EURETINA White Paper: Facing up to Gender Inequality in the Field of Retina.
Despite some advances in recent years, the harsh reality remains that the field of retina, albeit far from being the worst offender, falls well short in the quest for gender equality. EURETINA is fully committed to playing its part at an institutional level in making the field of retina a truly egalitarian, diverse and positive environment of which we can all be truly proud. To shed light on the extent of gender inequalities in the subspecialty and formulate possible measures to improve the situation, EURETINA commissioned an online survey to understand the perceptions and experiences of women working in the field of re...

Analysis of microvascular abnormalities in obesity: a comparative study with healthy subjects using swept source optical coherence tomography angiography and adaptive optics
CONCLUSION: Obesity was associated with retinal microvascular changes regardless of the presence of diabetes and hypertension. Our findings suggest the presence of infraclinical microvascu lar changes directly associated with obesity, which can be identified noninvasively through retinal imaging. (Source: Ophthalmologica)

Heritability and risk factors of incident small and large drusen in the Copenhagen Twin Cohort Eye Study: a 20-year follow-up
Introduction: The transition from a normal fundus to one with early drusen ( ≥ 20 small hard drusen) to age-related macular degeneration (AMD) in the form of drusen ≥ 63 µm in diameter is of interest, because small hard drusen may be precursors of large drusen. Study of AMD precursors lesions may provide valuable insight into factors that initiate AMD. Here, the progres sion of drusen was studied over an interval of 20 years in a population-based twin cohort. Methods: Single-center, 20-year follow-up of 138 twins, including biometry, fundus optical coherence tomography, and fundus photography. Macular characteristic...

Refractive Outcomes of Combined Cataract Surgery and Vitrectomy Compared to Cataract Surgery Alone
Conclusion: Combined cataract surgery and vitrectomy allows excellent refractive outcomes equal to cataract surgery alone, allowing each procedure to be performed independently by separate anterior and posterior segment surge ons. Combined procedures can be performed in eyes with a variety of retinal indications and can include fluid-gas exchange with minimal risk of PCIOL malposition or change in targeted refraction. Prevalence and Area of Retinal Pigment Epithelium and Outer Retinal Atrophy in eyes with Non-Exudative Macular Neovascularization
Conclusion: In our study, the presence of NE-MNV was not associated with the prevalence of cRORA and/or FAF GA. Nonetheless, eyes with N E-MNV presented smaller areas of GA, which suggests that this type of neovascularization may prevent the progression of RPE and outer retinal atrophy. Longitudinal studies are required to confirm these preliminary results. (Source: Ophthalmologica)

Sex Hormones in Males and Females with Active Central Serous Chorioretinopathy
Conclusions: In this study, we did not find evidence for a disturbance in sex hormone levels in males with CSC. The lower levels of sex hormone-binding globulin in females with CSC, leading to a disturbed free testosterone/estradiol-ratio, warrants further investigation into the role of androgens in females with CSC. Intravitreal aflibercept versus brolucizumab for treatment-naive neovascular age-related macular degeneration with type 1 macular neovascularization: Comparison of short-term outcomes
Introduction: The purpose of this study was to evaluate the outcomes of loading-phase treatment with intravitreal aflibercept (IVA) or brolucizumab (IVBr) for treatment-na ïve neovascular age-related macular degeneration (nAMD) associated with type 1 macular neovascularization (MNV). Methods: We retrospectively studied 108 consecutive eyes undergoing IVA and 103 consecutive eyes administered IVBr. Best-corrected visual acuity (BCVA), central macular thickness (CMT), central choroidal thickness (CCT), dry macula achievement, polypoidal lesion regression, and development of intraocular inflammation (IOI) were all assessed. ...

Is there a Dose-Response Relationship? Real-World Outcomes of Anti-Vascular Endothelial Growth Factor Treatment in Neovascular Age-Related Macular Degeneration
Conclusions: A dose-response relationship between anti-VEGF injections and VA outcome was found only for patients with low baseline VA. Individual patient characteristics might need to be included to precise individualized treatment regimen and improve visual outcome. (Source: Ophthalmologica)

Analysing Subgroups and Treatment Discontinuation in a Finnish Cohort of Patients with Neovascular AMD
Conclusions: Long-term VA stabilization was obtained in a regional material. Patients with neovascular AMD consist of cohorts with varying visual outcome and treatment time. Many of the patients benefit from the treatment for some time, however. When comparing real-world results, the outcome of the different follow-up time cohorts should be considered. This calls for new methods for analysing real-world nAMD treatment results. (Source: Ophthalmologica)

Scleral Buckling Alone or in Combination with Pars Plana Vitrectomy for Rhegmatogenous Retinal Detachment Repair: A Meta-Analysis of 7212 Eyes
Conclusions: There was no significant difference in final BCVA between SB+PPV and SB alone in RRD. SB alone offers a slightly higher final reattachment rate along with a reduced risk of macular edema and cataract. Primary reattachment rate and the inci dence of other complications were similar between the two procedures. (Source: Ophthalmologica)

High User Acceptance of a Retina e-Learning App in Times of Increasing Digitalization of Medical Training for Ophthalmologists
Conclusions: The COVID-19 pandemic has led to a significant change in training behaviour in ophthalmology towards e-learning and online courses, which has not been accompanied by a general decline in training activity. The exemplarily investigated application showed a high user acceptance among ophthalmologists.Ophthalmologica (Source: Ophthalmologica)

High user acceptance of a retina e-learning app in times of increasing digitalisation of medical training for ophthalmologists
Introduction: The aim was to identify changes in continuing education and training in ophthalmology in the context of the Covid-19 pandemic and advancing digitalisation and to analyse the acceptance of e-learning tools among German ophthalmologists using a novel Retina Case App as an example. Methods: The participants’ training behaviour before and during the Covid-19 pandemic was surveyed. Furthermore, the acceptance and usability of the Retina Case App was evaluated using the System Usability Scale (SUS). A possible influence of the app on everyday clinical practice was assessed. Results: A total of 145 ophthalmolog...

The Therapeutic Conversation and a Holistic Approach to the Management of Patients with Neovascular Age-related Macular Degeneration
In the recent paper by Zvi et al. in Ophthalmologica, they demonstrate that reassuringly in the management of patients with neovascular age-related macular degeneration, virtual evaluation results in similar treatment decisions compared to standard face-to-face clinical examination. However, we would caution against the exclusive use of virtual clinics to manage macula clinics but rather intersperse virtual clinic appointments with regular face to face assessments. Many macula patients who are often experiencing progressive visual loss derive benefit from a face to face “therapeutic conversation”. Furthermore, there ar...
