Failed Bilateral Total Silastic First Metatarsophalangeal Joint Implant Reconstructed with Osteochondral Allograft Implantation
J Am Podiatr Med Assoc. 2023 Nov-Dec;113(6):22-072. doi: 10.7547/22-072.ABSTRACTFirst metatarsophalangeal joint (MPJ) arthritis is a very common form of arthritis seen in the foot. Some signs and symptoms include pain, swelling, decreased passive and active range of motion, difficulty with shoe gear, and so forth. Surgically, options for alleviating symptomatic hallux limitus and arthritis fall into two broad categories: joint sparing and joint sacrificing. In this case study, we present a patient with a bilateral failed total silastic implant of the first MPJ and our proposed revision using an osteochondral bone allograft...

Surgical Excision of Intractable Plantar Keratoses (Corns) of the Foot: A Scoping Review
CONCLUSIONS: There is modest evidence that excision of the lesion with either primary closure or healing by means of secondary intention can be useful for the management of IPKs. A further consideration is an emerging hypothesis that many of these IPKs are viral in origin, rather than mechanical, which implies that prospective studies are required with cross-reference to lesion excision by anatomical site and histopathologic confirmation of the diagnosis.PMID:38175699 | DOI:10.7547/22-044 (Source: Journal of the American Podiatric Medical Association)

The Nightmare of Calcaneus: Gustilo-Anderson Type IIIB Fractures
CONCLUSIONS: A long and laborious process is required for both the patient and the surgeon in the treatment of open calcaneal fractures. However, it is possible to obtain good results with soft-tissue reconstructions.PMID:38175701 | DOI:10.7547/21-179 (Source: Journal of the American Podiatric Medical Association)

Macrotraumatic Fractures of the Multisegmented Ankle-Foot Complex in Military Tactical Athletes: A Cohort Study
CONCLUSIONS: Sex, occupation, branch, and rank were salient factors for macrotraumatic ankle-foot fractures. These findings can be used to inform and increase precision in medical planning and in the targeted development of preventive interventions.PMID:38175726 | DOI:10.7547/22-114 (Source: Journal of the American Podiatric Medical Association)

Syndesmosis Injuries in Lateral Malleolar Fractures Accompanied by a Posterior Malleolar Fracture: A Nonfixed Posterior Fracture Fragment May Not Affect Postoperative Tibiofibular Joint Malreduction Rates
CONCLUSIONS: In lateral malleolar fractures accompanied by small-fragment posterolateral or avulsion-type posterior malleolar fractures, closed syndesmotic screw fixation does not cause syndesmosis malreduction.PMID:38170588 | DOI:10.7547/21-105 (Source: Journal of the American Podiatric Medical Association)

Combined Open and Percutaneous Plating for the Treatment of Pilon Fracture
CONCLUSIONS: A combined open and percutaneous plating approach is a feasible option for the treatment of pilon fracture. This combined plating technique involving a combination of a small anterolateral approach and a small anteromedial approach (or a small direct medial approach) yielded satisfactory outcomes without major soft-tissue complications.PMID:38170594 | DOI:10.7547/21-228 (Source: Journal of the American Podiatric Medical Association)

Is Tenderness of Medial Malleolus Circumference a Sign of Syndesmosis Injuries?
CONCLUSIONS: The absence of tenderness around the medial malleolus in Weber B ankle fractures indicates no syndesmosis injury, whereas the presence of tenderness around the medial malleolus does not mean that there is a syndesmosis injury.PMID:38170597 | DOI:10.7547/22-128 (Source: Journal of the American Podiatric Medical Association)

Pathergy After Endovenous Ablation for Lower-Extremity Venous Disease: A Case Report of an Unexpected Complication
J Am Podiatr Med Assoc. 2023 Nov-Dec;113(6):21-127. doi: 10.7547/21-127.ABSTRACTA 64-year-old woman presents with wounds to her left ankle. Although her soft-tissue cultures and arterial Doppler and duplex studies were unremarkable, her venous reflux studies showed right and left small saphenous vein insufficiency. After 8 weeks of standard treatment, her wounds to the left ankle did not improve, and she developed a wound to her right anterior leg. Her left ankle wound healed 8 months after initial presentation, and her right leg wound healed in 3 months. Thereafter she underwent an endovenous ablation of her left small sa...

Comparative Radiographic Analysis of the Width of the Lesser Digit Proximal Phalangeal Heads
CONCLUSIONS: The results of this investigation provide evidence in support of an anatomical and structural contribution to digital deformities. The width of the heads of the lesser digit proximal phalanges decreased from the second to the third to the fourth toes, and then subsequently increased with the fifth proximal phalangeal head.PMID:38170599 | DOI:10.7547/21-211 (Source: Journal of the American Podiatric Medical Association)

Is Subclassification by Number of Fracture Fragments Necessary for Sanders Type IV Calcaneal Fractures?
CONCLUSIONS: Clinical results of Sanders type IV fractures were not related to the number of bone fragments but to the degree of injury to the Böhler angle, Gissane angle, and vertical height. Therefore, subtype classification of type IV calcaneal fractures is superfluous, and it is important to try to restore these parameters during surgery.PMID:38170605 | DOI:10.7547/21-135 (Source: Journal of the American Podiatric Medical Association)

Relationship Between Diabetes-Related Large-Fiber Neuropathy and Dorsiflexion Range of Motion at the Ankle and First Metatarsophalangeal Joints
CONCLUSIONS: These findings suggest that DPN contributes to reduced ankle and first metatarsophalangeal joint range of motion. Due to the established link between reduced ankle and first metatarsophalangeal joint range of motion and risk of diabetes-related foot ulcer, we recommend that clinicians assess dorsiflexion range of motion at these joints as part of routine foot assessment in people with diabetes, especially those with DPN. Globally, approximately 436 million adults aged 20 to 79 years are living with diabetes.1 Diabetes is the leading cause of lower-limb amputation and is associated with a lifetime incidence of ...

Clinicopathologic Characterization of Kaposi Sarcoma on the Foot and Ankle: Analysis of 11 Patients Seen in Our Clinics
CONCLUSIONS: Kaposi sarcoma is still prevalent in both HIV-infected and HIV-uninfected patients with a variety of clinical presentations. Biopsy, with histologic evaluation, in combination with immunohistochemistry is essential for the differential diagnosis. The patient should be treated according their clinical manifestation, staging, comorbidity, and immune function.PMID:38170608 | DOI:10.7547/21-175 (Source: Journal of the American Podiatric Medical Association)

Treatment of Recalcitrant Isolated Congenital Fibular Pseudarthrosis: Fibular Segment Transfer and Tibiofibular Synostosis with Unilateral External Fixator
We report a case of isolated congenital fibular pseudarthrosis with valgus deformity of the ankle. The patient had a history of two failed operations. We used a novel surgical plan that combined tibiofibular synostosis with fibular segment transfer through a unilateral external fixator. The patient showed good early results with fibular union. We advocate the combination of tibiofibular synostosis and fibular segment transfer to restore the integrity and stability of the ankle in recalcitrant isolated congenital fibular pseudarthrosis cases with a history of failed surgery.PMID:38170609 | DOI:10.7547/21-255 (Source: Journa...
