Multimodality Imaging Approach for Planning and Guiding Direct Transcatheter Tricuspid Valve Annuloplasty.
Interest in transcatheter treatment of tricuspid regurgitation (TR) has grown significantly in recent years due to increasing evidence correlating TR severity with mortality, and to limited availability of surgical options often considered high-risk in these patients. Although edge-to-edge repair is currently the main transcatheter therapeutic strategy, tricuspid valve (TV) direct annuloplasty can also be performed safely and effectively to reduce TR and improve heart failure symptoms and quality of life. Right Ventricular –Pulmonary Arterial Coupling and All-Cause Mortality in Patients with Mitral Annular Calcification–Related Mitral Valve Dysfunction
Mitral annular calcification (MAC) is an increasingly recognized cause of mitral valve (MV) dysfunction.1 However, there remains no established consensus on assessment of its severity, which renders risk stratification and patient selection for MV intervention challenging. Given that MV dysfunction can result in right ventricular (RV) failure over time, measures of RV –pulmonary arterial (RV-PA) coupling—which evaluate RV contractility indexed to its afterload2—may aid in risk stratification. Tele-, Proto-, or Holosystolic Mitral Regurgitation . . . Time to Learn From Physical Examination and Go Beyond PISA
Traditionally, the first step in evaluating any valvular heart disease is always the history and physical examination, heavily relying on the characteristics of the associated murmur. The classic example is mitral valve prolapse (MVP); diagnostic bedside findings and implications of late systolic murmurs are immortalized in Dr. Barlow ’s seminal papers.1 However, the presence of nonholosystolic murmurs is not exclusive to MVP. This can occur, for example, in acute mitral regurgitation (MR) due to a ruptured papillary muscle2 or due to paroxysms of wide-open functional MR. Clinical Impact of the Volumetric Quantification of Ventricular Secondary Mitral Regurgitation by Three-dimensional Echocardiography
The assessment of ventricular secondary mitral regurgitation (v-SMR) severity through effective regurgitant orifice area (EROA) and regurgitant volume (RegVol) calculations using the proximal isovelocity surface area (PISA) method and the two-dimensional echocardiography volumetric method (2DEVM) is prone to underestimation. Accordingly, we sought to investigate the accuracy of the three-dimensional echocardiography volumetric method (3DEVM) and its association with outcomes in v-SMR patients. Temporal Trends in Right Ventricular to  Pulmonary Artery Coupling in Patients Undergoing Transcatheter Mitral Valve-in-Valve Replacement for Degenerated Mitral Bioprostheses
Advances in transcatheter therapies have provided a safe and effective valve-in-valve option for degenerated bioprosthetic mitral valves (MVs) in patients with prohibitive surgical risk.1 However, long-standing pulmonary congestion secondary to severe bioprosthetic MV degeneration creates an adverse environment for the right ventricle (RV).1,2 Right ventricular (RV) dysfunction secondary to pressure overload is important when predicting patient outcomes post —MV intervention. Right atrial remodeling and outcome in patients with secondary tricuspid regurgitation
In patients with secondary tricuspid regurgitation (STR), right atrial remodeling (RAR) is a proven marker of disease progression. However, the prognostic value of RAR, assessed by indexed RA volume (RAVi) and reservoir strain (RAS) remains to be clarified.
