Uses of the NIH Toolbox(R) in Clinical Samples: A Scoping Review
The objective of this scoping review was to assess the extent to which the NIH Toolbox has been used with clinical populations. Methods Guided by the Joanna Briggs Methods Manual for Scoping Reviews, records were identified through searches of PubMed MEDLINE, PsycINFO, ClinicalTrials.gov, EMBASE, and ProQuest Dissertations and Theses Global (2008–2020). Database searches yielded 5,693 unique titles of original research that used at least one NIH Toolbox assessment in a sample characterized by any clinical diagnosis. Two reviewers screened titles, abstracts, and full texts for inclusion in duplicate. Conflicts at eac...

Diagnostic Accuracy of Clinical Signs and Biochemical Parameters for External Ventricular CSF Catheter-Associated Infection
This study provides Class I evidence that in individuals requiring an external ventricular CSF catheter, an elevated CSF leukocyte count and an increased percentage of PMNs are the strongest indicators of catheter-associated infections in the days before CSF culture positivity. Inpatients With Dementia Referred for Palliative Care Consultation: A Multicenter Analysis
Discussion Inpatients with a primary diagnosis of dementia receiving PC consultation were older and more functionally impaired than those with other illnesses. They were more likely to have a code status of DNR/DNI at discharge. Few reported distressing symptoms. These results highlight the need for routine clarification of GOC for patients with dementia. Scoliosis in Spinal Muscular Atrophy Type 1 in the Nusinersen Era
The objective of this study is to document the development and progression of scoliosis in patients with SMA1 treated with nusinersen. Methods A descriptive single-center study was conducted in patients with SMA1 who were treated with nusinersen before 6 months of age. Data were collected on patients who met criteria, including age at the first nusinersen dose, number of nusinersen doses, degree of scoliosis, respiratory parameters, feeding route, and motor scores at baseline and follow-up. The Children's Hospital of Philadelphia Infant Test of Neuromuscular Disorders (CHOP-INTEND) was subanalyzed using axial (AxS) and ap...

Area Postrema Syndrome: An Initial Presentation of Double-Seropositive AQP4 and MOG Antibodies
Discussion Double-seropositive APS-onset NMOSD has not been previously reported in literature. An early diagnosis and treatment result in the resolution of APS-related symptoms and prevent further progression of the disease. (Source: Neurology Clinical Practice)

Prevalence of Rhinorrhea in Parkinson Disease: A Systematic Review and Meta-analysis
Purpose of Review To perform a systematic review and determine the prevalence of rhinorrhea in Parkinson disease (PD). Recent Findings Of 451 patients with PD and 233 controls, the pooled prevalence of rhinorrhea in patients with PD was 45.0% (95% confidence interval 33.94–56.40), which was significantly greater than that in controls (p < 0.001). The prevalence of self-reported olfactory dysfunction was greater in patients with PD; however, a pooled analysis of studies using objective scores showed no difference. The mean age of patients with PD was greater than that of controls (p = 0.002). The mean age of patie...

Risk Factors for New Neurologic Diagnoses in Hospitalized Patients With COVID-19: A Case-Control Study in New York City
Discussion Hospitalized patients with SARS-CoV-2 and new neurologic diagnoses have significant morbidity and mortality postdischarge. Further research is needed to define the effect of neurologic diagnoses during acute hospitalization on longitudinal post-COVID-19–related symptoms including neurocognitive impairment. Predictors of Disease Activity and Worsening in Relapsing-Remitting Multiple Sclerosis
Discussion Prospective data from a large clinical trial cohort show that younger MS patients with high BL relapses and MRI lesion burden have the highest risk of subsequent disease activity. Trial Registration Information Clinical trial registration number NCT00211887. CombiRx was registered at ClinicalTrials.gov (NCT00211887) on September 21, 2005. Study enrollment began in January 2005. (Source: Neurology Clinical Practice)

Severity of Epilepsy and Response to Antiseizure Medications in Individuals With Multiple Sclerosis: Analysis of a Real-World Dataset
This study supports the notion that individuals with MS + E can have more severe epilepsy than those with E – MS. Seizure severity among individuals with MS + E treated with sodium channel blockers/modulators vs other ASM classes shows no significant differences. Classification of Evidence This study provides Class III evidence that individuals with MS + E can have more severe epilepsy than those with E – MS. (Source: Neurology Clinical Practice)

Transient Global Amnesia Recurrence: Prevalence and Risk Factor Meta-analysis
Discussion The analysis showed that approximately 1 of 8 participants may experience recurrence, with an increased risk in the case of a history or current state of migraine, depression, or sexual intercourse before the event. A personal history of migraine and depression was associated with 2 and 4 times risk, respectively. (Source: Neurology Clinical Practice)

Picking Brains on a Global Scale: Practice Current and Practice Buzz
When faced with challenging clinical situations, we often ask our colleagues: Can I pick your brain? As clinicians, we aim to provide evidence-based care, often presented as standardized protocols and guidelines. However, practice differences are frequently encountered when discussing challenging cases with peers. These may be more frequent among clinicians of different institutions and/or countries. The reasons for these variations are endless: lack of enough evidence, disparities in access to resources, local population characteristics, among others. Understanding these variations is important for a few key reasons. It c...

Role of the Neurologist in the Care of Athletes With Head Injuries
Some American neurologists seem uncertain about their role in caring for head-injured athletes. Tackling Quality--It's Never a Level Playing Field: Companion Piece to the Neurology Outcome Measure Set
The American Academy of Neurology (AAN) has recently proposed 3 outcome metrics crafted to be both broadly applicable across neurologic diseases and to function as potential tools to facilitate quality improvement. These measures should be of interest to physicians and payers due to the increasing linkage of reimbursement to quality care. However, the use of quality measures cannot exist in a vacuum as external factors outside of physician control can negatively affect these metrics. The original Centers for Medicare and Medicaid (CMS) value-based programs illustrate the necessity for iterative review and revision of outco...
