Successful treatment of rhinoorbital mucormycosis due to Rhizopus arrhizus with liposomal amphotericin B, posaconazole and surgical debridement in a child with neuroblastoma
We present a case of rhinoorbital mucormycosis in an 11-year old male with neuroblastoma successfully treated with aggressive surgical debridement and antifungal combination of liposomal amphotericin B and posaconazole. Our patient developed signs of paranasal sinus infection and culture of fine needle biopsy grew Rhizopus arrhizus. An unusual cause of bowel obstruction: Rhizopus Arrhizus diverticulitis
We present a patient who developed a bowel obstruction one month after bone marrow transplant and was diagnosed with Rhizopus arrhizus diverticulitis despite antifungal therapy since transplantation. She underwent surgical removal with immediate fungal resurgence, declined further invasive intervention and was discharged on palliative isavuconazole. Complicated urinary tract infection by Trichosporon loubieri
We report two cases of complicated Urinary Tract Infection, one with nephrostomy tube left in-situ and other with bladder outlet obstruction, caused by Trichosporon loubieri. Both patients responded well to antifungal treatment along with change/removal of catheters. In both the cases, correct identification of T. loubieri was done by IGS1 sequencing. Prompt identification and timely management headed to good clinical outcome. Hence, clinicians should be aware of T. loubieri as an emerging fungi causing human infections. Fungal keratitis caused by Didymella gardeniae (formerly Phoma gardeniae) successfully treated with topical voriconazole and miconazole
We report a case of keratitis caused by Didymella gardeniae, formerly known as P. gardeniae. The patient had a history of stromal herpetic keratitis and had been treated with long-term topical betamethasone. He developed infectious keratitis in the left eye, with the causative fungus identified by ribosomal DNA sequencing. Combination treatment with topical voriconazole and miconazole proved effective. Molecular diagnosis of kerion celsi caused by Trichophyton tonsurans in a Italian child
We report a case of Kerion celsi caused by Trichophyton tonsurans in a child following an excoriation to the scalp caused by a fall in a public park. The use of multiplex PCR assay has enabled rapid diagnosis of tinea capitis from T. tonsurans with a result in less than 48 hours and therefore the possibility of quickly starting antifungal therapy. Mediastinal cryptococcosis simulating thyroid neoplasia in immunocompetent patient with prior diagnosis and treatment for COPD
We report a case of mediastinal cryptococcosis with thyroid invasion in a patient who presented dry cough and persistent dyspnea. This patient has been treated for COPD, presenting no clinical responses. Spirometry showed fixed flattening of inspiratory and expiratory loops, suggesting extrinsic tracheal compression. A freezing biopsy revealed a mass consisting of lymph nodes and yeasts with characteristics of Cryptococcus spp. After surgical resection of the lesion, the patient was treated with fluconazole, showing total remission of the disease and symptomatology.
