HIP, HIPAA, Hooray: Responding to Negative Online Reviews
From home furniture to hotels to healthcare, many of us now rely on online reviews before trying something new.1 Online physician ratings for academic oral and maxillofacial surgeons are generally positive.2 However, there are some patients who may post false, inflammatory, or negative comments.3 The existence of these reviews can have an outsized influence on a provider ’s practice and their reputation given the relatively low volume of online reviews for many oral and maxillofacial surgeons.2 The provider is unable to respond directly in a patient-specific manner to address these negative reviews online due to patient ...

What is the Significance of the Modified Tinel Sign in Lingual Nerve Injuries?
On October 7th, 1915, the French neurologist, Jules Tinel, published an article in “La Presse Medicale” with the title, “Le signe du “Fourmillement” dans les lésions des nerfs périphériques.”1 Translated to English this reads, “The “Tingling” sign in peripheral nerve lesions. In the article, Tinel specifically distinguishes between eliciting pain from tingling on examination. He postulates, “regenerative tingling is not painful; [it is a] vaguely disagreeable sensation which sufferers usually compare to that of electricity. Which factors affect the reduction quality of open reduction internal fixation of mandibular subcondylar fractures?
Open reduction internal fixation (ORIF) of mandibular subcondylar fractures (MSF) involves several variables that could affect decision-making. There is insufficient data regarding factors influencing the outcomes of MSF ORIF. (Source: Journal of Oral and Maxillofacial Surgery)

Symmetrical midfacial growth after pediatric mandibular reconstruction with free fibula flap
Free fibula is the workhorse flap for mandibular reconstruction and is increasingly being used in pediatric patients. However, craniomaxillofacial growth and development involve interdependent processes, and it remains unknown whether mandibular reconstruction with free fibula allows symmetric growth of the midface. Clinical and Histomorphometric Outcomes of Bone Block Grafts Used for Alveolar Bone Regeneration: A Systematic Review
Despite advances in the field of implantology, severe alveolar bone  deficiencies still present a challenge to surgeons. Autogenous bone has long been considered the gold standard and most effective in bone regeneration due to its osteoconductive, osteoinductive, and osteogenic characteristics. Harvesting autogenous bone block grafts is associated with risk of dono r site morbidity, prolonged treatment time, possibility of injury to adjacent structures, and may involve a general anesthetic with hospitalization. Evaluation of the Local and Systemic Effects of Oral Submucosal Ketamine
Few therapeutic alternatives exist for patients with allergies to amide and ester local anesthetics. While diphenhydramine ’s efficacy has been verified experimentally, its use is associated with significantly decreased duration of pulpal anesthesia and increased subjective pain scores during exodontia procedures as compared to prilocaine.1 Additionally, complications such as confusion, diplopia, and skin necrosis hav e been reported. Several investigators have examined ketamine’s use as a postoperative analgesic adjunct when combined with commonly used amide anesthetics during oral surgery procedures. (Source: Journal...

A Comparison of the Effects of EMLA Cream and Topical 5% Lidocaine on Discomfort During Palatal Injection
Local anesthesia of the palatal mucosa is crucial for painless penetration of soft palate tissue during dental procedures. The administration of anesthetic agents to the palate can be very painful due to the region's mucosa being closely attached to the periosteum bone and having multiple peripheral nerves. The palatal mucosa is a commonly used area for local anesthesia and serves as an accurate test for assessing the effectiveness of topical anesthetics. The purpose of this study was to compare the efficacy of EMLA cream and lidocaine ointment in reducing the amount of injection pain experienced by the greater palatine ne...

Assessment and Validation of Preoperative Three-Dimensional Volumetric Analysis to Predict Bone Graft Success in Alveolar Cleft Reconstruction
Cleft lip and palate is the most common craniofacial congenital anomaly in the world, affecting 1 in 700 live births.1 Although alveolar bone grafting (ABG) with autologous iliac crest bone graft (ICBG) remains the standard of care to treat these bony defects, recombinant human bone morphogenetic protein-2 (rhBMP-2) with demineralized bone matrix (DBM) has shown equal graft success compared to ICBG.2 Despite significant advancements in ABG, reported graft success rates vary widely for both types ranging from 60-80% for ICBG and 54-90% for rhBMP-2.
