Vogt & #8211;Koyanagi & #8211;Harada Disease and systemic lupus erythematosus occurring during adalimumab therapy for ulcerative colitis
We report the case of a 36-year-old male patient known to have ulcerative colitis on adalimumab treatment for 2 years who presented with initial-onset acute uveitis associated with Vogt–Koyanagi–Harada disease. Uveitis was treated successfully with systemic corticosteroids combined with mycophenolate mofetil. The patient had complete resolution of posterior segment inflammation and exudative retinal detachment. One year after the initial presentation, the patient was diagnosed to have systemic lupus erythematosus and adalimumab was discontinued. This case suggests that adalimumab could induce seve...

Cutaneous cell-mediated delayed hypersensitivity to intravitreal bevacizumab
Anthony Fam, Paul T FingerMiddle East African Journal of Ophthalmology 2020 27(3):182-184 The literature contains few reports describing autoimmune reactions to intravitreal bevacizumab and no Type-IV delayed hypersensitivity reactions. This was unexpected, as administration of intravenous bevacizumab has frequently caused dermatologic side-effects. This difference was likely attributable in part to the minimum 300-times difference between intravitreal versus intravenous dosing. Here, we present a case of a 52-year-old male who was treated with plaque brachytherapy for a subfoveal choroidal melanoma. The patient was treat...

The effect of a screen protector on blue light intensity emitted from different hand-held devices
CONCLUSIONS: The screen protector decreased the intensity at 450 nm for every setting other than those at 0% brightness. Decreasing brightness and applying NS mode were more effective in reducing blue light. More research is needed to determine the benefits of decreasing blue light exposure from electronic devices. Comparison of outcomes of vision and eye screening of children of four orphanages and school children of Riyadh
CONCLUSION: Orphan children had less rate of myopia compared to school children. However, unattended ocular pathologies were detected during the screening campaign. The coverage of refractive services was low in these underprivileged children compared to school children. (Source: Middle East African Journal of Ophthalmology)

Evaluation of knowledge, attitude, and practices toward the outbreak pandemic (COVID-19) Virus Disease among ophthalmologists: A cross-sectional study
CONCLUSION: KAP among ophthalmologists in the Kingdom of Saudi Arabia toward COVID-19 are optimal. However, health education campaigns are needed to reinforce the current KAP and improve the level of understanding of the risks and prevention measures that, in turn, improve the confidence of ophthalmologists to provide the right care to their patients and protect themselves as well. Improved function of orbicularis oculi by dynamic transfer of contralateral orbicularis oculi muscle in patients with facial palsy
CONCLUSION: Muscle flap transfer technique using contralateral OOM for complete facial palsy can be considered as a helpful alternative in patients who are still symptomatic despite conventional treatment modalities. (Source: Middle East African Journal of Ophthalmology)

Prevalence of keratoconus in refractive surgery cases in Western India
CONCLUSION: The prevalence of KCN in refractive surgery cases was 1.61% which is higher than those found in the western population and lower than those found in the Middle East (Saudi Arabia and Iran). Furthermore, topographic examination performed during the routine screening of patients for refractive surgery can be a useful tool to diagnose new cases of KCN in asymptomatic patients. A foldable nonpenetrating keratoprosthesis: Medium to long-term clinical results in patients with corneal blindness due to noninflammatory anterior Cornea Disease
CONCLUSIONS: Medium and long-term outcomes of the KeraKlear indicate that this device is a viable alternative to corneal transplantation as a primary procedure in patients with non-inflammatory causes of corneal blindness, especially when corneal tissue is not available. The KeraKlear does not penetrate into the anterior chamber, and therefore, is less susceptible to the most serious complications of PK including endophthalmitis, expulsive hemorrhage, and glaucoma. The KeraKlear also has a comparable or improved adverse event rate compared to PK. (Source: Middle East African Journal of Ophthalmology)

Pneumocephalus after orbital decompression surgery for thyroid eye disease
We reported a 49-year-old man who presented with CSF leakage after transcaruncular medial wall decompression for proptosis due to thyroid eye disease. He underwent an endoscopic endonasal approach to surgical repair of the defect with nasoseptal flap. Rhinorrhea was stopped immediately after endoscopic repair. Optical coherence tomography angiography and multifocal electroretinogram findings in paracentral acute middle maculopathy
Ourania Fydanaki, Tryfonas Rotsos, Osama Giasin, Dimitrios Papakonstantinou, Konstantinos AndreanosMiddle East African Journal of Ophthalmology 2020 27(2):139-141 Paracentral acute middle maculopathy (PAMM) is an optical coherence tomography (OCT) finding seen in patients with retinal capillary ischemia. In this case report, we present a case of PAMM after a transient central retinal artery occlusion and the multifocal electroretinogram (mfERG) and other multimodal imaging findings. Clinical examination, OCT angiography, OCT en face, fluorescein angiography, and visual fields were performed at the baseline and follow-up e...

Multimodal imaging of nodular posterior scleritis: Case report and review of the literature
Waleed K Alsarhani, Ahmed M Abu El-AsrarMiddle East African Journal of Ophthalmology 2020 27(2):134-138 Nodular posterior scleritis represents a small percentage of all cases of posterior scleritis. Because of the scarcity of nodular posterior scleritis, it may be confused or even misdiagnosed as an intraocular tumor or posterior uveitis. Here, we are reporting a case of nodular posterior scleritis in a 25-year-old medically free male. Furthermore, we reviewed previously reported cases of nodular posterior scleritis. Our patient presented with a choroidal mass of about one disc diameter in size. In addition, the patient h...

Central retinal vein prethrombosis secondary to retinal vasculitis: Early detection and treatment
Piergiacomo Grassi, Caterina NaclerioMiddle East African Journal of Ophthalmology 2020 27(2):131-133 The aim was to report a case of central retinal vein prethrombosis (CRVP), responsive to systemic steroids. An 18-year-old male presented with right sudden blurred vision and central scotoma for 2 days. Right best-corrected visual acuity (BCVA) measured 6/36, and fundoscopy revealed vascular congestion and blurred disc margins. Fluorescein angiography (FA) showed CRVP secondary to retinal vasculitis. Systemic oral prednisone was started. Six months later, right BCVA was 6/6, FA showed reduced vascular congestion, and retin...

Foveal congenital simple hamartoma of retinal pigment epithelium: A report of two cases
We report two cases with foveal congenital simple hamartoma of the retinal pigment epithelium (CSHRPE), as both patients presented to our retina services complaining of a unilateral decreased vision. Full ophthalmic examination and multimodal imaging were performed including fundus photography, fundus autofluorescence, optical coherence tomography, fluorescein angiography, and electrophysiological testing. Both patients presented with 20/80 vision in the affected eyes. Foveal CSHRPE was found in both eyes, along with parapapillary hyperpigmented rim, multiple pinpoint macular lesions, and few posterior pole hyperpigmented ...
