Longitudinal Reoperation Risk After Apical Prolapse Procedures in Women Aged 65 Years and Older
CONCLUSION: The type of surgical repair is significantly associated with long-term risk of reoperation. Colpocleisis offers the least likelihood of reoperation for prolapse, followed by sacrocolpopexy; colpocleisis followed by USLS has the least risk of long-term reoperation for complication.PMID:38227947 | DOI:10.1097/AOG.0000000000005511 (Source: Obstetrics and Gynecology)

Activity Restrictions After Gynecologic Surgery
Obstet Gynecol. 2024 Mar 1;143(3):378-382. doi: 10.1097/AOG.0000000000005501. Epub 2024 Jan 11.ABSTRACTGynecologic surgeons have traditionally restricted the physical activity of postoperative patients. Minimally invasive surgery and enhanced recovery after surgery programs have contributed to decreased hospital stays and more expeditious recovery. In this narrative review, we review the current state of postoperative activity restrictions in gynecology and other specialties, the purported risks and potential benefits of postoperative activity, the available evidence to refute or support activity in the postoperative activ...

Urethral Bulking With Polyacrylamide Hydrogel Compared With Other Treatments for Stress Urinary Incontinence: A Cost-Effectiveness Analysis
Obstet Gynecol. 2024 Mar 1;143(3):428-430. doi: 10.1097/AOG.0000000000005503. Epub 2024 Jan 11.ABSTRACTOur objective was to perform a cost-effectiveness analysis comparing polyacrylamide hydrogel urethral bulking with other surgical and nonsurgical treatments for stress urinary incontinence (SUI). We created a cost-effectiveness analysis using TreeAge Pro, modeling eight SUI treatments. Treatment with midurethral sling (MUS) had the highest effectiveness (1.86 quality-adjusted life-years [QALYs]), followed by polyacrylamide hydrogel (1.82 QALYs), with a difference (Δ 0.02/year) less than the minimally important difference...

Geographic Access to Early Pregnancy Loss Management
Obstet Gynecol. 2024 Mar 1;143(3):435-439. doi: 10.1097/AOG.0000000000005505. Epub 2024 Jan 11.ABSTRACTEarly pregnancy loss (EPL) is common, but patients face barriers to the most effective medication (mifepristone followed by misoprostol) and procedural (uterine aspiration) management options. This cross-sectional geospatial analysis evaluated access in New Mexico to mifepristone and misoprostol and uterine aspiration in emergency departments (comprehensive) and to uterine aspiration anywhere in a hospital (aspiration) for EPL. Access was defined as a 60-minute car commute. We collected data from hospital key informants a...

It is important to understand the effects cancer treatment can have on sexual health and to have the tools necessary to identify and treat sexual health problems. This Clinical Expert Series discusses practical methods for routinely screening for sexual dysfunction and reviews sexual health treatment options for women affected by cancer. We review the limitations of the current literature in addressing sexual health problems among sexually a...

Association Between Length of Maternity Leave and Breastfeeding Duration in the United States: A Systematic Review
CONCLUSION: Results show a positive relationship between length of maternity leave and breastfeeding duration. Advocacy for longer, paid parental leave and more robust research rooted in rigorous methods are needed.PMID:38207334 | DOI:10.1097/AOG.0000000000005502 (Source: Obstetrics and Gynecology)

Effect of Uterine Leiomyomas on Noninvasive Prenatal Testing Parameters in the First Trimester
In conclusion, uterine leiomyomas are associated with lower fetal fraction and, in patients without obesity, with a higher rate of indeterminate results independent of leiomyoma size.PMID:38207338 | DOI:10.1097/AOG.0000000000005504 (Source: Obstetrics and Gynecology)

Evaluation of Recurrent Pregnancy Loss
Obstet Gynecol. 2024 Jan 4. doi: 10.1097/AOG.0000000000005498. Online ahead of print.ABSTRACTRecurrent pregnancy loss (RPL) affects approximately 5% of couples. Although RPL definitions vary across professional societies, an evaluation after a second clinically recognized first-trimester pregnancy loss is recommended. Good quality evidence links parental chromosomal rearrangements, uterine anomalies, and antiphospholipid syndrome (APS) to RPL. In contrast, the relationship between RPL and other endocrine, hematologic, and immunologic disorders or environmental exposures is less clear. Anticoagulant therapy and low-dose asp...

Breastfeeding Initiation, Duration, and Associated Factors Among People With Hepatitis C Virus Infection
CONCLUSION: Although HCV seropositivity is not a contraindication to breastfeeding regardless of viral load, rates of breastfeeding initiation were lower among people with HCV viremia than among those with an undetectable viral load.CLINICAL TRIAL REGISTRATION: ClinicalTrials.gov , NCT01959321 .PMID:38176013 | DOI:10.1097/AOG.0000000000005499 (Source: Obstetrics and Gynecology)

Equitable Care for Patients With Disabilities: Considerations for the Gynecologic Health Care Professional
This article establishes a comprehensive educational framework and accessible reference tools for gynecologic health care professionals to enhance their ability to offer inclusive and effective care to patients with disabilities. Insights in this article stem from expert consensus among clinicians experienced in this field and ongoing dialog with community-based disability care partners. We present actionable steps to cultivate an open, adaptable, and patient-centric method to actively engage patients and to provide suitable accommodations when needed.PMID:38176014 | DOI:10.1097/AOG.0000000000005493 (Source: Obstetrics and Gynecology)

Validity of Birth Certificate Data Compared With Hospital Discharge Data in Reporting Maternal Morbidity and Disparities
We examined whether conclusions about the incidence of maternal morbidity, including Black-White disparities, differ between birth certificate data and hospitalization data. Using linked birth certificate and hospitalization data from California and Michigan for 2018 (N=543,469), we found that maternal morbidity measures using birth certificate data alone are substantially underreported and have poor validity. Furthermore, the degree of underreporting in birth certificate data differs between Black and White individuals and results in erroneous inferences about disparities. Overall, Black-White disparities were more modest...

Early Ovarian Cancer Detection in the Age of Fallopian Tube Precursors: A Systematic Review
CONCLUSION: These findings suggest that although many types of biomarkers are being tested in ovarian cancer, most have similar or worse detection rates compared with CA 125 and have the same limitations of poor detection rates in early-stage disease. However, 27.5% of articles (36/131) reported biomarkers with better sensitivity and an AUC greater than 0.9 compared with CA 125 alone and deserve further exploration.PMID:38176019 | DOI:10.1097/AOG.0000000000005496 (Source: Obstetrics and Gynecology)
