"Watch and Wait" for complete clinical response after neoadjuvant chemoradiotherapy for rectal cancer.
Authors: Peacock O, Chang GJ Abstract The management of rectal cancer has evolved substantially over recent decades, becoming increasingly complex. This was once a disease associated with high mortality and limited treatment options that typically necessitated a permanent colostomy, has now become a model for multidisciplinary evaluation, treatment and surgical advancement. Despite advances in the rates of total mesorectal excision, decreased local recurrence and increased 5-year survival rates, the multimodal treatment of rectal cancer is associated with a significant impact on long- term functional and q...

Procedure and patient selection in bariatric and metabolic surgery.
CONCLUSIONS: This review examines various factors influencing patient and procedure selection in bariatric surgery. Authors feel it is currently not possible to make strict recommendations and surgeons should carefully discuss the pros and cons of bariatric surgery and that of various options available in their practice with the patients before making a final recommendation. Liver transplantation in patients with liver metastases from neuroendocrine tumors.
Authors: Spolverato G, Bagante F, Tsilimigras DI, Pawlik TM Abstract The prevalence of metastatic disease in neuroendocrine tumors (NETs) is very high (60-80%) and cancer-related death among these patients is generally due to metastatic disease. Numerous treatment options for cure and disease control have been investigated for patients with neuroendocrine liver metastases (NELM). Despite the success of liver directed therapy on slowing tumor progression and palliating symptoms, the chance of being cured by liver resection is 40-50% and only roughly 20% of patients have potentially resectable disease. As su...

A nationwide study on the adoption and short-term outcomes of transanal total mesorectal excision in the UK.
CONCLUSIONS: This study reports acceptable nationwide short-term outcomes of TaTME. However, adoption occurred in a rapid and non-standardized manner. A structured TaTME training program is therefore recommended. PMID: 31271276 [PubMed - in process] (Source: Minerva Chirurgica)

Neoadjuvant therapy versus upfront surgery for borderline-resectable pancreatic cancer.
CONCLUSIONS: It is necessary to demonstrate the efficacy of neoadjuvant therapy and to standardize the regimens through large-scale, multicenter, randomized controlled studies. PMID: 31115240 [PubMed - as supplied by publisher] (Source: Minerva Chirurgica)

Laparoscopic splenectomy in malignancies: is it safe and feasible?
CONCLUSIONS: The analysis of our data highlights that LS for hematologic malignancies is effective and feasible even if it associated with higher conversion rate due to splenomegaly and difficult hilum dissection. Besides, no differences in the patient outcome were highlighted. LS may be considered a safe procedure in the treatment of haematological malignancies of the spleen. Hypoxic pelvic perfusion with cisplatin and mitomycin C in multidisciplinary palliative treatment of patients with unresectable recurrent rectal cancer: a retrospective study.
CONCLUSIONS: Repeated HPP treatments with mitomycin C plus cisplatin, followed by cetuximab targeted-therapy, may represent a safe and efficacious palliative therapy in patients with unresectable recurrent rectal cancer, in progression following standard systemic chemo- and radio-therapy, and thus warrants confirmation in a larger phase III study. Biliovascuar complications: a price to pay for non operative management of major liver trauma.
CONCLUSIONS: Biliovascular complications are managed by multidisciplinary approach. Lacerations extending to porta and grade IV/V injuries have a higher chance of developing biliovascular complications and should be observed closely. Complications post-laparoscopic sleeve gastric resection: review of surgical technique.
CONCLUSIONS: The analysed surgical technique seems to decrease the risk of leak without significantly impacting weight loss, and we have noticed in our clinical experience a decrease in the incidence of fistula from the time this measure was adopted. Also the statistical analysis encourage the continuation of experimentation. PMID: 30761827 [PubMed - as supplied by publisher] (Source: Minerva Chirurgica)

Cholecystectomy for acute cholecystitis in octogenarians: impact of advanced age on post-operative outcome.
CONCLUSIONS: The results of this study suggest that cholecystectomy for acute cholecystitis in octogenarians is a relatively safe procedure with an acceptable risk of complications and a postoperative hospital stay comparable to younger ones.
