COCs with estradiol valerate/dienogest vs 17 β-estradiol/nomegestrol acetate: effects on metabolism and body composition in menopausal transition women
Context: Women during the menopausal transition (MT) have reduced fertility, and yet frequency of pregnancy in the absence of contraception is about 30%. Late reproductive age women are at a higher risk of adverse pregnancy outcome, and thus require effective contraceptive methods if pregnancy is to be avoided. The use of combined oral contraceptives (COCs), in addition to highly effective pregnancy prevention, is associated with important non-contraceptive benefits for perimenopausal women, such as an improved bleeding pattern, particularly in the setting of irregular or heavy menses, alleviation of vasomotor symptoms. Minimal invasive abdominal sacral colpopexy and abdominal lateral suspension: a multicentric prospective non-inferiority trial
Introduction and aim of the study: Abdominal Minimally Invasive Surgery (A-MIS), which is performed by minimally invasive approach such as laparoscopic, mini-laparoscopic and robotic has become widely used for advanced or multicompartmental prolapse treatment. Likewise, Abdominal Sacral Colpopexy (ASC) is considered the gold-standard for multicompartmental or advanced prolapse treatment. However, alternative surgical strategies, such as Abdominal Lateral Suspension (ALS) with mesh, have been developed in order to avoid the anatomical threats of suspension to the sacrum. Management of osteoporosis after gynecological cancer
Breast Cancer remains the most frequent cancer in women. Women with breast cancer now expect to live longer than before and the prevention of morbidity induced by cancer treatment is a concern. In ER-positive Breast Cancer, the goal of Adjuvant treatment is to inhibit the impact of estrogen on the breast. Aromatase inhibitors are associated with increased bone loss. Critical is the role of a careful baseline evaluation of the risk of fracture in all postmenopausal women with breast cancer and especially in the cases treated with aromatase inhibitors. Fertility at an advanced age: Its possibilities and risks
Recent decades have seen an increase in late parenthood, whether naturally or by means of assisted reproductive technology. With increasing maternal and paternal age the risk of obstetric pathologies increases for mothers, and perinatal pathologies increase for both mothers and offspring. We must consider this when discussing family planning with our patients. What risks must we take into account in this situation? Further research is necessary on the long-term effects of late parenthood on the offspring. (Source: Maturitas)

Menopause hormonal therapy in pathological conditions
Spanish Menopause Society (AEEM) has promoted the systematic reviews and metanalyses of studies about several common clinical conditions in which menopausal hormone therapy (MHT) can show different levels of indication, and a consortium of scientific societies coordinated by AEEM have formulated recommendations for the use of MHT. These set of based on clinical evidence elegibility criteria have been published in Maturitas and are available in our web site in order to help health professionals to feel sure when prescribing HTM. Is it feasible to treat painful hand osteoarthritis with menopausal hormonal treatment? Results from a randomised controlled trial (the HOPE-e study)
Background: Hand osteoarthritis (OA) is more common in females and relative risk increases around the menopause, implicating sex hormone deficiency. We ran a feasibility randomised controlled trial (RCT) of a form of MHT (conjugated estrogens (CE)-bazedoxifene) in post-menopausal women with painful hand OA. (Source: Maturitas)

Metabolic disturbances in menopause
Menopause maybe associated with several short, medium and long-term consequences. Metabolic disturbances represent a significant negative effect of the progressive reduction of estradiol as well as of the increased androgens/estrogens ratio occurring during perimenopause (1). Besides, other hormonal (such as, hypercortisolism and/or altered thyroid function) or non-hormonal (family hystory, dietary habits, smoking, alcohol consumption, etc.) factors, could negatively impact on metabolism in an individualized manner. Common morbidity problems of midlife Turkish women
Life expectancy is increasing while the age of menopause remains relatively unchanged, so women spend more of their life in the postmenopausal period. In 2019, the population in Turkey was reported to be around 81.4 million, with a ratio of 49.9% for women and a life expectancy of 83 years for them. The average age of natural menopause seems to be around 48.9 years in Turkey. In a survey conducted among perimenopausal women in 2008, hot flashes were the most common complaint occurring in 96.5% of women. (Source: Maturitas)

Peri- and postmenopause in patients with Polycystic Ovarian Syndrome (PCOS)
With a prevalence of 5-13 % PCOS (Polycystic ovarian syndrome) is the most common endocrine disorder of women during reproductive age. However, still little is known about PCOS during peri- and post menopause. PCOS currently is percieved to be a multifactorial genetic disease whose various phenotypes are the result of interactions with environmental factors. Past and current diagnostic criteria only allow for a diagnosis during reproductive age. While the relevance of characteristic features of PCOS such as anovulatory infertility and bleeding disorders fades with time, some features such as hyperandrognism may persist. (Source: Maturitas)

One year of specialized multidisciplinary menopause outpatient care: evaluation and sample characteristics
Introduction: It is well known that menopausal transition can be accompanied by psychological distress, hence integral care is important. In an innovative specialized multidisciplinary menopause outpatient care clinic (MMCC) specialised integral treatment is provided by a unique collaboration between gynaecologists and psychiatrists for perimenopausal patients with psychiatric distress. Changes in Iron biomarkers over menopause and associations with cardiovascular risk factors
Background: The association between iron biomarkers and cardiovascular disease risk factors (CVD-RFs) remains unclear. We aimed to 1) evaluate the cross-sectional and longitudinal associations between iron status and CVD-RFs among pre and postmenopausal women, and 2) explore if these associations were modified by menopausal status. The impact of hot flashes on sleep
The prevalence of sleep disturbances is 35-60% among postmenopausal women. During the menopausal transition, hormonal changes along with the vasomotor symptoms (VMS) and night sweats are responsible from poor sleep quality independent of age, obstructive sleep apnea syndrome and mood. The decline in estradiol and increase in follicle stimulating hormone (FSH) levels are associated with trouble falling asleep and sleep maintenance. In the SWAN study, the patients with HF complained about sleep disturbances (trouble falling asleep, waking up several times and waking up earlier than planned) more than the women who did not ha...

Ovaries out of control – hormonal strategies
The perimenopausal transition represents a therapeutic challenge for clinicians as the follicular activity of the ovaries does not decline gradually and evenly, but is accompanied by a pronounced up and down of hormone production. The female menstrual cycle involves a highly regulated chain of neuro-endocrine phenomena, maintained by a constant dialogue between the hypothalamus, the pituitary gland and the ovaries. Besides FSH, LH, estradiol and progesterone, other components of this dialogue are inhibin A and B, AMH as well as the KNDy neurons in the arcuate nucleus. (Source: Maturitas)
