Is the practice of oral pathology reaching a crossroads?
In 2017, the United States Food and Drug Administration approved marketing of the first whole slide imaging system that allows for primary diagnosis of digitized surgical pathology slides prepared from biopsied tissue.1 The College of American Pathologists in collaboration with the American Medical Association subsequently created and approved 43 distinct digital pathology digitization procedure codes that have laid the groundwork for eventual reimbursement for these services.2 During the COVID-19 pandemic, the use of digital pathology, including within large academic centers, emerged as a timely and powerful technology fo...

An expansile radiolucent lesion of the maxilla in an 11-year-old male
An 11 year-old male presented to the dentist with painless right mid-facial swelling. His mother reported that the swelling had been progressing slowly over the course of the last few years, but that recently it has been more pronounced. He was otherwise in good health. There was no history of trauma to the face or jaws. The dentist recommended further evaluation by an Oral& Maxillofacial Surgeon. On examination, the facial swelling was firm and non-tender to palpation. There was no associated cervical lymphadenopathy. Is maxillary sinusitis and radiographic maxillary sinus opacification associated with an altered microbiology of MRONJ?
This study aims to characterize the microbiome of maxillary MRONJ with concurrent maxillary sinusitis and radiographic maxillary sinus opacification to determine if there is a relationship between the microbiome of MRONJ and sinus disease. Aggressive presentation of ameloblastic fibro-odontoma: A clinical-pathological enigma
Ameloblastic fibro-odontoma (AFO) is a rare, gnathic, benign, mixed odontogenic tumor that commonly presents in the first or second decade of life as a unilocular and rarely multilocular radiolucency with variable amounts of calcified material. Tumor progression is typically indolent and generally-accepted treatment is surgical enucleation and curettage. This case report describes an atypical presentation in a 14-year-old male with a multilocular, aggressive AFO requiring hemimandibulectomy with immediate osseous and dental, “Jaw in a Day” (JIAD), reconstruction. Photodynamic therapy for refractory erosive oral lichen planus: a case series study
Oral lichen planus is a chronic inflammatory disease that occurs on the oral mucosa and is an oral potentially malignant disease. Treatment of oral lichen planus aims to promote healing of the erosion, relieve pain, reduce recurrence of the erosion and prevent canceration. Corticosteroids are the first line of treatment for oral lichen planus. Refractory oral lichen planus is a clinical classification of oral lichen planus that has not responded to corticosteroid treatment for more than two months. Primary intranodal Warthin-Like variant of mucoepidermoid carcinoma
This report describes a rare case of primary intranodal Warthin-like mucoepidermoid carcinoma (WL-MEC) presenting as a left level II lymph node mass in a 48-year-old man. Warthin-like mucoepidermoid carcinoma is a recently defined variant of MEC that bears a close histologic resemblance to Warthin tumor. Whereas MEC has readily identifiable key histologic features that render diagnosis relatively straightforward, WL-MEC is a challenging diagnosis due to overlapping histologic features and only limited case reports in the literature. Clinical Pathology Correlation Case 1: Multiple painful lesions of the tongue
A 33 year-old black male presented to the oral pathologist with a two month history of painful lesions on the right side of the tongue. The patient had no significant medical history, and he reported occasional marijuana use. Clinical evaluation revealed several firm white patches on the right lateral and ventral tongue that were extremely tender both to palpation and at rest (Figure 1). The remainder of the oral mucosa was within normal limits, and the patient reported no other lesions. (Source: Oral Surgery, Oral Medicine, Oral Pathology, Oral Radiology, and Endodontics)

Clinical pathologic conference case 2: expansile sclerotic lesion in the left maxilla
A nine-year-old male presented with expansion and mild pain of the left maxilla of unknown duration. The submitting clinician suspected fibrous dysplasia. The radiograph demonstrated a lesion of the left maxilla involving the entire superoinferior thickness of the maxilla with slight expansion into the sinus and extended from the lateral left maxilla to the midline. The internal structure was largely radiopaque and homogenous ( “ground glass”) and surrounded permanent and developing teeth without displacement (Figures 1 and 2). Case 3: aggressive tumor presenting as chronic sinusitus and odontogenic infection.
A 47-year-old female presented to the Emergency Department (ED) for evaluation of suspected odontogenic sinusitis. She reported a several-month history of sinus congestion with recent onset of intermittent epistaxis from the right nostril, swelling in the right maxilla, and severe pain of the right cheek. The pain radiated to the head and interfered with sleep. The patient incidentally tested positive for COVID-19 during her ED stay.Prior to the ED visit, the patient had dental interventions in the right posterior maxillary area. Clinical Pathologic Conference Case 4: A Multilocular Radiolucency of the Anterior Mandible
A 42-year-old female with a 15 pack-year history of smoking presented for extraction of root tip #20. A well-defined multilocular radiolucency of the anterior mandible of unknown duration was discovered on a panoramic radiograph (Figure 1).
