Neoadjuvant vs adjuvant chemotherapy for muscle-invasive bladder cancer: a propensity matched analysis.
CONCLUSIONS: The use of NAC+RC was associated with improved OS compared to RC+AC for patients diagnosed with T2-T4, N0, M0 bladder cancer. The increased survival benefit associated with NAC compared to AC among patients undergoing RC may be due to decreased progression to node positivity and pathological downstaging. Detrusor overactivity and underactivity: implication for lower urinary tract symptoms related to benign prostate hyperplasia diagnosis and treatment.
CONCLUSIONS: In patients with DUA or DO, surgery for LUTS/BPH provides overall good results. However, a number of factors can affect these outcomes. PMID: 32026666 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Impact of urodynamic evaluation on the treatment of women with idiopathic overactive bladder: a systematic review.
CONCLUSIONS: UDS can have a useful role in the diagnosis of idiopathic OAB in women given the possibility to gain a precise diagnosis and, therefore, a tailored treatment based on the underlying cause. The integration of clinics with UDS and all the other diagnostic available tools is desirable. PMID: 32026667 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Thulium-laser Retrograde Intra Renal Ablation (T-RIRA) of upper urinary tract transitional cell carcinoma: an ESUT study.
CONCLUSIONS: On the short term, thulium Laser ablation of UTUC is safe and feasible especially in lowgrade UTUC. PMID: 32026668 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Clinical predictors and significance of adherent perinephric fat assessed with Mayo Adhesive Probability (MAP) score and Perinephric Fat Surface Density (PnFSD) at the time of partial nephrectomy for localized renal mass. A single high-volume referral centre experience.
CONCLUSIONS: In experienced hands, APF did not impact on intra- or perioperative outcomes after PN. Metabolic syndrome was the only significant predictor of APF in our series. PMID: 32026669 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

En-bloc endoscopic enucleation of the prostate: a systematic review of the literature.
CONCLUSIONS: En bloc endoscopic enucleation of the prostate has been shown to be technically feasible and safe, with potential technical advantages over the classic three-lobe technique. Larger comparative studies are needed to evaluate the ultimate impact of the en-bloc approach on postoperative outcomes, in light of the surgeon's learning curve. En-bloc resection of bladder tumors (ERBT) for pathological staging: the value of lateral margins analysis.
CONCLUSIONS: ERBT may improve the pathological report of bladder tumor. There was a trend toward increased rate of recurrence in patients with dysplasia or malignancy in their lateral margins; however, this was not statistically significant on multivariate analysis. Further studies with larger sample sizes are required to assess the value of lateral margin analysis. PMID: 32003203 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Incidental prostate cancer after transurethral resection of the prostate: analysis of incidence and risk factors in 458 patients.
CONCLUSIONS: In a contemporary cohort of patients undergoing TURP for the treatment of LUTS, the risk of IPCA was not negligible with a rate of being 6,6%. BMI and serum leukocyte count were found to be independent factors that were positively associated with the risk of IPCA. What urologists need to know about male to female genital confirmation surgery (vaginoplasty): techniques, complications and how to deal with them.
Authors: Drinane JJ, Santucci R Abstract Vaginoplasty is the most commonly performed genital surgery for gender affirmation. Male-to- female (MTF) patients are roughly four times more likely to undergo genital surgery than female-to-male (FTM) patients. Penile inversion vaginoplasty is the most common technique used today, although there are also lesser used alternative methods including visceral interposition and pelvic peritoneal vaginoplasty. In general, outcomes are excellent, and many of the complications are self-limited. Most surgeons performing genital surgery for gender dysphoria adhere to the Wor...

How to manage a partial detachment of the ureter - too close to the anastomosis line- during robotic radical prostatectomy?
We present a technique to ensure the successful outcome in cases of a partial detachment of the ureter during RARP. When the orifice is identified after the incision of the bladder neck and it appears very close to the anastomosis line setting up a partial detachment of the ureter, before implanting a DJ stent, one tip could be to perform a small incision of the anterior wall of the orifice to spatulate it and then proceeding to a short slip of the ureter: the eversion of the mucosa - creating a sort of "folded shirt cuff"- allows the fixation of the ureter to the bladder wall. We successfully performed this technique in t...

The optimal core number and site for MRI-targeted biopsy of prostate? A systematic review and pooled analysis.
CONCLUSIONS: Increasing the number of samples per target from one to two, or two to three resulted in a nonnegligible incremental detection rate of clinically significant lesions, while obtaining more than 3 cores per target provided a diminished incremental value. And performing targeted cores accurately through center of the lesions may help improve diagnostic accuracy. Role of prostate specific antigen and prostate specific antigen density as biomarkers for medical and surgical treatment response in men with lower urinary tract symptoms: a systematic review and meta-analysis.
CONCLUSIONS: PSAD may represent an objective treatment outcome parameter and should be evaluated during the follow up of men treated for LUTS due to BPE as marker of treatment response. PMID: 31920062 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

Overview of potential determinants of radical prostatectomy versus radiation therapy in management of clinically localized prostate cancer: results from an Italian, prospective, observational study (the Pros-IT CNR study).
CONCLUSIONS: Most patients with clinically localized prostate cancer undergoing definitive treatment at Italian institutions receive RP instead of RT. Moreover, those who are younger, married, working, as well as those with better physical and sexual function are more likely to undergo surgery. Efficacy and safety of renal drainage options for percutaneous nephrolithotomy.
CONCLUSIONS: Tubeless PCNL encompasses lower morbidity and should be considered as an option for select patients, particularly with less stone burden and uncomplicated procedures. Regarding ureteral stents, SL is a safe option and does not require further procedures for removal. PMID: 31920064 [PubMed - as supplied by publisher] (Source: Minerva Urologica e Nefrologica)

The utilization status of neoadjuvant chemotherapy in muscle-invasive bladder cancer: a systematic review and meta-analysis.
CONCLUSIONS: The present study shows the low utilization rate of NAC in MIBC patients.
