Comparative Effectiveness of Carbidopa/Levodopa Enteral Suspension and Deep Brain Stimulation on Pill Burden Reduction in Medicare Fee-for-Service Patients with Advanced Parkinson ’s Disease
ConclusionCLES led to a significant reduction in pill burden and oral LEDD compared with CLES-NI and DBS patients. Pill burden reduction could be considered a treatment goal for patients with APD challenged by complex polypharmacy regimens that interfere with activities of daily living and quality of life.Graphical Abstract (Source: Neurology and Therapy)

Off-time Treatment Options for Parkinson ’s Disease
AbstractMotor fluctuations (MF) are deemed by patients with Parkinson's disease (PD) as the most troublesome disease feature resulting from the increasing impairment in responsiveness to dopaminergic drug treatments. MF are characterized by the loss of a stable response to levodopa over the nychthemeron with the reappearance of motor (and non-motor) parkinsonian clinical signs at various moments during the day and night. They normally appear after a few years of levodopa treatment and with a variable, though overall increasing severity, over the disease course. The armamentarium of first-line treatment options has widened ...

Design of a Non-Interventional Study to Assess Neurologists ’ Perspectives and Pharmacological Treatment Decisions in Early Alzheimer's Disease
AbstractIntroductionThe current therapeutic landscape of Alzheimer's disease (AD) is evolving rapidly. Our treatment options include new anti-amyloid- β protein disease-modifying therapies (DMTs) that decrease cognitive decline in patients with early AD (prodromal and mild AD dementia). Despite these advances, we have limited information on how neurologists would apply the results of recent DMT trials to make treatment decisions. Our goal is to i dentify factors associated with the use of new AD DMTs among neurologists applying concepts from behavioral economics.MethodsThis non-interventional, cross-sectional, web-based s...

Clinical and Scientific Challenges to Effectiveness Studies Under Coverage with Evidence Development in Alzheimer ’s Disease
AbstractThe Centers for Medicare and Medicaid Services (CMS) has recently issued a national coverage determination for US Food and Drug Administration (FDA)-approved anti-amyloid monoclonal antibodies (mAbs) for the treatment of Alzheimer ’s disease (AD) under coverage with evidence development (CED). CED schemes are complex, costly, and challenging, and often fail to achieve intended objectives because of administrative and implementation issues. AD is a heterogeneous, progressive neurodegenerative disorder with complex care pathw ay that additionally presents scientific challenges related to the choice of study design ...

Comparative Bioequivalence of Tablet and Capsule Formulations of Ulotaront and the Effect of Food on the Pharmacokinetics of the Tablet Form in Humans
ConclusionsThe results showed geometric mean ratios and 90% CIs for bothCmax and AUC0 –∞ for ulotaront were well within typical bioequivalence criteria of 80 –125% for both the BE and FE studies, thereby confirming the bioequivalence of the two dosage forms and no significant food effect. Causal Relationship Between Basal Metabolic Rate and Alzheimer ’s Disease: A Bidirectional Two-sample Mendelian Randomization Study
ConclusionOur study showed that higher BMR reduced the risk of AD, and patients with AD had a lower BMR. Because of a positive correlation with BMR, height and weight may have a protective effect on AD. The two metabolism-related diseases, hy/thy and T2D, had no causal relationship with AD. Frameless Robot-Assisted Asleep Centromedian Thalamic Nucleus Deep Brain Stimulation Surgery in Patients with Drug-Resistant Epilepsy: Technical Description and Short-Term Clinical Results
ConclusionsFrameless robot-assisted asleep surgery is a precise and safe approach for placing CMT electrodes in patients with DRE, shortening the surgery time. The segmentation of the thalamic nuclei enables the precise location of the CMT, and the flow of physiological saline to seal the burr holes is a good way to reduce the influx of air. CMT-DBS is an effective method to reduce seizures. (Source: Neurology and Therapy)

Familial Mediterranean Fever and Transverse Myelitis: A Causal Relation?
This report presents the first case of transverse myelitis following FMF attacks in which neurologic manifestations were resolved using colchicine treatment. Due to relapses of FMF, which were accompanied by transverse myelitis, rituximab was administered, which resulted in stabilizing disease activity. Accordingly, in the case of colchicine-resistant FMF and FMF-related demyelinating conditions, rituximab could be considered as a potential therapeutic option to alleviate both polyserositis and demyelinating manifestations. The Migraine Stigma Kaleidoscope View
AbstractThe stigma of migraine, despite an impressive body of data on the epidemiological of migraine and the associated disability, burden and cost of migraine to patients, has not been fully evaluated as an important factor in the chronicization of the disease and social isolation of the patient. In this commentary, we present three points of view. From the point of view of a European advocacy body active in the field of migraine medicine, actions to improve the perception of migraine within the community regarding the stigma for people with migraine are presented at the personal, relational and occupational levels. From...

Genetically Determined Levels of mTOR-Dependent Circulating Proteins and Risk of Multiple Sclerosis
ConclusionMolecules in the mTOR signaling pathway may bidirectionally regulate the occurrence and development of MS. PKC- α is a protective factor, while RP-S6K is a risk factor. Further explorations of pathways underlying the association between mTOR-dependent proteins and MS are required. PKC-α and RP-S6K might be used as future therapeutic targets for screening high-risk individuals and potentially improving oppor tunities for targeted prevention strategies.
