Endometrial cancer
The incidence of endometrial cancer in the UK has risen steeply since the 1990s. It is not one but several different diseases with different aetiologies, histological and molecular characteristics and prognosis. Survival has improved significantly over several decades but outcomes for 25% of women remain poor. Surgery is the main treatment. Radiotherapy and chemotherapy are used to reduce recurrence, and for upfront treatment in selected cases. Modern considerations for perioperative care in gynaecology
Modern lifestyle has led to an increased number of surgical patients having comorbidities, often related to increased perioperative morbidity and mortality. Early recognition of risk factors and preoperative optimization can improve outcomes. The continuous trend of minimal access procedures in gynaecology and the application of enhanced recovery programmes have improved clinical and cost effectiveness. More recently, perioperative medicine has faced the challenges of the COVID-19 pandemic, requiring policies to reduce inpatient transmission. Surgical management of tubal disease and infertility
Reproductive surgery remains an important treatment option for some women with subfertility secondary to tubal disease. This review offers a comprehensive update on current evidence and guidance. Accurate investigation of the cause, site, extent is first paramount, with fertility-preserving surgical procedures reserved for women with mild tubal disease and no confounding subfertility factors. Genetics in gynae-oncology: diagnostic and therapeutic implications
Cancer genetics is rapidly evolving and improves our understanding of the pathways of disease. This allows us to tailor the therapy and management of women with cancer, and also to help women who carry a predisposition to developing gynaecological cancers. Whilst the majority of gynaecological cancers are sporadic, a predisposition to develop cancer more frequently and earlier can be seen in more than 10% of cases. Abortion care in the time of COVID-19: an illustration of how to adapt medical care during a pandemic
Shortly after the first lockdown was announced in March 2020, a comprehensive guideline was released by the Royal College of Obstetrics and Gynaecologists documenting the changes that were acceptable to enable this essential service to continue to provide care to women. Abortion care providers had to act quickly to adapt their services; increasing the use of telemedicine and reducing the number of visits to hospitals and clinics in order to reduce risk to women requesting termination of pregnancy and making safe abortion accessible. Chronic pelvic pain
is a major public health problem that impacts all areas of a woman's life. The diagnosis is frequently difficult and delayed with women often presenting to a variety of specialties and undergoing multiple investigations before a diagnosis is reached. Aetiology is frequently multifactorial with both precipitating and perpetuating factors. Contraception in women with medical conditions
Pregnancy in women with certain medical conditions can result in significant fetal and maternal morbidity and mortality. It is therefore crucial that these women are able to plan and space their pregnancies appropriately through the use of contraception. Obstetricians and gynaecologists will often encounter these women in the inpatient and outpatient setting and be asked to give advice about the suitability of contraception. Maternal cardiac disease in pregnancy

Neurological disease in pregnancy
Neurological disease encompasses a broad spectrum of conditions which may be affected by pregnancy, present de novo in pregnancy, or are caused by the pregnancy itself. Neurological diseases remain the second most common cause of indirect maternal deaths in the UK. It is imperative that a thorough history and examination are performed when assessing pregnant women presenting with neurological conditions, and that early specialist advice is sought.
