Enhancing skin preparation to mitigate Cutibacterium acnes infections in breast augmentation and reconstruction
Skin flora pose a significant infection risk in breast reconstruction where the axilla, inframammary fold, and periareolar regions harbor higher bacterial concentrations. Cutibacterium acnes (C. acnes) is of particular concern due to antibiotic resistance and its ability to form biofilms. These factors complicate the treatment for breast implant infections. Current CDC-recommended skin preparation methods, such as alcohol-based antiseptics, fall short against C. acnes. There are many surgical strategies aimed to treat the symptomatic neuroma of the superficial branch of the radial nerve (SBRN). It is still difficult to treat, despite many attempts to reveal a reason for surgical treatment failure. The lateral antebrachial cutaneous nerve (LACN) is known to overlap and communicate with SBRN. The aim of our study is to state the frequency of spreading of LACN fibers into SBRN branches through a microscopic dissection to predict where and how often LACN fibers may be involved in SBRN neuroma. ‘Rosebud’ inset of DIEP flap in delayed breast reconstruction for consistent projection, lateral flow, and ptosis
Here we describe a template of DIEP flap inset that prioritises projection, lateral flow and natural ptosis; key elements of an aesthetically successful delayed breast reconstruction. By not excising the full length of the mastectomy scar, and preserving the scar laterally, we increase the 3-dimensional aesthetic of the breast, moving the final reconstructed breast aesthetic further away from an unintentional 2-dimensional resurfacing.
