Abstract No. 135 Five-Year Outcomes After MRI-Guided Transurethral Ultrasound Ablation (TULSA) of Localized Prostate Cancer
In-bore MRI-guided transurethral ultrasound ablation (TULSA) sonographically coagulates prostate tissue within focal or whole-gland ablation plans, while automatically adjusting treatment parameters based on real-time MRI thermometry feedback. The TACT pivotal study demonstrated favorable safety and efficacy at 1 year in men with localized, predominantly intermediate-risk prostate cancer (PCa). Patients with a solitary kidney and RCC are at risk of renal function deterioration and potentially ESRD with treatment, though quantification of the actual risk to inform decision-making and patient counseling is challenging. The purpose of this study was to analyze the degree of renal function deterioration and incidence of ESRD based on renal cryoablation volume parameters in patients with a solitary functioning kidney. The AUA treatment algorithm for suspicious renal masses has shifted to favor nephron-sparing surgeries including percutaneous ablation for small renal masses. A 77-year-old male patient presented with uncontrolled hematuria because of a large renal mass (maximum diameter 9.3 cm; biopsy-proven renal cell carcinoma (RCC)) that also invaded the adjacent liver segments V and VI. He was under anticoagulant therapy for atrial fibrillation and on hemodialysis because of diabetes. He was on long-term Cabozantinib treatment with stable lung and soft tissue metastases.
