Fluid management in cardiac surgery patients: pitfalls, challenges and solutions.
In conclusion, the application of a close monitoring and a pre-defined goal-directed protocol are far more important than the choice of a single fluid. This review examines the available evidence on fluid management in cardiac surgery and in the ICU, and analyzes the key steps of fluid strategy in these settings. Influence of the thoracic epidural anesthesia on the left ventricular function: an echocardiographic study.
Authors: Miró M, Sanfilippo F, Pérez F, García Del Valle S, Gómez-Arnau JI Abstract BACKGROUND: Thoracic epidural anesthesia (TEA) is widely used for major surgery, but studies assessing its impact on left ventricular (LV) systolic and diastolic function are limited, and such studies have assessed patients already under general anesthesia and/or receiving volume expansion between examinations. METHODS: Observational study at a secondary university hospital including consecutive awake patients undergoing major abdominal surgery without significant pre-existing cardiac disease. Patient...

The Committee, in this statement, points out the clinical and ethical appropriateness of palliative sedation as a therapeutic procedure. As a result, palliative sedation has to be considered today useful, scientifically safe and reliable, and acknowledged as an integral part of good clinic...

How to manage blood pressure after brain injury?
Authors: Carteron L, Taccone FS, Oddo M Abstract Manipulation of blood pressure (BP) is a mainstay of therapy in patients with acute brain injury (ABI). In the early emergent phase (first hours from injury), depending on intracranial pathology, BP manipulation aims to 1) limit the progression of parenchymal hematomas or hemorrhagic transformation (in patients with ischemic/hemorrhagic stroke and aneurysmal subarachnoid hemorrhage [SAH]), and 2) attenuate hypoperfusion and secondary cerebral ischemic insults (in patients with traumatic brain injury [TBI]). During the intensive care unit (ICU) phase, BP management is...

Opioid less perioperative care.
Authors: Cata JP, Bugada D, De Andrés J Abstract Opioids are the most frequently administered analgesics in the perioperative period. The analgesic potency of opioids is without question. While the opioid- free or opioid-less perioperative care concept is not a reality in most surgical centers of the United States and other developed countries, there is a significant number of healthcare problems (i.e. adverse events, opioid-induced hyperalgesia and opioid diversion) related to the indiscriminate use of opioids that warrants the implementation of multimodal analgesia strategies. Although it has been suggeste...
