Pleural aspergillosis in a patient with recurrent spontaneous pneumothorax: the challenge of an optimal therapeutic approach
We report a case of PA in a young patient after pleurodesis for recurrent pneumothorax. Clinical resolution was achieved with systemic and local antifungal therapy combined with surgical debridement. Hepatotoxicity led to a switch from voriconazole to isavuconazole, with a successful outcome. (Source: Medical Mycology Case Reports)

The Robust and Rapid Role of Molecular Testing in Precision Fungal Diagnostics: A Case Report
Publication date: Available online 13 February 2020Source: Medical Mycology Case ReportsAuthor(s): Julie M. Steinbrink, David K. Hong, Stephen P. Bergin, Rami N. Al-Rohil, John R. Perfect, Eileen K. MaziarzABSTRACTDiagnosis of invasive fungal disease remains an ongoing challenge for clinicians, while continuously evolving treatment regimens increase patient risk for invasive infection. This case highlights how molecular testing led to the diagnosis of co-infection with two fungal pathogens producing invasive disease in a hematopoietic stem cell transplant recipient with graft-versus-host disease (GVHD). Unusual case of otomycosis caused by Saksenaea vasiformis
We report an unusual case of S. vasiformis otitis occurring in a diabetic woman after penetration of an insect in the right ear. Direct microscopic examination of the clinical sample showed hyaline and non septate hyphae belonging to the order Mucorales. Fungal identification was performed by sequencing the ITS region of the rDNA. To our knowledge, this is the first report of S. vasiformis infection in Tunisia. (Source: Medical Mycology Case Reports)

Onychomycosis caused by Pichia guilliermondii: a case report and mini-review
Publication date: Available online 8 February 2020Source: Medical Mycology Case ReportsAuthor(s): Mei-jie Zhang, Guan-zhao Liang, Huan Mei, Ge Song, Wei-da LiuAbstractOnychomycosis has been reported to be mainly caused by dermatophytes. Recently, more attention has been paid to yeast for its increasing morbidity, especially the candida specices. Here we reported a fingernail infection caused by Pichia guilliermondii, the sexual reproduction period of Candida guilliermondii. Itraconazole was used for three courses, and the patient achieved improvement without any significant side-effects. This might be the first onychomycos...

Neurological worsening during treatment of an immunocompetent adult with Cryptococcus neoformans meningitis
We describe here a young immuncompetent adult of Indian origin from Ghana, West Africa with cryptococcal meningitis who had several ups and downs during his treatment. First he developed neurologic worsening due to premature transition from intensive to consolidation phase therapy. Subsequently he deteriorated due to “immune reconstitution inflammatory syndrome (IRIS)” like phenomenon that necessitated the prolonged use of steroids. Additionally he suffered serious adverse effects due to antifungal drugs including liposomal amphotericin B and flucytosine. He recovered after 16 months of treatment. The case highlights t...
