Effectiveness of percutaneous endoscopic gastrostomy in amyotrophic lateral sclerosis.
CONCLUSIONS: PEG placement is as an effective, safe nutritional method with a low complication rate in patients with ALS, with or without non-invasive ventilation. The BMI and albumin levels stabilize after PEG placement, indicating benefits of early placement. PMID: 32724029 [PubMed - in process] (Source: Minerva Gastroenterologica e Dietologica)

Hypertriglyceridemic pancreatitis.
Authors: De Pretis N, De Marchi G, Frulloni L Abstract Hypertriglyceridemic acute pancreatitis is an emerging issue in gastroenterology, frequently underdiagnosed in clinical practice. Despite the rarity of the disease, hypertriglyceridemia should be considered as a leading cause of acute pancreatitis, especially in defined subsets of patients. Primary and secondary forms of hypertriglyceridemia need to be considered and excluded during the diagnostic work-up of all patients with acute pancreatitis. An accurate diagnosis is crucial to establish an appropriate treatment and to reduce the risk of recurrences...

Helicobacter pylori and non-alcoholic fatty liver disease.
Authors: Buzás GM Abstract Helicobacter pylori is the most prevalent infection worldwide, while non-alcoholic fatty liver disease emerged as the most frequent liver disease. The common occurrence can be either by chance or due to certain pathogenetic factors. Epidemiologic studies revealed that the risk of non-alcoholic liver disease is increased in patients infected with Helicobacter pylori. DNA fragments of Helicobacter pylori were rarely identified in human samples of liver carcinoma and fatty liver. Helicobacter pylori could influence the development of non-alcoholic fatty liver either by hormonal (gh...

Fibromax and inflamatory markers cannot replace liver biopsy in the evaluation of non-alcoholic fatty liver disease.
CONCLUSIONS: The Fibromax™ test and the inflamatory markers (IL 1 beta, IL-6, IL-8, TNF-alpha, MCP-1) did not present a satisfactory performance to be considered a good alternative to replace liver biopsy in the evaluation of NAFLD. Impact of E-cadherin and its transcription regulators on assessing epithelial-mesenchymal transition in chronic HCV infection.
CONCLUSIONS: E-cadherin and its transcriptional regulators; snail and slug may serve as indicators for assessing the stage of fibrosis and the progression of HCC associated with HCV infection but not for assessing the degree of hepatitis activity. Therefore, the snail family could be a promising target for designing effective preventive and therapeutic strategies for chronic HCV infection and its serious co-morbidities. Groove pancreatitis: a case report with a brief review of this diagnostic challenge.
Authors: Valentini G, Surace M, Grosso S, Vernetto A, Serra AM, Andria I, Mazzucco D Abstract Groove Pancreatitis is an uncommon and still under-recognized form of recurrent or chronic pancreatitis that involves the space between the pancreatic head, the duodenum and the common bile duct, the so called groove area. Its importance is that it can mimic pancreatic carcinoma, it may coexist with it or even masks it and should be considered in the differential diagnosis of pancreatic masses or duodenal stenosis. Groove Pancreatitis represents a diagnostic challenge because, from a clinical point of view, its ma...

Ocular manifestations of inflammatory bowel diseases: an update for gastroenterologists.
Authors: Castellano F, Alessio G, Palmisano C Abstract Inflammatory bowel diseases (IBD), that includes ulcerative colitis and Crohn's disease, can affect not only the gastrointestinal tract but a wide spectrum of organs. The extra-intestinal manifestations (EIMs) are one of the most challenging aspect of IBD, playing a significant role for the life-time care and the quality-life of patients. Ocular manifestations are the third most frequent EIMs, preceded by articular and dermatological ones. The aim of this narrative review is to describe the different types of ocular involvements, focusing on their clin...

Small bowel capsule endoscopy: where do we stand after 20 years of clinical use?
Authors: Pennazio M, Rondonotti E, Pellicano R, Cortegoso Valdivia P Abstract Small bowel capsule endoscopy (SBCE) was introduced into clinical practice almost 20 years ago and, nowadays, it is an essential tool for the study of the small bowel. SBCE allows a radiation-free examination of the entire mucosal surface of the small bowel with high-quality images, limited invasivity and a good safety profile. Nevertheless, the main limitation of SBCE is the lack of any possible direct therapeutic intervention. Indications for SBCE have evolved throughout the years, from "old" ones such as suspected small bowel ...

Elastography for characterization of focal liver lesions: current evidence and future perspectives.
Authors: Martelletti C, Armandi A, Caviglia GP, Saracco GM, Pellicano R Abstract Focal liver lesions (FLLs) are a common finding during routine abdominal ultrasound (US). The differential diagnosis between diverse types of FLLs, especially between benign and malignant ones, is extremely important and can often be particularly challenging. Radiological techniques with contrast administration and/or liver biopsy are mostly necessary for establishing diagnosis, but they have several contraindications or complications. Due to limitations of these tools, there is urgent and still unmet need to develop a first l...

Conservative treatment and radiological follow-up in a case of pneumatosis intestinalis associated with enteral tube feeding.
Authors: Brandi N, Parmeggiani A, Brocchi S, Balacchi C, Gaudiano C, Golfieri R Abstract Pneumatosis Intestinalis (PI) is a rare radiological finding and is defined as the presence of extra- luminal gas within the intestinal wall. PI can be distinguished in a benign form and a life-threatening form, burdened with high mortality. The correct management of PI must always take into account patient's history, his clinical context, laboratory test results and radiological findings, in order to administrate a proper therapy and avoid unnecessary surgical intervention. The pathogenesis of primary PI is still unkn...
