Impact of microvascular invasion on clinical outcomes after curative ‐intent resection for intrahepatic cholangiocarcinoma
BackgroundMicrovascular invasion (MiVI) is a histological feature of intrahepatic cholangiocarcinoma (ICC) that may be associated with biological behavior. We sought to investigate the impact of MiVI on long ‐term survival of patients undergoing curative‐intent resection for ICC.MethodsA total of 1089 patients undergoing curative ‐intent resection for ICC were identified. Data on clinicopathological characteristics, disease‐free survival (DFS), and overall survival (OS) were compared among patients with no vascular invasion (NoVI), MiVI, and macrovascular invasion (MaVI).ResultsA total of 249 (22.9%) patients had M...

Outcome after reconstruction of proximal femoral tumors: A systematic review
Tumor resection followed by reconstruction with a proximal femoral endoprosthesis or an allograft ‐prosthesis composite are the two main alternatives for treatment of proximal femoral malignancies. This review describes the revision rate, implant survival, limb salvage rate, and function. Overall revision rates are high and reasons for failure differ between treatment modalities. Rate and reas ons for amputation are comparable between both methods. Functional outcome was reasonable to good on average for both treatment modalities. Level of evidence: IV, systematic review and meta‐analysis. (Source: Journal of Surgical Oncology)

Predictors of hospital discharge in cancer patients with pericardial effusion undergoing surgical pericardial drainage
ConclusionAlmost half of the patients never left the hospital. Patients with no chemotherapy within 45 days of procedure, patients with renal failure and higher ECOG status are at greater risk of not getting discharged. PHI remains a serious condition with high mortality. (Source: Journal of Surgical Oncology)

Preoperative monocyte ‐to‐lymphocyte ratio predicts recurrence in gastrointestinal stromal tumors
Background and ObjectivesSeveral inflammation markers were found to have a prognostic value in cancer. We investigated the significance of preoperative white cell ratios in determining gastrointestinal stromal tumors (GISTs) outcome.MethodsClinicopathological features of patients who underwent surgery for GIST were reviewed. The following peripheral blood inflammation markers were calculated: neutrophil ‐lymphocyte ratio (NLR), monocyte‐lymphocyte ratio (MLR), platelet‐lymphocyte ratio (PLR), neutrophil‐white blood cell ratio (NWR), lymphocyte‐white cell ratio (LWR), monocyte‐white cell ratio (MWR), and platele...

Primary tumor location in stage III colon cancer has prognostic impact on subsequent liver metastasis
Background and ObjectivesWe aim to investigate whether a difference exists between right ‐sided and left‐sided colon cancer at the same disease stage and subsequent liver metastasis and identify whether tumor location can independently influence survival.MethodsRight ‐sided colon cancer was defined as malignancy arising from the cecum to the transverse colon; left‐sided colon cancer was defined as malignancy arising from the splenic flexure to the sigmoid colon. Clinicopathological features and survival data were collected for analysis.ResultsOverall, 1442 patients were included for analysis. The median follow ‐u...

The interaction between BRAF mutation and microsatellite instability (MSI) status in determining survival outcomes after adjuvant 5FU based chemotherapy in stage III colon cancer
PurposeThe predictive role of biomarkers in colon cancer is still being defined. The aim of this study is to determine the interaction betweenBRAF mutation and microsatellite instability (MSI) status in determining survival benefit after adjuvant 5 ‐FU based chemotherapy in stage III colon cancer.MethodsWe performed a retrospective cohort study including all curatively resected stage III colon cancer cases over a 33 ‐year period. A clinicopathological database was collated (adjuvant chemotherapy, age, gender, obstruction, perforation, tumor location, grade, mucin, nodal stage, extramural vascular, and perineural invasi...

Pre ‐operative predictors of early recurrence/mortality including the role of inflammatory indices in patients undergoing partial hepatectomy for spontaneously ruptured hepatocellular carcinoma
ConclusionsTumor size  >  10 cm, free tumor rupture, and an AFP >  200 ng/mL were useful predictors in avoiding “futile surgery” in patients with srHCC undergoing a partial hepatectomy. Preoperative inflammatory markers appear to be less useful as predictors of early recurrence/mortality in this cohort of patients. (Source: Journal of Surgical Oncology)

The everlasting issue of prolonged air leaks after lobectomy for non ‐small cell lung cancer: A data‐driven prevention planning model in the era of minimally invasive approaches
Background and ObjectivesProlonged air leaks (PAL) are the most frequent complication after lobectomy for non –small cell lung cancer, even in case of minimally invasive approaches. We developed a novel score to identify high‐risk patients for PAL during minimally invasive lobectomy.MethodsA dedicated database was created. We investigated preoperative candidate features and specific intraoperative variables. Univariate and subsequent logistic regression analysis with bootstrap resampling have been used. Model performance has been assessed by reckoning the area under the receiver operating characteristics curve and ...
