Circumferential digit marking to prevent inadvertent wrong site surgery or anesthetic administration
Never event data between April 2021 and March 2022 for NHS England found three incisions and four injections to the wrong finger.1 A survey of 1050 hand surgeons showed 16% of them had prepared the wrong site and 21% of them operated on wrong site surgery at least once.2 In some circumstances, the operating surgeons may not be the same surgeon who marked the surgical site. Following one adverse event of local anesthesia administration into the wrong finger, our aim was to prevent similar errors re-occurring. Flap loss in head and neck reconstruction: is there a singular cause for failure?
This study aims to comprehensively assess the factors influencing flap revision and free tissue transfer survival in head and neck reconstruction.The study included 70 patients with defects in the lower two-thirds of the head and neck who underwent reconstruction using free flaps. Patient age, gender, smoking status, comorbidities and data on the defect's location and aetiology, the specific type of flap employed, the recipient artery and vein chosen, instances of revision, and the overall success of the flap were collected. Reconstruction of Plantar Forefoot in Diabetic Foot Ulcers with microvascular tissue transfer using tunneled pedicle with dorsal anastamosis
Traditionally, the free flaps to the plantar forefoot region were based on the donor vessels of the posterior tibial artery and its branches around the ankle. To access the posterior tibial artery, the whole of the sole needs to be dissected, and this adds to the morbidity of the procedure. Herein, we describe the use of the dorsalis pedis artery as a recipient vessel through trans-metatarsal pedicle route for the microvascular reconstruction of plantar forefoot defects in diabetic foot ulcers. Functional outcomes of reconstructive flap surgery for soft tissue sarcoma: long-term outcomes of functional restoration using innervated muscle transplantation
We examined the functional outcomes of reconstructive flap surgery for soft tissue sarcoma retrospectively. Twenty patients underwent innervated muscle transplantation with pedicled or free flaps for functional reconstruction of resected muscles. Thirteen latissimus dorsi muscles and one vastus lateralis muscle combined with an anterolateral thigh flap were transferred as free flaps using the epi-perineural suture technique. Robotic Harvest of the Free Gracilis Muscle Flap
In this report we describe the robotic harvest of the free gracilis muscle flap in order to reduce the morbidity associated with the open incision. Through three ports, the robotic system (Da Vinci Xi, Intuitive Surgical) enables precise dissection of the gracilis muscle, the vascular pedicle, and the obturator nerve; thus, enhancing surgical control, optimizing visualization through magnification, aiding in detailed vascular pedicle dissection and minimizing human error.
