Tumors arising from the ampulla of Vater are entities with a low prevalence and high mortality. Management is surgical and approaches range from open to endoscopic surgery. Due to their risk of mortality and recurrence, radical surgical interventions are preferred, but it has been documented that in selected cases transduodenal ampullectomy is a safe approach for certain ampullary pathologies. Charlson Comorbidity Index Guides Treatment Decisions for HER2 Positive Breast Cancer in Older Women
Woman,over age 70, compose 30% of breast cancer cases. Over expression of HER2 occurs in 15% of cases. NCCN guidelines recommend systemic treatment for tumors over 1 cm. The Charlson Comorbidity Index can quantify multiple comorbidities and predict a limited life expectancy that allows stratification of women over age 70 who may not benefit from systemic treatment of HER2 breast cancer.
