primary therapy for acute postoperative pain, despite being associated with opioid-induced respiratory depression (OIRD). The purpose of this study was to improve nurses' knowledge, confidence, and ability to recognize, prevent, and treat OIRD in postoperative inpatients and evaluate the feasibility of using the Pasero Opioid-Induced Sedation Scale (POSS). Registered nurses completed three tools: (1) an Opioid Knowledge Self-Assessment, which was administered pre- and post-education; (2) a Confidence Scale, which was administered pre- and post-education; and (3) a POSS Perceptions and Usability Scale that w...

NCPD Tests: Evaluation and Treatment of Low Back Pain in Adult Patients

Evaluation and Treatment of Low Back Pain in Adult Patients
It is estimated that over 80% of adults will have difficulty with low back pain during their lifetime. Back pain is generally described as acute, subacute, and chronic based on the duration of symptoms. Although most back pain complaints are self-resolved and not considered emergent, some do hold an increased level of urgency. Recognizing red flags early in the evaluation is crucial to expedite appropriate treatment. In the absence of red flags routine imaging should be avoided. A single standard of care for patients with low back pain has not been established. The myriad of diagnoses and treatment choices makes it difficu...
